While dapsone is typically recommended, alternative modalities may be considered based on accessibility, severity, and prior response to therapy. This review aims to provide an updated overview of the clinical characteristics and treatment outcomes for LABD/CBDC. A systematic review was conducted using MEDLINE and Embase in adherence to the Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines. Results from 650 articles, encompassing 1,627 cases, revealed 52% of cases were male. Vesicles (49%) and bullae (47%) were the predominant morphologies occurring across multiple body locations simultaneously, such as the legs (52%), abdomen (49%), and back (49%). Vancomycin was the most reported causative medication; however, most cases were not drug-induced. Dapsone was the primary therapy for LABD/CBDC while biologics were reported with higher complete response rates. Other immunomodulators, such as IVIG and mycophenolate mofetil, were reported to have comparative responses in CBDC cases, but lower response rates in LABD cases. Colchicine and amoxicillin-clavulanate had lower response rates overall. Specifically, lesions can now be depicted at a much higher magnification - up to 400 x - using optical super-high magnification dermoscopy (OSHMD).</p><p><strong>Patients and methods: </strong>This is a retrospective, observational study assessing 99 melanocytic lesions in patients from the University Hospital Heidelberg. Dermoscopy (20 x) and OSHMD images (90 x, 120 x, 150 x, 180 x and 270 x) were acquired. OSHMD images were assessed for the presence/absence of pigment network, distribution, size and color of cells, dots and roundish nests (small/large), structureless areas and vessels in nevi versus melanomas. Correlation studies with histopathology were performed.</p><p><strong>Results: </strong>We found that in OSHMD atypical pigment network, irregular dark dots, atypical vessels and irregular grey out-of-focus cells are clues to melanoma. Black dots and small roundish nests in OSHMD images corresponded to nests of atypical melanocytes in melanomas in histopathology. The patient's journey can be arduous as diagnosis and therapy are challenging.</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Patients and Methods</h3>\u0000 \u0000 <p>In this cross-sectional study, a wide range of characteristics of the patient's journey were assessed and evaluated from a total of 72 patients with mucosal LP who were treated in the dermatology departments of six German university medical centers between 02/2022 and 07/2023.</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Results</h3>\u0000 \u0000 <p>On average, 18.1 months elapsed between the onset of symptoms and diagnosis. Until the correct diagnosis was made, an average of 3.1 different physicians of the same or different specialties were consulted. 28.1% of patients also had cutaneous involvement. Therapeutically, 68% of patients received at least one systemic drug. Both topical (90%, 65/72) and systemic (oral, 50% of patients, 36/72; intravenous, 33%, 24/72) glucocorticoids were most frequently used. Systemic agents were most often discontinued due to ineffectiveness (46%, 50/110). Sadik","doi":"10.1111/ddg.15638","DOIUrl":"10.1111/ddg.15638","url":null,"abstract":"<p>Immune checkpoint receptors and ligands such as cytotoxic T lymphocyte antigen-4 (CTLA-4), programmed death-1 (PD-1) and ligand-1 (PD-L1) are widely expressed on immune and non-immune cells and fine tune the activation level of immune cells, thus, enabling, preventing, or terminating immune responses. Blockade of CTLA-4, PD-1 or PD-L1 by checkpoint inhibitors (CIs), unleashing immune responses, has become a mainstay in the treatment of diverse types of cancer. The induction of autoinflammatory, yet unspecific tissue damage in diverse organs is called an immune related adverse event (irAE), a class side-effect of CIs and may require the discontinuation of immunotherapy. Among frequent skin rashes, CIs targeting the PD-L1/PD-1 axis can elicit the IgG autoantibody- and granulocyte-driven bullous pemphigoid (BP) in about 0.3% to 0.6% of treated patients. Pathogenesis of BP requires a complex cellular inflammatory response after anti-BP180 autoantibody binding to the dermal epidermal junction. The prevalence of autoantibodies against BP180 in healthy blood donors of approximately 0.52% equals the prevalence of irBP among treated cancer patients, underlining the potential relevance of the PD-1 mediated regulation of tissue inflammation for spontaneous BP.
