However, growing evidence over the past decade has revealed that this disease is in fact far more heterogeneous than previously thought - both in its clinical manifestations and in the underlying molecular and immunological mechanisms. This short review explores the evolving conceptual framework used to analyze atopic dermatitis, encompassing clinical presentations (phenotypes), immune polarization profiles (immunotypes), and the background disease-related and context-related factors that shape them. The aim of this review is to highlight how such heterogeneity influences disease severity, progression, and therapeutic response, particularly in the context of increasingly personalized treatment strategies. By synthesizing accumulated scientific and clinical insights, this short review provides clinicians with a practical guide to navigating the complex spectrum of AD and its underlying therapeutic targets. Despite its distinct histopathological features, WS remains an underdiagnosed disease due to its variable clinical presentations and overlap with other dermatological conditions. This study aims to provide a comprehensive overview of WS to enhance diagnostic and management.</p><p><strong>Patients and methods: </strong>A retrospective analysis for the period 2018-2023 was conducted on 48 patients with WS at the Dermatology Unit of the University of Bologna, Italy. Data collected included demographics, clinical manifestations, hematological findings, histopathological characteristics, therapies, and outcomes. Descriptive statistics were used to analyze and summarize the data.</p><p><strong>Results: </strong>The mean onset age was 60.7 years, with 56% of patients having localized WS and 44% generalized WS. The most common subtype was urticaria-like WS (38%), followed by papulonodular, vesiculobullous, granuloma annulare-like, and cellulitis-like forms. First-line treatments included topical and systemic corticosteroids. In contrast, recurrence was observed in 29% of cases during the follow-up.</p><p><strong>Conclusions: </strong>WS is a heterogeneous underdiagnosed condition requiring clinical-histologic correlation for accurate diagnosis. Tailored therapeutic strategies are essential, particularly for generalized or refractory cases. The aim of this study was to characterize cutaneous NTM infections in Germany over a 24-year-period.</p><p><strong>Patients and methods: </strong>73 patients with cutaneous NTM infections diagnosed at 17 different German University Hospitals from 2000-2011 and 21 patients treated at the Department of Dermatology of the University Hospital Cologne, from 2010-2024. Retrospective analysis of patient, demographic, clinical information, and data on different diagnostic workups and therapeutic approaches/responses.</p><p><strong>Results: </strong>Together, 94 cases were analyzed (71% males, mean age 50, > 75% immunocompetent). Exposure to fish tanks was the dominant risk factor. Pathogen detection by nucleic acid amplification test and/or culture was successful in 76%. The most common pathogen was Mycobacterium (M.) marinum (> 65%), followed by M. abscessus/chelonae. Immunosuppression was associated with NTM other than M. marinum. Over 90% were treated with oral antibiotics, in more than 50% with a single antibiotic (mostly clarithromycin). The most common combination was clarithromycin plus rifampicin. The mean duration of therapy was more than 4 months.
