A systematic search conducted on December 11, 2024, identified 97 relevant videos from an initial pool of 545. Videos were analyzed using the DISCERN tool and Global Quality Score to assess reliability and educational quality. Reliable videos, comprising 54.6% of the sample, were significantly more often uploaded by universities, professional organizations, or nonprofit physician groups when compared to non-reliable videos (P = 0.006). When comparing different uploader subgroups, videos from universities, professional organizations, or nonprofit physician groups demonstrated significantly higher DISCERN and Global Quality Scores (P = 0.006, P < 0.001, respectively). In contrast, based on the overall distribution of uploader types among the entire video sample, videos uploaded by individuals or for-profit entities were more frequently classified as non-reliable (P = 0.005). Notably, viewership metrics did not differ significantly between reliable and nonreliable videos (P = 0.552), highlighting the challenge of discerning accurate content based on popularity alone. The findings underscore the importance of promoting high-quality, evidence-based information from credible sources on platforms like YouTube. This review article aims to describe various preoperative, intraoperative and postoperative procedures and factors that may preserve or increase penile length during PP surgery. Preoperative strategies by the surgeon include vacuum erection devices (1 month), penile traction devices (2-4 months), and optimal surgical timing in priapism (early vs. delayed), and patient decisions include choosing a high-volume centre (>25 PP/year) and implant type (malleable vs. inflatable). The intraoperative manoeuvres included the subcoronal no-touch technique, cavernosal tissue-sparing dilatation, ventral phalloplasty, suspensory ligament release, suprapubic lipectomy, the sliding technique, the multiple-slit technique, and tunical expansion procedures. Simultaneous radical prostatectomy and PP surgery is gaining momentum to preserve the penile length in patients who have preexisting ED. Postoperative protocols included immediate postoperative inflation (60-80% inflated for 6 weeks), pump inflation protocol and the use of a vacuum device (without the ring, for 12 weeks). Using the above factors, the gain in penile length may range between 0.2 and 6.0 cm. However, patients with a large body habitus often struggle to manipulate a pump placed in the scrotum. Here, we describe a novel technique for placing the IPP pump in the mons fat pad to improve accessibility. This technique follows the traditional approach for cylinder and reservoir placement, but instead creates a subcutaneous pocket in the mons fat pad overlying the inguinal region, where the IPP pump is secured to the dermis to prevent migration. The tubing was tunneled through the initial incision and connected using the traditional approach. Three patients with a mean BMI of 37.0±5.2 underwent this technique at our institution, including one primary implantation and two revisions due to difficulty accessing a scrotal pump. At follow-up ranging from 4 to 16 months, all devices remained functional, and all patients reported easy pump accessibility. No infections, erosions, or other complications were observed. However, few studies focus on the process of penile detumescence. This pilot study explores the feasibility of measuring the perceived duration of post-ejaculatory penile detumescence in real-life conditions in a convenience sample. Voluntary participants could access a survey between November 2020 and February 2021, with measurement instructions shared by the researchers across several online social networks. Participants measured penile detumescence, starting a stopwatch at ejaculation and recording two time points: onset of flaccidity and complete flaccidity. The survey collected basic demographics for description purposes. At survey closure, full responses of 83 (mean age 26.9 ± 8.9 years) participants were analyzed. The mean duration to onset of flaccidity was 57.9 ± 35.5 s, and to complete flaccidity was 180.7 ± 83.5 s, both following an approximately normal distribution. These durations were significantly correlated with each other (r = 0.47, p < 0.001), but not with any other variables. The results suggest that quantifying penile detumescence in real-life conditions is feasible. This condition often manifests with symptoms such as pneumaturia, fecaluria, and urinary tract infections. Despite its rarity, this condition presents significant clinical management challenges due to the lack of consensus on standardized treatment protocols, particularly in patients with a history of irradiation. This study aims to systematically review the available literature on the efficacy and safety of primary surgical repair of adult-acquired recto-urethral fistulas. We conducted a comprehensive bibliographic search of MEDLINE, Scopus, and Web of Science Core Collection databases in August 2024, following the PRISMA guidelines. A total of 10 studies, covering over 500 patients, were included in our review. The most common surgical approach used was the transperineal technique with gracilis muscle flap interposition. Findings indicate higher complication rates and diminished healing in irradiated patients compared to non-irradiated counterparts. Although transperineal approaches generally resulted in high success rates irradiated individuals frequently required additional surgeries or definitive urinary diversion. This narrative review explores the applications and ethical considerations of AI technologies, including machine learning, deep learning, and natural language processing, in key areas of men's health such as fertility, erectile dysfunction, Peyronie's disease, testosterone deficiency, and premature ejaculation. In reproductive medicine AI improves sperm morphology assessment, embryo selection, and prediction of seminal quality, offering more objective and accurate alternatives to traditional methods. For erectile dysfunction, AI supports diagnosis through advanced imaging, wearable sensors, and predictive modeling, while also contributing to drug discovery. In the context of Peyronie's disease, AI enhances curvature assessment and three-dimensional modeling, improving diagnostic precision and surgical planning. AI also holds promise in improving the evaluation of testosterone deficiency by optimizing symptom questionnaires. Furthermore, AI-powered chatbots are emerging as accessible tools for patient education across various men's health topics, including pre-consultation support for conditions such as premature ejaculation and vasectomy counseling. Despite AI's advancements in men's health, challenges remain in ensuring ethical implementation, protecting data privacy, patient autonomy, and promoting algorithm transparency. Evolving sociocultural values and drastic changes in the legal landscape have sparked important conversations about contraception among Americans. Effective contraception options are limited for men. Vasectomy is a safe, reliable, and affordable form of permanent contraception. Men's reproductive health is often stigmatized, and it is paramount to understand the drivers of this stigmatization, especially as interest in vasectomy appears to be rising. Cultural stigmatization is often reflected online. Social media has become ubiquitous in our culture, with both a broad reach and a strong societal influence. Other domains of reproductive health have seen substantial levels of misinformation online. While it is unclear how the male perception of vasectomy has evolved online over the past few years, the recent growing demand creates a unique avenue for urologists to engage in Men's Health both in the clinic and online. All patients treated with arterial embolization at our center between 2002 and 2017 for high-flow priapism were included. Demographic and clinical data were collected and analyzed. In July 2022, patients were reassessed using specific questions about their satisfaction with the treatment. Erectile function was evaluated using the International Index of Erectile Function-5 (IIEF-5) and the Erection Hardness Score (EHS). Thirteen men, with a median age of 30 years (IQR: 24-37), were included in the study. Superselective arterial embolization using permanent occlusive agents was performed in all cases. The blood flow in the fistula was interrupted leading to complete penile detumescence in all patients. At a median follow-up of 175 months (IQR: 74-197), the median IIEF-5 score was 24 (IQR: 21-25) and the median EHS was 4 (IQR: 3-4), indicating preserved erectile function. Additionally, 92.3% of patients expressed satisfaction with the treatment. Superselective transcatheter arterial embolization with non-absorbable agents demonstrates effectiveness and durability as a treatment for high-flow priapism, with preserved erectile function observed over a follow-up period exceeding 10 years.
