Though they are generally rare, referral bias exists in tertiary institutions. This study was performed to audit the various MST presenting to an oncology Multidisciplinary Team (MDT) in Northeast India.</p><p><strong>Materials and methods: </strong>An oncology MDT was set up in June 2016 at a tertiary hospital in Northeast India, which caters to a wide region comprising different ethnicities. In addition to patients diagnosed with MST, patients with suspected MST were also referred to the MDT. Data was collected prospectively and was analyzed up to March 2020. Our findings were also compared to those of other national and international studies.</p><p><strong>Results: </strong>During the study period, a total of 4704 cases (comprising 3426 patients) were discussed at the MDT meetings of which 174 (5%) were musculoskeletal [bone tumors {n = 110 (63.2%)}, soft tissue {n = 42 (24.2%)}] and skin lesions {n = 22 (12.6%)}]. The malignant among the bone tumors, soft tissue, and skin lesions were 59%, 88%, and 95%, respectively. After excluding secondary bone tumors and multiple myeloma, the total number of patients with primary bone tumor was found to be 88 (80%), out of which both malignant and benign were 50% each. The commonest site and diagnoses among malignant primary bone tumors were femur (15.9%) and osteosarcoma (25%), respectively. The commonest site and diagnosis among benign primary bone tumors were tibia (15.55%) and giant cell tumors (8%), respectively. The majority (56.9%) were male, with a median age of 39 years (range 3-90). The intent of treatment was curative in 71.9% of the malignant lesions.</p><p><strong>Conclusion: </strong>Osteosarcoma was found to be the most common malignancy among MSTs with male predominance. The accurate diagnosis of MST is aided by the MDT. We aimed to assess the pDDIs in In-patient Department (IPD) patients receiving cancer chemotherapy in a tertiary care hospital.</p><p><strong>Methods: </strong>A prospective, observational study was conducted in the oncology department of a tertiary care hospital for a period of 6 months. Patient information was noted in the data collection form, and pDDIs were assessed using the Micromedex® database. Mann-Whitney U, Chi-square, and Spearman's correlation were the tests used for statistical analysis. P value of less than 0.05 was considered statistically significant.</p><p><strong>Results: </strong>In a sample size of 145 patients having a confidence interval of 95% and response distribution of 50%, the margin of error was found to be 8.11%. Male predominance (57.2%) was seen in the study. Although the adult patient population (56.5%) dominated the study, pediatric (26.9%) and geriatric (16.6%) patients were also included. A total of 115 pDDIs were found in 41% of the total patient population, out of which 56% (n = 64) were moderate and 44% (n = 51) were major in severity. The number of drug interactions was found to have correlation with the number of drugs (rho = 0.2, P = 0.01) prescribed during hospital stay.</p><p><strong>Conclusion: </strong>The present study shows that cancer patients are relatively at risk for drug-drug interactions. Stress is a frequent and most commonly neglected complaint of the patient that affects the outcome of therapy. In the present study, we will assess the distress level in cancer patients and evaluate for any correlation between distress score and compliance to treatment.</p><p><strong>Methods and material: </strong>During the primary visit the patient's distress score is assessed using the National Comprehensive Cancer Network (NCCN) distress thermometer (DT) and problem list. And those patients who were illiterate or unable to read or write due to other reasons were questioned and questionnaire filled by an unbiased third party. Patients were followed up until the end of their treatment and the defaulters were noted down.</p><p><strong>Results: </strong>Forty three (19%) patients out of 224 defaulted treatment. Among this 70% had high distress risk but the difference in mean distress scores for defaulters and completed patients were statistically insignificant. {"title":"Guided motor imagery and deep breathing exercises on radiation therapy-induced fatigue in head and neck cancer patients.","authors":"Simmy Thomas, Purusotham Chippala","doi":"10.4103/ijc.ijc_1075_21","DOIUrl":"10.4103/ijc.ijc_1075_21","url":null,"abstract":"<p><strong>Objective: </strong>To compare the effects of guided motor imagery and deep breathing exercises on radiation therapy-induced fatigue in head and neck cancer patients with those of conventional physiotherapy interventions.</p><p><strong>Aims: </strong>To evaluate the impact of guided motor imagery and deep breathing exercises on fatigue caused by radiation therapy in patients with head and neck cancer.</p><p><strong>Setting: </strong>Participants experiencing radiation therapy-induced fatigue were recruited from a tertiary care hospital in South India.</p><p><strong>Design: </strong>The study was conducted as a single-blinded randomized controlled trial.</p><p><strong>Methodology: </strong>A total of forty-two patients participated in the study. They were randomized using the opaque sealed envelope method and assessed by a blinded evaluator. Group A (n = 21) received guided motor imagery, deep breathing exercises, and conventional physiotherapy, while Group B (n = 21) received only conventional physiotherapy. The interventions were administered three times a week over a period of three weeks. The Brock model is a predictive model that shows good accuracy for determining likelihood of malignancy in pulmonary nodules, but both have not been validated and compared in sub-solid pulmonary nodules (SSNs); thus, the purpose of this study was to compare the Brock model and LU-RADS in differentiating malignant from benign SSN.</p><p><strong>Methods: </strong>The clinical data of patients with SSN who underwent surgical resection and were pathologically confirmed from January 2018 to December 2021 were retrospectively analyzed. According to the pathological results, the patients were divided into two groups, the benign SSN group and the malignant SSN group. Their clinical data were then statistically analyzed. The malignant probability of SSN was calculated by the Brock model. The LU-RADS category of SSN was determined by two radiologists independently. The receiver operating characteristic curves of the Brock model and LU-RADS were drawn, and the area under curve (AUC) was compared.</p><p><strong>Results: </strong>A total of 133 patients with SSN were included in this study. LU-RADS categories 4A and 4B of the malignant SSN group were more than the benign SSN group (56 versus 4, P < 0.05); the malignant probability of the malignant SSN group was significantly higher than that of the benign SSN group (0.21 versus 0.06, P < 0.05). The Brock model has a good correlation with LU-RADS (r = 0.75, P < 0.01) and had a comparable performance in diagnosing lung cancer for the SSN (Brock versus LU-RADS, AUC: 0.83 versus 0.78, P = 0.16). Sub-group analysis showed that the Brock model had a higher performance in diagnosing malignancy for the mixed ground-glass nodules than LU-RADS (Brock versus LU-RADS, AUC: 0.92 versus 0.85, P = 0.03), but both had a similar lower performance in the detection of malignancy from the pure ground-glass nodules (Brock versus LU-RADS, AUC: 0.59 versus 0.55, P = 0.66).</p><p><strong>Conclusion: </strong>The Brock model was more suitable for distinguishing malignant from benign mixed ground-glass nodules. Therefore, we conducted a pilot trial as an initial evaluation of our constructed psychological resilience intervention program.</p><p><strong>Materials and methods: </strong>The intervention program was initially evaluated through a quasi-experimental study in which participants were grouped according to time of admission, with the control group receiving usual care and the intervention group receiving a psychological resilience intervention by the intervention team.</p><p><strong>Results: </strong>The pilot study was completed by 31 participants, with a recruitment rate of 73.3%, a completion rate of 93.9%, and a compliance rate of 60%. The intervention was well-received by all participants. Furthermore, in terms of the intervention's effectiveness, the higher hospe of the intervention group (N = 15) significantly increased (t = -3.682, P = 0.019). Although a remarkable increase in resilience (t = -2.584, P = 0.019) was observed, the difference was not obvious (t = -1.280, P = 0.211).</p><p><strong>Conclusion: </strong>The intervention program for psychological resilience in patients with stomach cancer during chemotherapy was successful. This study presents two case reports of perianal paget's disease treated with surgery in a tertiary care oncologic center in India. Case 1: A 72 year old man presented with an itchy white patch in the perianal region, with a biopsy done in 2015 revealing extra-mammary paget's disease. The patient reported to us in November 2018, following which he was reevaluated and subsequently underwent wide local excision, diversion loop colostomy, and bilateral medial thigh perforator fasciocutaneous flaps reconstruction. Final histopathological examination confirmed diagnosis of Paget's disease. Case 2: A 69 year old man presented with recurrent perianal paget's disease after surgical excision and radiotherapy in 2016. The patient underwent wide local excision, diversion colostomy, bilateral sentinel lymph node biopsy, left ilioinguinal block dissection, and reconstruction with multiple medial thigh fasciocutaneous flaps in December 2019. Discussions: Perianal paget's disease is a rare cutaneous malignancy. Although studies regarding effective treatments are lacking, surgical excision is considered the best modality. Wide local excision with or without reconstruction remains the treatment proven to be successful, regardless of which long term follow up is crucial to identify recurrence. We evaluated associations among maximum standardized uptake value (SUVmax), metabolic tumor volume (MTV), total lesion glycolysis (TLG), and clinicopathological findings by dividing the specimen into three tumor subtypes according to estrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor (HER-2) expressions.</p><p><strong>Results: </strong>There was a significant difference between the three groups in the SUVmax value of axillary lymph node metastasis (P = 0.014), but no significant difference was revealed for TLG and MTV (P = 0.858 and P = 0.083, respectively). This study reports and analyzes the impact of the peer review rounds of a modern radiotherapy department during the first 4 years.</p><p><strong>Subjects and methods: </strong>Prospectively documented data for 183 peer review sessions consisting of 3725 patients from June 2017 to May 2021 were obtained. The proportion of cases that required major (unacceptable and requires an immediate correction before delivery) and minor plan changes (not ideal, but still allowable for delivery) were analyzed on a site-specific basis. Pareto charts were plotted for each site to demonstrate the reasons for plan changes.</p><p><strong>Results: </strong>Among the cases reviewed, 197 plans (5.29%) required any plan modifications; among these, 111 (2.98%) were major changes, while 86 (2.31%) cases required minor changes. The major changes suggested had decreased from 5.84% to 1.58% over time. The reasons for plan changes were site dependent. Frequent reasons for major plan change were modification in clinical target volume (63.06%), followed by a change in dose or fractionation (24.32%).
