Here, we report two cases of PPP, one of which shows cytokeratin (CK) positivity, along with a literature review. In the first case, a 76-year-old male patient presented with a right-lung peribronchial mass, while in the second case, a 53-year-old male, presented with a right middle lobe mass. In both cases, the guided biopsy revealed a similar morphology comprising a tumor with polygonal cells in a back-to-back nested pattern, showing strong immunoexpression with synaptophysin and chromogranin. In the first case, a heterogenous immunoexpression of CK was observed, which was an unusual finding. In small biopsy specimens, PPPs with an aberrant CK expression can be misdiagnosed as pulmonary carcinoids or non-small cell lung carcinoma, which have distinct prognostic and therapeutic implications. Both cases underwent lobectomy, and the findings were confirmed on the resection specimen. The study aimed to assess the concerns of patients with advanced cancers and their caregivers on the day of referral to the palliative care clinic.</p><p><strong>Methods: </strong>A survey was conducted on 140 subjects of advanced malignancies attending the palliative care clinic. These included 70 patients along with their caregivers. The patients were assessed for their psychosocial concerns and their information needs on their first visit to the palliative care clinic.</p><p><strong>Results: </strong>The concerns of both the patients and the caregivers were separately documented. Pain followed by fatigue and insomnia were the most bothersome symptoms reported by the patients. While most of the patients (91.4%) wanted symptom relief, the patient's life expectancy was a major concern of the caregivers. The other common concerns of the patients were the disease status and curability of the disease whereas the caregivers were more concerned about the alternative treatment measures. Of the psychosocial problems, the majority of them were related to financial concerns followed by settlement of children and loneliness.</p><p><strong>Conclusion: </strong>A difference between the needs of patients and their caregivers was observed in the present study. This may be attributed to less focus on the psychosocial problems in the busy outpatient clinics. {"title":"Diagnostic values of the monocyte to hemoglobin ratio and neutrophil-lymphocyte ratio to albumin ratio in non-small cell lung cancer.","authors":"Si-Ting Zhao, Fen-Hong Qian","doi":"10.4103/ijc.ijc_241_23","DOIUrl":"10.4103/ijc.ijc_241_23","url":null,"abstract":"<p><strong>Objective: </strong>To evaluate the diagnostic utilities of monocyte to hemoglobin ratio (MHR) and neutrophil-lymphocyte ratio to albumin ratio (NLRAR) in the detection of non-small cell lung cancer (NSCLC).</p><p><strong>Methods: </strong>We retrospectively collected and analyzed the clinical and laboratory data, including 196 NSCLC cases and 204 controls.</p><p><strong>Results: </strong>The area under the curve (AUC) for 95% confidence interval (95% CI) of MHR and NLRAR in the diagnosis of NSCLC were found to be 0.822 (0.781-0.864) and 0.712 (0.662-0.762), respectively. The combined use of MHR, NLRAR, and carcinoma embryonic antigen (CEA) exhibited higher diagnostic sensitivity (AUC: 0.855; 95% CI: 0.818-0.892; sensitivity: 70.4%; specificity: 89.2%) than the use of individual markers or two markers combined. MHR was a potential biomarker to detect early-stage NSCLC.</p><p><strong>Conclusions: </strong>MHR and NLRAR are effective auxiliary biomarkers to predict the presence of NSCLC. Although newer drugs, advanced surgical and radiation techniques have revolutionized the treatment of rectal cancers, cure rates are still less because of the frequent locoregional and distant failures.</p><p><strong>Materials and methods: </strong>This ambispective study was conducted at a university teaching and tertiary care referral hospital. The data of the patients were collected from the patient records without the active participation of the patients. The data were collected with regard to demographics, clinical presentation, treatment received, and patterns of failure.</p><p><strong>Results: </strong>Rural patients were in majority, Male patients were dominant in number, Married patients were more compared to unmarried patients, Smokers outnumbered never-smokers, Mean age at presentation was 52 years, Neoadjuvant treatment was received by 22% of patients, Curative surgical resection was done in 62% of patients, Palliative treatment was received by 25% of patients. No recurrence was seen in 43.01% (n=151) of patients, while locoregional recurrence, distant recurrence (metastasis) or both were reported in 20.80% (n=73), 10% (n=35) and 8.53% (n=30) respectively. However, primary pure ovarian angiosarcoma is even rarer with a few cases published in the literature. Here, we present a case of a perimenopausal woman who presented with a 16 cm adnexal mass. She underwent bilateral salpingo-oophorectomy with omentectomy, which on histopathological examination revealed an ovarian angiosarcoma with tumor deposits in the pelvic peritoneum. No other component was demonstrable within the tumor, in light of which it was reported as a primary pure ovarian angiosarcoma. She received adjuvant paclitaxel- and carboplatin-based chemotherapy after discussion in a multispecialty tumor board. {"title":"IPD regimen effect on the levels of VEGF and IL-6 in elderly patients with recurrent multiple myeloma.","authors":"Huiying Zhao, Shengyu Ma, Wan Li","doi":"10.4103/ijc.ijc_51_24","DOIUrl":"10.4103/ijc.ijc_51_24","url":null,"abstract":"<p><strong>Background: </strong>To investigate IPD regimen effect on vascular endothelial growth factor (VEGF) and interleukin-6 (IL-6) levels in elderly patients with recurrent multiple myeloma (MM).</p><p><strong>Methods: </strong>Eighty-two elderly patients with relapsed multiple myeloma in our hospital from January 2019 to December 2021 were selected and randomly divided into TD group and IPD group, 41 cases in each group. The TD group was treated with thalidomide + dexamethasone, while the IPD group was treated with ixazomib + pomalidomide + dexamethasone. The treatment effect and the changes of VEGF and IL-6, TNF-α, levels before treatment, 6 months, and 12 months after treatment were compared by the log-rank test. The Kaplan-Meier method determined the progression-free survival (PFS) of patients.</p><p><strong>Results: </strong>The clinical efficacy distribution of IPD was better than that of the TD group (Z = 2.407, P = 0.016). No significant difference was showed in serum VEGF and IL-6, TNF-α, concentrations before treatment (T0) (P > 0.05). The serum concentrations of VEGF and IL-6, TNF-α, in the IPD group were lower at 6 months and 12 months after treatment. The incidence of progression in the IPD group was 9.76% (4/41), and the estimated time of no progression was 11.73 ± 0.21 months. The incidence of progression was 19.51% (8/41) in the TD group, and the estimated time without progression was 10.95 ± 0.38 months, showing no significant difference (χ2 = 1.718, P = 0.190).</p><p><strong>Conclusion: </strong>IPD regimen for relapsed elderly multiple myeloma has the characteristics of improving clinical efficacy and inhibiting VEGF and IL-6, TNF-α, concentrations increase. There is still no consensus on whether breast-conserving surgery or mastectomy should be performed after NAC and there is no certain point about how the axilla should be approached. These problems stem from concerns about survival.</p><p><strong>Methods: </strong>We retrospectively evaluated the 10-year survival results of 251 patients who either had breast-conserving surgery (BCS) or mastectomy after NAC.</p><p><strong>Results: </strong>Better loco-regional control was achieved when pathologic axillary node positivity was below %50. Best disease-free (DFS) and overall survival (OS) results were obtained when the breast had a complete response after NAC regardless of axillary response. Data regarding the outcome in children from developing countries are scarce. We analyzed the clinical characteristics and outcome of pediatric SS.</p><p><strong>Materials and methods: </strong>Children ≤14 years of age, diagnosed to have SS between January 1, 2007 and December 31, 2017 at a tertiary cancer center in India, were retrospectively analyzed. Details regarding clinical presentation and treatment were recorded, and the outcome was analyzed using Kaplan-Meier survival analysis.</p><p><strong>Results: </strong>Fourteen children were diagnosed with SS. The median age was 132 months (range 54-168 months), and the male:female ratio was 1.8:1. Group I, II, III, and IV disease was seen in four (28.6%), six (42.9%), three (21.4%), and one (7.1%) child, respectively. Extremity was the most common site of primary (n = 13, 92.8% children). Tumor size was ≤5 cm in nine (64.3%) children. The treatment modalities were surgery only (n = 2, 14.3%), surgery and radiation (n = 1, 7.1%), surgery and chemotherapy (n = 5, 35.7%), and surgery, radiation, and chemotherapy (n = 6, 42.9%). At a median follow-up of 62.5 months (range 18-125 months), the 4-year event-free survival (EFS) and the overall survival were 77.7% and 85.7%, respectively. The 4-year EFS for children with smaller tumors (≤5 cm) was 77.8%, compared to 60% for children with larger tumors. The 4-year EFS for male was 53.3%, compared to 100% for female patients.</p><p><strong>Conclusion: </strong>Our study demonstrates that good survival can be achieved in children with SS in low- and middle-income countries (LMICs) by appropriate utilization of the available resources judiciously.
