This has been a major challenge in patients with acquired immunodeficiency. However, the ethical aspects of protecting individual rights of other people related to the patient are very important. Therefore, in this study, a case has been presented with the ethical challenge of protecting the confidentiality of a patient with acquired immunodeficiency in the hospital. Case Report: The patient was 44 years old, admitted to the urological ward of hospital because of kidney stones. The patient had already diagnosed with HIV and hepatitis B. The became dismayed and disconcerted by the fact that other people in the hospital knew his disease and by failing to keep the secrets by the custodian, and after his confrontation, without treatment and crushing surgery, he was taken off the hospital with satisfaction. Conclusion: The ethical challenge we face in this study is to keep confidentiality of the patient, which is of autonomous principles and very important, and on the other hand, because of the fatalities of these diseases, informing and maintaining the safety of the other treatment teams is also very necessary. In individuals without the qualifications for making decision, this decision is made by an alternate, and in cases where decisions affect the lives of others, participatory decision making occurs. But if there is a conflict of interest among the people in the current research, a moral challenge arises that the present report deals with in this regard. Patient introduction: A 34-year-old pregnant woman, and the first delivery, refers to the hospital with the onset of her first labor pain. The doctor considers cesarean section essential for the health of the fetus because there will be a risk of fetal loss, given the clinical condition of the mother giving birth. On the other hand, because of the past bitter experience of the complications of cesarean section for her sister, who after the cesarean section still has been suffering from severe headache and back pain, she is afraid of cesarean section and is not satisfied with cesarean section. Conclusion: In this case, at the time of making a decision, attending the hospital and arranging for documentation, the prosecutor's representative decides to perform the caesarean section. On the one hand, the challenge has been referred to the Hospital Ethics Committee for review to determine who makes the decision in these cases. The aim of this study was to introduce the boy twins who turned blind due to retinopathy resulting from child prematurity and parents’ insufficient education. Case Report: A 20-year-old mother, experiencing her first pregnancy, which was of twins type, and with complaints from rupture of membranes and high blood pressure, referred to a birth center and delivered. The result of this vaginal birth was two boys that were 28 weeks old and went through ten days of oxygen therapy, due to preterm delivery, and then were discharged. Two months after the birth to twins, the mother noticed their lack of response to light and after going to the doctor, retinopathy due to prematurity was diagnosed. Babies did not respond to treatment and both turned blind. The twins’ mother considers the lack of her awareness and negligence of the care team regarding post-clearance education and lack of follow up as the reasons for the delay in her referring to the hospital.
