This has been a major challenge in patients with acquired immunodeficiency. However, the ethical aspects of protecting individual rights of other people related to the patient are very important. Therefore, in this study, a case has been presented with the ethical challenge of protecting the confidentiality of a patient with acquired immunodeficiency in the hospital. Case Report: The patient was 44 years old, admitted to the urological ward of hospital because of kidney stones. The patient had already diagnosed with HIV and hepatitis B. The became dismayed and disconcerted by the fact that other people in the hospital knew his disease and by failing to keep the secrets by the custodian, and after his confrontation, without treatment and crushing surgery, he was taken off the hospital with satisfaction. Conclusion: The ethical challenge we face in this study is to keep confidentiality of the patient, which is of autonomous principles and very important, and on the other hand, because of the fatalities of these diseases, informing and maintaining the safety of the other treatment teams is also very necessary.
