In individuals without the qualifications for making decision, this decision is made by an alternate, and in cases where decisions affect the lives of others, participatory decision making occurs. But if there is a conflict of interest among the people in the current research, a moral challenge arises that the present report deals with in this regard. Patient introduction: A 34-year-old pregnant woman, and the first delivery, refers to the hospital with the onset of her first labor pain. The doctor considers cesarean section essential for the health of the fetus because there will be a risk of fetal loss, given the clinical condition of the mother giving birth. On the other hand, because of the past bitter experience of the complications of cesarean section for her sister, who after the cesarean section still has been suffering from severe headache and back pain, she is afraid of cesarean section and is not satisfied with cesarean section. Conclusion: In this case, at the time of making a decision, attending the hospital and arranging for documentation, the prosecutor's representative decides to perform the caesarean section. On the one hand, the challenge has been referred to the Hospital Ethics Committee for review to determine who makes the decision in these cases.
