{"title":"Hypoglycemia Refractory in Patient with Metastatic Gastrointestinal Stromal Tumor (GIST): Case Report and Literature Review","authors":"M. Camandaroba","doi":"10.31031/GMR.2021.05.000619","DOIUrl":"https://doi.org/10.31031/GMR.2021.05.000619","url":null,"abstract":"A case of NICTH secondary to gastrointestinal stromal tumor (GIST) is described, male patient admitted to the emergency referring asthenia and drowsiness, and measured blood glucose of 20mg/dL. Patient continued to present episodes of recurrent hypoglycemia: 33/55/32mg/dL, even with continuous reposition of 10% glucose solution. Hypoglycemia can be related to several tumors, including islet cell and non-islet cell tumors. Non-islet cell tumor hypoglycemia (NICTH) is a rare and serious complication of malignancy, and its presence is usually associated with a poor prognosis. The most common cause of this type of hypoglycemia is tumor overproduction of insulin-like growth factor II (IGF-II), which stimulates the insulin receptor, leading to decreased insulin and glucagon levels. The diagnosis of NICTH is based on clinical and laboratory findings. Patients with NICTH have low serum concentrations of insulin and C-peptide during hypoglycemia. The treatment of this condition is related to the control of the underlying neoplasia. In patients who are unable to undergo invasive procedures, the use of corticosteroids is a good treatment option, leading to normalization of glycemia. Its management is of unquestionable importance to ensure the patient’s quality of life. Different factors involved in pathogenesis like environmental, genetic, and sedentary life. Recent studies demonstrates that Vitamin D3 deficiency has a growing role in pathogenesis of MS. Many studies had addressed the relationship between serum Vitamin D3 and metabolic syndrome but to the best of our knowledge, few studies about this relationship in Egyptian population. Aims: To assess the relationship between deficiency of Vitamin D3 with metabolic syndrome in Egyptian population. Patients and methods: This is cross sectional study invite 200 participants from healthy clubs but only 180 accept to participate. According to the presence or absence of metabolic syndrome, our participant were divided into two groups. All participants were subjected to detailed medical history, clinical and anthropometric assessment, and laboratory analysis including serum Vitamin D3 level. Result: The mean age of the studied patients was 40.91±8.72, and majority were female (66.7%). Metabolic syndrome cases were significantly higher as regard the age, SBP, DBP, TG, FBS, WC and BMI but significantly lower regard HDL and Vitamin D3, and no significant association with sex. Vitamin D deficiency cases were significantly higher as regard Age, SBP, DBP, TG, FBS, WC and BMI but sig lower regard HDL. There was significant different between patients with and without Vitamin D3 deficiency as regard the presence of metabolic syndrome (P value=.00). There was moderate negative correlation between Vitamin D3 deficiency and age, SBP, DBP, FBS, WC and BMI with positive weak correlation with HDL level. Vitamin D3 deficiency < 20ng/ml and BMI >35 were significant predictors for metabolic syndrome with (P value 0.00, 0.04) respectively. {"title":"Role of Biofield Energy Healing Treatment Based Test Formulation on Colon Cytokines Using TNBS-Induced Ulcerative Colitis in Sprague Dawley Rats","authors":"M. Trivedi, S. Each ingredient of the test formulation was divided into two parts, one part was defined as control and the other part was described group. The level of TNF-α was significantly reduced by 12.2%, 30% ( p< 0.001), 59.1% ( p <0.001), 49.2%, and 15.5% ( p< 0.001) in the G5, G6, G7, G8, and G9 groups, respectively as compared with the G2 group. Further, the level of IFN-γ was significantly decreased by 15.6%, 40.5% ( p< 0.001), 62.1% ( p <0.001), 52.6% ( p< 0.001), and 28.4% by G5, G6, G7, G8, and G9 respectively, as compared with the G2. Besides, IL-10 level was significantly increased by 31.7% ( p <0.001), 14.3%, and 18.7% ( p <0.05) in the G5, G6, and G9 groups, respectively as compared with the G2. Thus, it can be concluded that the Trivedi Effect®-Consciousness Energy Healing significantly modulates the colon cytokines after treatment with the test formulation and Biofield Energy Treatment per se . Up to 20% of patients with rectal cancer have metastatic patients rectal cancer was localized in the rectosigmoid part, in 4 patients in the proximal part of the rectum, in 2 patients with metastasis, rectal cancer was localized in the mid rectum and in 4 patients rectal cancer was in distal part of the rectum. Rectal cancer is spread malignant disease worldwide and it is third common malignancy after breast and lung cancer in woman and prostatic and lung cancer in male population. Occurrence of metastatic dissemination when primary rectal cancer has been diagnosed is not so rare condition but is very important to be aware of the factors that are associated with metastatic dissemination of rectal cancer. Sister Mary Joseph nodule or Sister Mary Joseph Sign refers to a palpable nodule bulging into the umbilicus as a result of metastasis of cancer in the abdomen or the pelvis. It is a rare occurrence, but it is a sign of abdominal cancer, most commonly an adenocarcinoma metastasis from a gastrointestinal (gastric, colonic, pancreatic cancer) or gynecologic primary malignancy. We present the case of a 61-year-old man with an acute, something less than 1cm in diameter non-tender mass located in the umbilicus, diagnostic indication of an SMJN. This patient, 4 months before was diagnosed with pancreatic carcinoma and underwent a Whipple procedure with pathophysiology result of-Adenocarcinoma capitis pancreatic pTNM= pT2 pN1 pMX. This little nodal change in the umbilicus was an indication for further investigation so a computer tomography (CT) scan was done. Younger age(less than 65 years) female gender, advanced Tand N-stage, a primary tumor of signet ring cells or linitis plastica, and primary tumors covering multiple anatomical locations of the stomach were all associated with a higher odds ratios of developing PC. PC is a frequent condition in patients presenting with gastric cancer, especially in younger patients with advanced tumor stages. In this article, we present a case of a 63 years old woman who was operatively treated for gastric cancer. Total gastrectomy was obtained and the patohistology report was signet ring cell carcinoma, stage T3N3M0. Computer tomography (CT) was done before the operative treatment and 6 months after the treatment. The CT before the treatment did not show any presents of peritoneal metastasis, there were no signs of peritoneal involvement. The next CT was done 6 months after the treatment where there was a sign of peritoneal involvement, with the diffuse spread of metastatic deposits (MS) all over the peritoneum. The present study showed a positive correlation between a more advanced T-stage and N-stage and the risk of presenting with PC. In literature, these factors were also associated with worse survival. Objective:The target of this study was to measure the association between 25-hydroxyvitamin D, systemic inflammation and endothelial function biomarkers in patients with type 2 diabetic nephropathy. Material and Methods: Two hundred Saudi obese type 2 diabetes mellitus (T2DM) patients (114 males and 86 females), their body mass index (BMI) was 31-35Kg/m2and the chronicity of diabetes was 11.87±2.95 year enrolled in the present study. Smokers and patients with renal insufficiency, congestive heart failure, pregnancy, respiratory failure and hepatitis were excluded. Participants were enrolled into three equal groups: group (A) 25-OHD<20ng/ml (deficiency of vitamin D), group (B): 25-OHD=20-30 ng/ml (insufficiency of vitamin D) and group(C) 25-OHD >30ng/ml (normal vitamin D). Consent from was signed by participants. Results: Mean values of VCAM-1, ICAM-1, E-selectin, TNF-α, IL-6 and CRP were significantly greater in group(A) compared to group(B) and group(C). However, vitamin D showed a strong inverse relationship with these parameters in the three groups (P<0.05). Herein, we report an interesting case of a 69-years-old male with renal cell carcinoma with pulmonary metastases and a synchronous localized colon cancer, who achieved significant responses to both cancers after first-line treatment with pazopanib and second-line treatment with axitinib. We reported a 37-year-old female patient newly diagnosed as endocrine-dependent breast cancer with synchronous the diffuse thickness of stomach and extensive metastases in the peritoneum as well as bilateral ovaries. The endoscopic view demonstrated a sign of linitis plastic. Histopathological results from gastric biopsy showed molecular features in favor of breast origin, which could be the major source of antidiastole.
