The company's ability to deal with developments in the globalization era, needs to strive to continuously improve quality and competitiveness in meeting customer requirements. Quality is one of the keys to winning the competition, to be able to survive in business competition both in the national market and in the global market the company must be able to improve the quality of its products so as to achieve customer satisfaction. One way that companies can do is by implementing QMS. QMS is a series of interacting processes carried out by a company to achieve quality objectives. CV HER-5 Engineering Manufacturing is a company engaged in manufacturing, with the resulting product being a swing arm. Observations at the company found that the company had implemented QMS based on ISO 9001:2008, but not up to the certification stage. In addition, the company experienced problems in the form of product returns by consumers. This is because the resulting product does not comply with the approved requirements. This study aims to determine the current condition of the QMS CV HER-5 Engineering Manufacturing, and provide suggestions for the fulfillment and design of QMS documents in accordance with the requirements of ISO 9001:2015. This study used ISO 9001:2015 as the QMS standard and the gap analysis method was carried out by scoring clauses 4 to 10 of ISO 9001:2015. The percentage of implementation results obtained was 50,15%, and the proposed design of complete QMS documents based on ISO 9001:2015 includes quality guidelines, procedures and forms to fulfill clauses that have not been fulfilled. \u0000Keywords: Quality Management System, ISO 9001:2015, Gap Analysis. \u0000Abstrak. Kemampuan perusahaan dalam menghadapi perkembangan era globalisasi, perlu berupaya untuk terus-menerus meningkatkan mutu dan daya saing dalam memenuhi persyaratan pelanggan. Mutu merupakan salah satu kunci dalam memenangkan persaingan, untuk dapat bertahan dalam persaingan bisnis baik di pasar nasional maupun pasar global perusahaan harus mampu meningkatkan mutu produknya sehingga tercapai kepuasan pelanggan. Salah satu cara yang dapat dilakukan perusahaan yaitu dengan menerapkan SMM. SMM merupakan rangkaian proses yang saling berinteraksi dilakukan oleh perusahaan untuk mencapai sasaran mutu. CV HER-5 Engineering Manufacturing merupakan perusahaan yang bergerak di bidang manufaktur, dengan produk yang dihasilkan yaitu lengan ayun (swing arm). Hasil pengamatan pada perusahaan diketahui bahwa perusahaan telah menerapkan SMM berdasarkan ISO 9001:2008, namun tidak sampai tahap sertifikasi. The Cibuntu tofu industry is a research site located in Babakan Ciparay Village, Bandung Regency which is an association of Micro, Small and Medium Enterprises (MSMEs) that produce tofu. Cibuntu Tofu Industry has about 16 small-scale to industrial-scale craftsmen in the village. Four entities play a role in the tofu factory supply chain network: suppliers, tofu production houses, retailers, and consumers. Suppliers have two categories involved in this supply chain: soybean suppliers, which provide the main raw materials for tofu production, and firewood providers, which provide fuel for cooking. The two suppliers will talk about the supply of raw materials to the factory after reviewing the demand of tofu manufacturers. Soybean suppliers receive their products from a variety of different sources with firewood providers. Performance design is carried out using the GreenSCOR model, by weighting performance indicators using the AHP method using Expert Choice software. Based on the results of the assessment carried out, the overall performance value of the green supply chain of the Cibuntu tofu industry is in the Good category. However, the company must further improve some performance indicators that are still considered unsatisfactory. The results of the performance indicator assessment there are 12 KPIs that are still below the target, where improvements must be made in order to improve the company's performance by providing recommendations for improvements to performance indicators that are still below the assessment standard. \u0000Abstrak. Industri tahu Cibuntu menjadi tempat penelitian yang terletak di Kelurahan Babakan Ciparay, Kabupaten Bandung yang merupakan suatu perkumpulan Usaha Mikro, Kecil dan  Menengah (UMKM) yang memproduksi tahu. Industri Tahu Cibuntu memiliki sekitar 16 pengrajin skala kecil hingga skala industri di desa tersebut. Empat entitas berperan dalam jaringan rantai pasokan pabrik tahu: pemasok, rumah produksi tahu, pengecer, dan konsumen. Pemasok memiliki dua kategori yang terlibat dalam rantai pasokan ini: pemasok kedelai, yang menyediakan bahan baku utama untuk produksi tahu, dan penyedia kayu bakar, yang menyediakan bahan bakar untuk memasak. PT. Langgeng Sentosa Garmindo is a company that produces apparel with one of its products being polo shirts. The problem that is being faced by the company is related to product quality, it is known that during quality inspections product defects are often found so that it needs to be reworked. Quality problems are reinforced by data on the number of product defects that still exceed the tolerance for defects by 1%. Product quality problems can be identified by the causes of the problems with the New Seven Tools method and the proposals with 5W+2H. The identification results obtained are for the sewing part which is the focus of repair with the causes of defects including lack of supervision from supervisiory, absence of periodic machine maintenance schedules, lack of knowledge about thread requirements, operators pursuing high wages, absence of work SOP reminders, absence of special operator training, unused items not being immediately disposed of and the company's climate is hot. The results of the proposed improvement of the 5W+2H method include making machine maintenance and cleaning schedules, making checksheets to carry out machine maintenance activities, giving instructions to supervisors to carry out strict supervision, increasing understanding of work instructions and providing motivation, making displays, making samples of calculating yarn requirements, making operator training schedules, making checksheets for machine conditions and disposing of or moving damaged machines, as well as cleaning building ventilation and adding temperature control devices in the form of exhaust fans. \u0000Keywords: New Seven Tools, Product Quality, 5W+2H. \u0000Abstrak. Masalah yang sedang dihadapi oleh perusahaan berkaitan dengan kualitas produk, hal tersebut diketahui ketika pemeriksaan kualitas sering dijumpai kecacatan produk sehingga perlu dilakukan rework. Permasalahan kualitas dikuatkan dengan data jumlah kecacatan produk yang masih melebihi toleransi kecacatan sebesar 1%. Permasalahan kualitas produk dapat diidentifikasi penyebab-penyebab masalahnya dengan metode New Seven Tools serta usulannya dengan 5W+2H. PT Dhanar Mas Concern I is a company engaged in spinning cotton into yarn. Producing quality thread products is not easy, defects are often found during production. There are 6% -7% of defective products from total production, as well as waiting activities which are a waste. This research is expected to provide suggestions for improvements related to wastage of defective products and other waste. The lean six sigma approach is used to solve the problems found. The tools used include value stream mapping, 7 waste, process cycle efficiency (PCE), DPMO and sigma level, fishbone, 5W+1H, and visual control. The calculation results show a PCE value of 69% which can be said that it is still not maximally efficient. Meanwhile, the determination of the sigma level during June–July 2021 is 2.28, so it still needs to be increased to a standard sigma level of 3.4. The recommendations suggested are scheduling machine maintenance, conducting routine training for operators, making work standards in visual form, providing industrial portable evaporative air coolers, changing batch transfers and designing conveyors from the ring frame work station to the winding work station. Consultations were carried out with the Head of Production regarding recommendations for improvements, which could increase the PCE value from 69% to 84% and the sigma level from 2.28 to 2.48 which affected the productivity and quality of the yarn produced.. \u0000Keywords: lean six sigma, six sigma, process cycle efficiency, level sigma. \u0000Abstrak. PT Dhanar Mas Concern I merupakan perusahaan yang bergerak di bidang pemintalan kapas menjadi benang. Menghasilkan produk benang yang berkualitas bukanlah hal yang mudah, sering kali ditemukan produk cacat selama produksi. Terdapat 6%-7% produk cacat dari total produksi, serta aktivitas menunggu yang merupakan pemborosan. Penelitian ini diharapan dapat memberikan usulan perbaikan terkait pemborosan produk cacat dan pemborosan lainnya. Pendekatan lean six sigma digunakan untuk menyelesaikan permasalahan yang ditemukan. Adapun tools yang digunakan, diantaranya value stream mapping, 7 waste, process cycle efficiency (PCE), DPMO dan level sigma, fishbone, 5W+1H, dan visual control. Hasil perhitungan menunjukan nilai PCE sebesar 69% yang dapat dikatakan bahwa masih belum efisien dengan maksimal. Sementara penentuan level sigma selama bulan Juni–Juli 2021 sebesar 2.28, sehingga masih perlu ditingkatkan hingga standar level sigma sebesar 3.4. Riyadlul Ulum Integrated Senior High School is an integrated boarding school-based school, by combining the State Education curriculum, Salafi Islamic Boarding School curriculum and Gontor Islamic Boarding School curriculum. One of the school administrators who really need the use of information technology is the administrator in the field of KMI (Kulliyatul Muallimiatul Islamiyah) or can be called the academic field at this school. This field is responsible for everything that happens in the KBM process. One form of service provided by the academic field is licensing. Licensing is a service for teachers and students who cannot take KBM. Licensing services have several stages in the application process. Licensing is divided into 2 categories, namely activity permits and sick permits. Kirana Semesta Pangan is a company engaged in fast food with production results, namely meatballs, the company has 15 work stations where each machine is not fully automated, material transfer is carried out in the meatball production process, namely with the help of manual conveyances such as walky pallets and racks push made of stainless steel. The problems faced by the company are the length of the transfer path between work stations which causes high material handling costs and there are work stations that are not in accordance with the process, causing backtracking. The purpose of this research is to analyze and find out the current conditions of the production floor layout used by the company, as well as to fix the layout problems mathematically to reduce material movement distances, eliminate backtracking in the production process and reduce material handling costs in the company. In making layout improvements according to the problem, the tools used are the Computerized Relative Allocation of Facillities Technique (CRAFT) algorithm and the software used is WinQSB 2.0. The input needed in the WinQSB 2.0 software is OMH data from each work station. Calculation of material handling costs is done manually. Based on the calculations performed on the current layout conditions, a total daily OMH of Rp. 961,127 and a displacement of 197.1m per day, after repairs were made, a new layout output was obtained by obtaining an OMH of Rp. 756,488/day and a distance of 163.9m/day. So it can be concluded that the proposed layout has a much cheaper OMH acquisition of 21% and a shorter distance of about 17% from the previous layout conditions \u0000Abstrak. PT. Kirana Semesta Pangan merupakan perusahaan yang bergerak dibidang makanan cepat saji dengan hasil produksi yaitu bakso, perusahaan memiliki 15 stasiun kerja yang masing-masing mesin belum sepenuhnya otomatis, perpindahan material yang dilakukan dalam proses produksi bakso yaitu dengan bantuan alat angkut manual seperti walky pallet dan rak dorong berbahan stainless steel. Adapun permasalahan yang dihadapi oleh perusahaan adalah panjangnya jalur perpindahan antar stasiun kerja yang menyebabkan ongkos material handling tinggi dan tedapat stasiun kerja yang letaknya tidak sesuai proses sehingga menyebabkan backtracking. Tujuan dari penelitian ini yaitu untuk menganalisa dan mengetahui kondisi tata letak lantai produksi yang diterapkan perusahaan saat ini, serta memperbaiki permasalahan tata letak tersebut secara matematis untuk mengurangi jarak perpindahan material, menghilangkan backtracking pada proses produksi dan mengurangi ongkos material handling di perusahaan. Dalam melakukan perbaikan tata letak sesuai dengan permasalahan, tools yang digunakan adalah algoritma Computerized Rellative Allocation of Facillities Technique (CRAFT) dan perangkat lunak yang digunakan adalah WinQSB 2.0. UMKM Ngablue is UMKM engaged in clothing convection including shirts, screen printing shirts, polo shirts, vests and jackets. complaints from customers in terms of product quality produced by UMKM will affect the level of customer confidence itself. Complaints obtained from customers include the results of cut materials that do not match the pattern, which is around 18.37%, screen printing images that do not match 24.06%, dirty shirts caused by screen printing ink 23.27%, dirty shirts caused by table glue 19.68% and untidy sewing results 21.10% . The total percentage of defects obtained is about 1.04% of the total produced 169,325 pcs. Therefore the Fault Tree Analysis (FTA) method is used as an effort to identify the causes of failure, Failure Mode and Effect Analysis (FMEA) to determine priority repairs based on the RPN value. This company implements make to order in the order process. The orders come to the company from requests outside the city of Bandung with an increasing number. The increasing number of productions found several phenomena found include uneven prints, uneven cuts. Looking at the phenomenon that occurs in the company, it can be concluded that the company is experiencing quality problems. Quality issues are related to materials, costs, labor and company time. Identifying problems that occur using the six-sigma method, which is a method of identifying problems in a structured and systematic manner using statistical tools. Measuring the quality control conditions that occur to get a dpmo value of 12743 and a sigma value of 3.71. Determining the largest type of disability using a pareto diagram obtained the largest defect type is uneven printing, uneven cutting of this type of defect will be carried out the repair process. The use of the control-p map found that five data were outside the control limit caused by special causes so that improvements needed to be made. Looking for the root cause of the biggest types of defects using fishbone diagrams is caused by decreased work concentration, lack of discipline, absence of method documentation, different operator skills, inappropriate materials, stuck water rolls, leaking ink tubs. Musculoskeletal Disorders (MSDs) affect tissues throughout the body including the nerves and tendon sheaths, and sometimes involve the arms and back. Repetitive motion and vigorous exertion are risk factors for upper extreme disorders such as heavy lifting, bending and twisting. CV HER-5 Engineering Manufacturing is engaged in the automotive industry, with the products produced namely motor body spare parts. The spare part product currently being worked on by the company is the swing arm. The results of initial observations, obtained information about production costs increasing because there are many defective products. This is because defective products must be reprocessed. The most defects are in the wrong hole size. The hole size is not correct because the position of the component to be processed does not match the position of the drill bit. During the work, there is an unnatural operator working position. This is because the operator sits on a round bench with a diameter of 35 cm, a bench height of 15 cm and a diameter of 27 cm. In addition, operator space is limited because there is no place to store components. Based on the existing problems, this study aims to identify work risks. The Workplace Ergonomics Risk Assessment (WERA) measurement method is used for work risk assessment. The final risk assessment score shows a value between 34 – 42. The final score is in the range of medium or moderate risk levels. This indicates that the work requires further investigation and requires improvement. CV HER-5 Engineering Manufacturing bergerak di bidang industri otomotif, dengan produk yang dihasilkan yaitu spare part body motor. Produk spare part yang saat ini sedang dikerjakan oleh perusahaan adalah swing arm. Hasil pengamatan awal, diperoleh informasi mengenai biaya produksi semakin meningkat karena banyak produk cacat. Meraya Garment is a company engaged in the Garment industry located in Katapang, Bandung Regency. Based on the results of observations at Meraya Garment, the total loss resulting from these defects reached 5.58% in one year, the pants produced had a fairly high number of defects. The defect at Meraya Garment currently exceeds the tolerance limit previously set by the company, namely 3% of the total production of between 500 and 3000 pants. The total number of defective products in 2021 is quite a lot, reaching a total of 852 pcs. The method used in this problem is the Failure Mode and Effect Analysis (FMEA) method and the Fault Tree Analysis (FTA) method. Based on the results of FTA assistance, it shows that the root causes of defects in pants products at Meraya Garment can be caused by several factors, namely humans, machines and the environment. The results of FMEA calculations improve the processes that have the highest RPN values ​​as an effort to reduce the occurrence of failures. The potential size failure mode caused by the absence of training for workers gets the highest RPN value of 225, which is the main focus of improvement. Whereas the lowest RPN value for skip stitches with the potential for lack of maintenance and oil causes the machine to jam and causes the jump stitches to get an RPN value of 48. Berdasarkan hasil observasi di Meraya Garment, total kerugian yang diakibatkan dari kecacatan tersebut mecapai 5,58% dalam satu tahun, produk celana yang dihasilkan memiliki jumlah cacat cukup tinggi. Kecacatan di Meraya Garment saat ini melampaui batas toleransi yang ditetapkan perusahaan sebelumnya yaitu 3% dari total produksi antara 500 hingga 3000 produk celana. Total produk cacat pada tahun 2021 cukup banyak yaitu mencapai total 852 pcs. Oleh karena itu jika produk cacat tersebut terus dibiarkan maka akan berdampak pada kerugian perusahaan yang cukup besar.
