Navakumar, Vidyalekshmy R, Brinda Sabu, Roshini Ambat, Sajitlh Mohan, Lumiya Malik, Suresh Chandran
Introduction: Myasthenia Gravis (MG) is an autoimmune neuromuscular disorder affecting skeletal\u0000muscles, particularly in women of reproductive age. Though uncommon with a prevalence of only 0.3 to\u00002.8 per 100,000, the incidence peaks during 2nd and 3rd decades of life, and can have signicant impact in pregnancy. The\u0000course of the disease in pregnancy is not entirely understood. Methodology: A retrospective case series analysis of 4 cases\u0000were carried out for a period of 5 years from 2018 in a tertiary care centre in South India to identify and describe the disease\u0000course in pregnancy and effect of disease in the pregnant women and their fetuses. From this study, we aimed to describe the\u0000disease course of MG in pregnancy and formulate an optimal management plan, learning from our experience. Results: All\u0000patients included in the study were in a state of remission prior to conception. With vigilant monitoring and collaborative\u0000multidisciplinary approach, two patients successfully carried their pregnancies to term without encountering any\u0000complications, maintaining a stable course throughout the antenatal, intrapartum, and postpartum periods. However, one\u0000patient experienced an acute exacerbation upon contracting COVID-19 category C. Subsequently, her pregnancy was\u0000complicated by late-onset fetal growth restriction, preterm premature rupture of membranes (PPROM), preterm birth and\u0000postpartum exacerbation. Despite these challenges, both maternal and fetal outcomes were favorable, attributed to\u0000meticulous surveillance and critical care interventions. Another patient experienced an exacerbation at 10 weeks of gestation\u0000but otherwise had an uneventful pregnancy, labour, and postpartum recovery. Conclusion: One third of Myasthenia gravis\u0000patients can have exacerbations in pregnancy with a small but signicant subset experiencing potentially life threatening\u0000Myasthenic crisis. All necessary precautions should be pro-actively taken to prevent factors that could trigger a myasthenic\u0000crisis, including cautious drug selection during anaesthesia and the avoidance of medications that may exacerbate MG during\u0000acute infections. Caesarean section should be for obstetric indications only. Managing myasthenia gravis in pregnancy is\u0000quite challenging and requires multidisciplinary input with close monitoring and follow up.

It is an enormous\u0000economic and social burden, with high morbidity and mortality. The DECAF score, a simple tool regularly employed by\u0000clinicians, is a predictive measure for in-hospital mortality and can be conveniently applied at the bedside for risk stratication\u0000during Acute Exacerbations of COPD (AECOPD). Material And Methods: A cross-sectional observational study was\u0000conducted involving 100 patients, aged between 40 and 82 years, with a mean age of 60, who presented to the OP and IP of the\u0000Department of Respiratory Medicine. Participants who met the inclusion criteria with a diagnosis of COPD were evaluated and\u0000their data was collected. Subsequently, a DECAF score was assigned to each patient. The study then observed in-hospital\u0000mortality and conducted a comparative analysis of the characteristics between survivors and non-survivors. Results: The study\u0000included 100 conrmed cases of COPD, ranging from 40 to 82 years old, with a mean age of 60.66 years. A male predominance\u0000was noted, as there were 77 males and 23 females in the study. An increase in the DECAF score corresponded to a higher\u0000utilization of ventilation. In the low-risk group (DECAF 0-1), only one patient required mechanical ventilation, while in the\u0000intermediate-risk group, 1 (6.25%) patient and in the high-risk group, 14 (87.5%) patients required mechanical ventilation.\u0000Conclusion: In patients admitted with acute exacerbation of COPD, the DECAF score is a benecial clinical tool for in-hospital\u0000prognosis. Assessing the DECAF score after admission due to AECOPD aids in decisions regarding early escalation of care,\u0000therapeutic interventions, evaluating the necessity for ICU admissions, determination of the need for assisted ventilation,\u0000decision-making in end-of-life care, and provides valuable information to clinicians for informing patients and relatives about\u0000life-threatening risks associated with exacerbation.

Etiology and pathogenesis, clinical\u0000manifestations, diagnostic strategies, and treatment modalities are extensively discussed. The etiology of SVTs and\u0000preexcitation involves accessory pathways, and their manifestation varies from asymptomatic cases to life-threatening\u0000arrhythmias. Diagnosis relies on electrocardiography, electrophysiological studies, and imaging. Ablation is the preferred\u0000treatment for symptomatic cases, with catheter-based procedures demonstrating efcacy. Prognosis is generally favorable,\u0000with low recurrence rates post-ablation. In conclusion, this narrative review emphasizes the signicance of tailored therapeutic\u0000approaches and the need for vigilant follow-up in SVTs and preexcitation syndromes.
