In seinem Eröffnungsvortrag beleuchtete Wim Gijselaers (Universität Maastricht, Department of Educational Research and Development) die interprofessionelle Kommunikation in der Medizin. Werden in autoritär geführten Teams also weniger Fehler gemacht?

Because the number of unoccupied positions for doctors is increasing and more and more young doctors are turning their backs on Germany, being family-friendly is more important than ever. This is because when making career decisions, highly qualified medical students do not only decide based on salary and career opportunities but increasingly pay attention to whether their prospective employer is considerate of their family responsibilities. \u0000 \u0000In Germany conditions are needed which enable physicians to perform well in their profession and to take responsibilities within the family. The expansion of childcare and child benefits are important steps in doing so. With the corporate program "Erfolgsfaktor Familie” (success factor family), the Federal Ministry for Family Affairs supports employers in implementing family-conscious personnel policies. The numerous changes and the restructuring of the healthcare system and the increasing economisation of patient care in particular contribute to this view. The challenge today is to ensure patient-centred, world-class patient care, innovative research and teaching and structured education and training at the highest level against the backdrop of increasing output volume (higher life expectancy, multi-morbidity, innovation) and new ethical and legal issues. How can the personal aspects of the physician be reconciled with such requirements? \u0000 \u0000An important step in increasing the attractiveness of the medical profession and to combat the skills shortage is the definition and establishment of modern, family-friendly working patterns. Specifically by offering structured education and training to junior doctors. \u0000 \u0000Examples: \u0000 \u0000 \u0000The adaptation of the daily work (hospital/research/teaching) to the changing ways of life (“work-life balance”) of the next generation of the medical profession, resulting in an optimisation process in clinical practice. \u0000 \u0000 \u0000Shifting activities which do not have to be carried out by doctors to trained non-medical personnel. \u0000 \u0000 \u0000Establishing family-friendly structures, such as flexible work schedules and a motivating work atmosphere. \u0000 \u0000 \u0000Promotion of a variety of career designs - science/clinic/research with time for research, time for engagement in teaching, time for clinical specialisation, time for the acquisition of management and communication skills, establishing mentoring programs in education and training, ensuring transparency and planability. \u0000 \u0000 \u0000To increase attractiveness of the medical profession it must be ensured that female doctors progress their career (percentage of female doctors against all doctors in Germany in 1991 was 33.6%; in 2008 41.5%; working in hospital in 2008: 48.1%; the percentage of female medical graduates in 2008 was 58% (Federal Statistical Office), the percentage of female undergraduate students in 2007 was 70%). \u0000 \u0000 \u0000 \u0000 \u0000 \u0000 \u0000Competing interests \u0000The author declares that she has no competing interests.

In the competition for the best minds at our universities and university hospitals, a family-friendly HR policy is key, especially in medicine. More and more well-trained doctors are lost, especially specialist surgeons, to other European countries. Therefore Germany urgently needs to create more family-friendly medical workplaces. This needs to start in medical school and needs to continue during specialisation. \u0000 \u0000For this reason we are promoting the project “Family-friendly medical studies in Baden-Wurttemberg" at the University of Ulm. In addition, the Association for Student Affairs supports students through childcare facilities, family housing and counselling centres at the universities. \u0000 \u0000The fact that 39% of employed physicians have not specialised is unsatisfactory. With a share of 58% of graduates, specialist training in all specialisations should be more easily reconcilable with family responsibilities in order to safeguard medical care in Germany in the coming years.

Family-friendly working conditions as an advantage in the competition for the next generation of doctors
Family-friendly working conditions as an advantage in the competition for the next generation of doctors
Despite a constant number of medical students and graduations, the exodus from the curative medicine has led to a shortage of physicians. The reasons for this - in addition to work intensification which is frequently coupled with long and often unpaid working hours, excessive bureaucracy and increasing economising - can be also found in the changing expectations and roles of young men and women who have a stronger desire for leisure and time for their families. The growing proportion of female doctors reinforces this tendency, as they are more likely to work part-time or to work limited hours or not be available at all to the workplace when pregnant or with young children. Having children usually falls into the period of professional development, so part-time employment or cessation of employment leads to longer training periods or on occasion to a complete withdrawal from a woman’s career. The under-representation of women in executive positions may also be down to the fact that when there is the double burden of work and family, career is often put on hold regardless of potential earlier plans and desires. This is likely to be the most common at university hospitals where, in addition to an already labour intensive workload beyond normal working hours in patient care, additional engagement is expected in research and teaching. And a career predominantly means postdoctoral degrees and appointments via research. \u0000 \u0000The outlined development has caused calls for “family-friendly hospitals,” with better conditions for reconciling family and working life and with more and more regular leisure time. That aside, such working conditions would also represent an advantage in the competition for the next generation of doctors. Medical Practitioner Associations have raised this issue repeatedly in various campaigns and at events in the recent past. The handbook "Familienfreundlicher Arbeitsplatz fur Arztinnen und Arzte – Lebensqualitat in der Berufsausubung ” published by A Buhren, A E Schoeller (Federal Medical Association 2010), this issue is looked at in more detail. \u0000 \u0000The regulations on CPD attempt to recognise the desire for greater flexibility, especially for doctors of either gender with families, by recognising part-time work and CPD units with breaks, while bearing in mind the limitations caused by the need for quality.

As a result of the changing perception of family roles (both parents working, men playing a bigger part in childcare), the demand for appropriate support is growing. \u0000 \u0000Compared to other federal states, Baden-Wurttemberg is lagging behind in the provision of childcare facilities and needs to catch up and seriously expand capacities. This first of all requires investment and despite the ban of cooperation the federal government could do more, especially for medical schools in the universities. There is also, however, the issue of operating costs, which university hospitals cannot carry on their own. Public investment so far needs to be stepped up. \u0000 \u0000In terms of distribution, employee representation has a strong legal position. All practical experience suggests that they tend to lobby for quotas according to social aspects. This sometimes leads to other professional groups relevant to university hospitals and couples being disadvantaged. \u0000 \u0000So the necessary measures are clear: more state and federal money for investment and operating childcare facilities but also staff representation laws of Baden-Wurttemberg could also do with an update to make them more family-friendly.
