Urolithiasis is a polyetiological, polypathognomonic metabolic disease characterized by the formation of kidney stones. The study of trends in the urolithiasis prevalence is of fundamental importance in practical medicine. The study of endemic territories allows us to analyze all possible risk factors for the development of urolithiasis from genetic to nutritional. The aim of our study is a comparative analysis of the urolithiasis prevalence in the Russian Federation for different periods. Materials and methods. We analyzed urolithiasis prevalence and incidence in the entire population of Russia from 2005 to 2020. The work is based on official statistics of the Ministry of Health of the Russian Federation. Statistical analysis of the data was carried out using the SPSS Statistics 21 software package. Results and discussion. Urolithiasis prevalence in the Russian Federation is considered in detail from 2005 to 2020. We did a comparative analysis of the data on the general urolithiasis prevalence, the incidence of urolithiasis, urolithiasis prevalence separately in the adult and children's population of Russia in the period from 2005 to 2020 out. We gave the leading regions in the Russian Federation in terms of the number of urolithiasis cases. We spent correlation analysis of the urolithiasis incidence with the number of cases of diabetes mellitus type 2, obesity and the level of meat consumption in the Russian population. Conclusion. Urolithiasis incidence and prevalence among the adult population is steadily growing in all regions of the Russian Federation, while the incidence among children remains stable. Vesicovaginal fistula (VVF) is a pathological communication between the urinary bladder and the vagina. The lack of common approaches to the treatment choice, its timing and postoperative management of patients makes this disorder one of the challenges of modern urogynecology. Materials and methods. Relevant publications were searched using the PubMed database, the Google Scholar search system, and the scientific electronic library eLibrary.ru. Results. VVF is the most common form of urogenital fistulas. In developed countries, the formation of VVF mainly are the result of surgical trauma or radiation therapy, but in developing ones the majority of VVF have an obstetric etiology. There are different classifications of VVF; however, there is no overall accepted one that is suitable for predicting treatment efficacy and long-term outcomes regardless of etiology of VVF. Treatment of VVF with long-term bladder drainage should be performed only for small, benign and unformed VVF. Surgical closure of VVF is the primary method of treatment. Surgery can be performed using abdominal and vaginal access. Regardless of the access, a positive result (closure of the fistula) on average is achieved in 90,8% of cases. The treatment of post-radiation fistulas is more challenged and associated with a high failure rate. Conclusion. In this literature review we discuss the etiology, risk factors, and features of erectile dysfunction management in men with liver disease and cirrhosis. Materials and methods. A search to identify original articles and reviews in PubMed, еLibrary, ScienceDirect, Scopus, EMBASE databases suitable for the purposes of this review was conducted by combining the following terms: «hepatitis», «liver disease», «cirrhosis», «erectile dysfunction», «sexual dysfunction» in Russian and English. Results. Clinically significant erectile dysfunction in liver disease can be detected in more than half of patients with cirrhosis, but usually elude the attention of doctors and researchers. Although erectile dysfunction is not a life-threatening disorder, its impact on quality of life is important. Patients with cirrhosis often suffer from comorbidities that are also known to be involved in the etiopathogenesis of sexual dysfunction. While treating ED in cirrhotic patients is a clinical challenge, almost all studies are epidemiological or assess risk factors. Conclusions. The variety of symptoms and the significant deterioration in the quality of life need a multidisciplinary approach and timely intervention. The relationship between chronic abacterial prostatitis/chronic pelvic pain syndrome (CP/CPPS) and male infertility is the subject of numerous studies, and therefore various methods for correcting pathospermia have been proposed. The main concept is that in one way or another it is necessary to create conditions for a more perfect penetration of drugs into the zone of the prostate-vesicular complex. In this regard, the search for new and pathogenetically substantiated methods of therapy for such a patient seems to be very relevant Aim of study. To evaluate the effect of hypercapnic hypoxia on reproductive and copulatory functions of men in the complex treatment of CP/CPPS. Materials and methods. The study involved 24 patients who applied to a urologist with complaints about the absence of conception in marriage for 1 year or more. The examination revealed pathospermia and symptoms of CP/CPPS in these patients. Assessment of the severity of symptoms of CP/CPPS was assessed using a questionnaire on the symptoms of chronic prostatitis using the NIH-CPSI questionnaire. In order to identify and monitor violations of the male copulatory function, ICF questionnaires created by O.B. Loran and A.S. Segal and the International Index of Erectile Function (MIEF-5). Reproductive function analysis was monitored using a semen analysis and sperm DNA fragmentation index test. The patients were divided into 2 groups of 12 people. Both groups received phenotypically directed pathogenetic and symptomatic therapy. For patients of the 2nd group, the complex treatment included breathing training procedures using the carbonic simulator, which ensures blood saturation with CO2, leading to the effect of hypercapnic hypoxia. This phenomenon contributes to angiogenesis and improvement of blood circulation in organs and systems. The effectiveness of the treatment was evaluated after 1 and 3 months. Results. According to the results of the NIH-CPSI questionnaire, 3 months after treatment, therapy in patients of group number 2 was more effective. According to the results of the MIEF 5 and ICF questionnaires in patients with CP/CPPS, a violation of erectile and ejaculatory function was revealed, and according to the results of the spermogram and the sperm DNA fragmentation test (IFD), pathospermia was noted, which was expressed in a decrease in the concentration of spermatozoa (group number 1 – 15.3 ± 1.6 million / ml, group number 2 – 16.8±2.2 million/ml), their progressive mobility (group number 1 – 24 ± 2.6%, and in group number 2 23.25 ± 2.9%.), normal morphology (group number 1 – 2.1 ± 0.17%, group number 2 – 2.5 ± 0.29%) and an increase in IFD (group number 1 – 17.9 ± 2.06%, group number 2 – 19.75± 1.9%). 3 months after treatment, the concentration of spermatozoa was 21.1±2.16 million/ml in group number 1, and 31.8±2.5 million/ml in group number 2. Progressive mobility in group number 1 – 31.4±2.2%, in group number 2 – 38.4±1.9%. Despite the frequent occurrence of balanoposthitis, due to the wide variety of pathogens, it is often difficult to find an effective therapy. Purpose. To evaluate the extent of changes in complaints during therapy by the complex drug Balanex and its effectiveness. Materials and methods. The study was conducted as a prospective comparative cohort. The study included 120 patients with typical complaints for balanoposthitis. Patients were randomized according to chronic diseases and balanoposthitis therapy. The first group included 59 men with nonspecific balanoposthitis, for whom Balanex cream gel was prescribed topically 2 times a day as a therapy. The study observation group consisted of 32 men with balanoposthitis on the background of diabetes mellitus, for whom therapy Balanex cream gel was prescribed topically 2 times a day. The control group included 29 men with nonspecific balanoposthitis, for whom therapy by a combined cream (antibacterial + antifungal component) was used topically 2 times a day. Results and discussion. In all patients, the results of repeated microbiological examination were negative. The time of disappearance of the patient's disturbing symptoms ranged from 2 to 7 days, on average it was 3.2 ± 1.1 days, the median value was 3 days. At the same time, the presence of diabetes mellitus in the patient significantly prolonged the period of disappearance of symptoms to 3.9 days. The average value of this indicator was 3.0±0.6 vs. 3.9±1.6 vs. 3.0±0.8 days (p<0,001). Conclusions. Currently, ureteroscopy has become the method of choice for the treatment of patients with ureter stone location in lower third and middle third. Regardless of the fact that ureteroscopy is an endoscopic, minimally invasive intervention, it is associated with intraoperative complications. Purpose. The main objective of this literature review is to objectify the existing scales of ureter injury, demonstrate evolutionary changes from simple to more detailed, evaluate their advantages and disadvantages, and select the optimal and reproducible tool for assessing postureteroscopic complications. Materials and methods. In this literature review, we used research materials on existing scales of ureter injury published in the databases PubMed, the scientific electronic library of Russia (eLibrary), Scopus, EMBASE, websites of professional associations. Such keywords were used for searching: «postureteroscopic complications», «intraoperative complications of ureteroscopy», «ureteral injuries». This review focuses on the currently existing scales that assess ureteral injury, which can be universal and used in everyday practice. After a detailed check of the reliability of sources, the impact factors of journals and the sequence of presentation of the material, 28 sources were selected directly for citation. Results. According to literature review, historical and present-day knowledge of using postureteroscopic lesion scales in various surgical centers are presented here. Most of scales are focus on disruption of anatomical continuity of the ureter or on complications that develop during the implementation of surgical treatment, which are not specific for this procedure. However, the most detailed description of postureteroscopic complications makes it possible to distinguish between «complicated» and «uncomplicated» ureteroscopy. Conclusion. Thus, PULS scoring system is a standardized and easily reproducible tool in the evaluation of postureteroscopic complications. In modern reproductive medicine, increasingly attention is paid to pathospermia caused by the formation of oxidative stress, one of the causes of which is varicocele, leading to the formation of hyperthermia and testicular ischemia. The excess of reactive oxygen species, free radicals in the ejaculate during oxidative stress can provoke damage to spermatozoa at the subcellular level, leading not only to DNA fragmentation, but also damage to the acrosome, mitochondria, and increased apoptosis. The purpose of the study: to assess the ultrastructure of spermatozoa in adolescents, to identify changes in spermatozoa characteristic of varicocele. Material and methods. Electron microscopic examination of spermatozoa was performed on adolescents aged 17 years; 100 adolescents were diagnosed with left-sided varicocele; 30 teenagers without varicocele made up the comparison group. Results and discussion. In adolescence, the typical ultrastructure of spermatozoa is most often detected. In adolescents with varicocele, damage to chromatin, acrosome, mitochonria was more often detected, but no statistically significant differences were found compared to adolescents without varicocele. Conclusions. Simple nephrectomy is used for non-cancer diseases. That is, when the preservation of the kidney is unreasonable or dangerous. Since simple nephrectomy is a typical organ-delivering surgery, it is possible to optimize the perioperative period to reduce hospitalization and disability. The aim of the study was to compare the outcomes of simple laparoscopic nephrectomy using the enchanted recovery protocol and the standard approach. Materials and methods. A prospective randomized multicenter study included 27 patients with an established diagnosis of a non-functioning kidney, who were treated in the period 2018–2021 in urological hospitals in the city of Irkutsk. All patients were divided into two comparison groups: STANDART group (group I, n=14) and ENCHANCED RECOVERY group (group II, n=13). Results. Primary efficacy was comparable (p=0.892). It was found that the level of complications of class III according to Clavien-Dindo was statistically comparable in both groups (p>0.05), and the level of complications of class II was significantly higher in group I (p<0.05). When comparing the duration of hospitalization, there is a significantly (p <0.001) longer stay of patients in group I (9.5±2.2 days) compared to group II (1.0±0.0 days). Subjective satisfaction with the treatment performed (upon requesting a critical analysis of all possible complaints) was comparable and amounted to 57.1% for group I and 100% for group II (p=0.342). When conducting a logistic regression analysis for postoperative febrile condition, according to multivariate regression, the volume of intraoperative infusion was found to be a significant predictor (for each +1 ml; OR 0.003; 95% CI 0.0001; 0.005; p = 0.042); for postoperative pain syndrome >5 VAS points – increase in the size of the surgical approach per 1 mm (OR 0.14; 95% CI -0.01; 0.307; p = 0.077); for satisfaction with treatment, a negative effect of the duration of the postoperative stay was found (for each +1 day; OR -1.08; 95% CI -2.32; 0.15; p = 0.086). Conclusion. The problem of the pandemic caused by the COVID-19 virus is extremely relevant. It has been proven that SARS-CoV-2, which causes respiratory distress syndrome, is not only aggressive towards lung tissue, as was originally assumed. A promising direction in the study of the pathogenic properties of the pathogen is the effect on the male reproductive system. Materials and methods. The review authors searched the Pubmed and eLibrary databases for the keywords «COVID-19», 2 SARS-CoV-2», «male fertility», «reproductive health», «sperm», «erectile function», «erection». A total of 122 publications were found, of which 34 were selected for this review. Results. According to a review of scientific publications, coronavirus infection can negatively affect the reproductive system of men through a combination of ACE-2 receptors and cellular transmembrane serine protease 2 (TMPRSS2), due to the development of oxidative stress in testicular tissue, hyperthermia, secondary cytokine storm syndrome, side effects of medications taken, which leads to damage to the testicular tissue, a decrease in the quantity and quality of spermatozoa, and the development of various forms of pathospermia. The cascade of pathological processes in the body of a man, developing under the influence of the COVID-19 virus, leads to damage to the endothelium of the penile vessels, a decrease in nitric oxide level in the cavernous bodies, which is manifested by a decrease in erectile function. Conclusions. Due to the short duration of the studies performed, the heterogeneity of the observed groups of patients, and a limited sample, it is impossible to make final predictions of the severity of reproductive and sexual disorders after a new coronavirus infection, their relationship with the characteristics of the course of the disease and the degree of reversibility of pathological processes. The article discusses the results of studies and meta-analyses that have studied the risks of coronary heart disease (CHD), myocardial infarction (MI) and stroke in patients with urinary stone disease (USD). The purpose of this review is to critically analyze the available studies on the association of USD with CHD and stroke, as well as to evaluate USD among other cardiovascular disease (CVD) risk factors and to assess the role of USD metaphylaxis in reducing this risk. Materials and methods. The authors conducted a search in international and domestic specialized databases of scientific publications, 657 publications were found. For this review, 75 articles were selected that were published no later than April 1, 2021. Results. Despite conflicting research results and high statistical heterogeneity, all meta-analyses have shown that patients with USD have an increased risk of coronary artery disease, MI, and stroke by about 20-40%. In comparison with other risk factors for CVD, USD represents a moderate risk, which dictates not only the need to identify and correct «high» risk factors in patients with USD, but also the need for effective metaphylaxis. There are no studies to support the efficacy of USD metaphylaxis in reducing the risk of CVD. However, it can be assumed that effective metaphylaxis, especially in young patients, using remote technologies, will not only reduce the risk of urolithiasis recurrence, but also help correct metabolic disorders that trigger pathophysiological processes common with CVD. The possibility that simple and inexpensive interventions that reduce the risk of recurrent urolithiasis may also reduce the risk of MI and stroke, the most common cause of death and disability, warrants further study. Conclusions.
