{"title":"Advances in Osteosarcoma.","authors":"Isidora Panez-Toro, Javier Muñoz-García, Jorge W Vargas-Franco, Axelle Renodon-Cornière, Marie-Françoise Heymann, Frédéric Lézot, Dominique Heymann","doi":"10.1007/s11914-023-00803-9","DOIUrl":"https://doi.org/10.1007/s11914-023-00803-9","url":null,"abstract":"<p><strong>Purpose of review: </strong>This article gives a brief overview of the most recent developments in osteosarcoma treatment, including targeting of signaling pathways, immune checkpoint inhibitors, drug delivery strategies as single or combined approaches, and the identification of new therapeutic targets to face this highly heterogeneous disease.</p><p><strong>Recent findings: </strong>Osteosarcoma is one of the most common primary malignant bone tumors in children and young adults, with a high risk of bone and lung metastases and a 5-year survival rate around 70% in the absence of metastases and 30% if metastases are detected at the time of diagnosis. Despite the novel advances in neoadjuvant chemotherapy, the effective treatment for osteosarcoma has not improved in the last 4 decades. The emergence of immunotherapy has transformed the paradigm of treatment, focusing therapeutic strategies on the potential of immune checkpoint inhibitors. However, the most recent clinical trials show a slight improvement over the conventional polychemotherapy scheme. Using existing studies, we compare the prevalence of osteopenia and osteoporosis in men and women from sub-Saharan Africa to US and UK cohorts. Context-specific disparities in healthcare are discussed particularly the challenges in diagnosis and treatment of osteoporosis.</p><p><strong>Recent findings: </strong>There are few epidemiological data describing the burden of osteoporosis in sub-Saharan Africa. In the studies and cohorts presented here, osteoporosis prevalence varies by sex, country and area of residence, but is generally higher in African populations, than has previously been appreciated. Risk factors contributing to poorer bone health include HIV, malnutrition and \"inflammaging.\" Reprioritization towards care of ageing populations is urgently required. In this paper, we critically review applications of rigid image registration in terms of similarity measurement methods over the past three years (2019-2022) in the context of monitoring longitudinal changes to bone microstructure and mechanical properties using computed tomography. This review identifies critical assumptions and trade-offs underlying different similarity measurement methods used in image registration and demonstrates the effect of using different similarity measures on registration outcomes.</p><p><strong>Recent findings: </strong>Image registration has been used in recent studies for: correcting positional shifts between longitudinal scans to quantify changes to bone microstructural and mechanical properties over time, developing registration-based workflows for longitudinal assessment of bone properties in pre-clinical and clinical studies, and developing and validating registration techniques for longitudinal studies. In evaluating the recent literature, it was found that the assumptions at the root of different similarity measures used in rigid image registration are not always confirmed and reported. Each similarity measurement has its advantages and disadvantages, as well as underlying assumptions. Breaking these assumptions can lead to poor and inaccurate registration results. Thus, care must be taken with regards to the choice of similarity measurement and interpretation of results. This will help to reduce the risk of missing the diagnosis of osteoporosis as the underlying disorder while at the same time promoting the timely treatment of osteoporosis. The target is to minimize the incidence of post-traumatic disability and to reduce the imminent fracture risk. This article will present a Bone-Care algorithm for the diagnosis and management of fragility fractures in patients presenting for trauma surgery. This algorithm has been developed based on recently published national as well as international guidelines for implementation in standard clinical practice. International figures revealed that only a small proportion of those patients at high risk of sustaining a fragility fracture receive osteoporosis therapy. Based on the best currently available evidence, it is safe to start osteoporosis therapy in the acute post-fracture period (the optimal therapeutic window of romosozumab is the late endochondral phase/throughout bone remodeling). The right Bone-Care pathway ensures the delivery of a comprehensive management approach that meets the global call to action. With the increasing use of immunotherapy, specifically immune checkpoint inhibitors, understanding the response of bone metastases is necessary.</p><p><strong>Recent findings: </strong>The data on checkpoint inhibitors used for managing solid tumors are reviewed here with a focus on bone metastases. Albeit with limited available data, there is a trend toward poorer outcomes in this setting, presumably due to the unique immune microenvironment within bone and bone marrow. Despite the potential to improve cancer outcomes with use of ICIs, bone metastases remain challenging to manage and may have different responses to ICIs than other disease sites. Transforming growth factor-beta (TGFβ) signaling, which is crucial for embryonic and postnatal bone development and maintenance, has been found to be essential for several osteocyte functions. There is some evidence that TGFβ might be accomplishing these functions through crosstalk with the Wnt, PTH, and YAP/TAZ pathways in osteocytes, and a better understanding of this complex molecular network can help identify the pivotal convergence points responsible for distinct osteocyte functions. This review will summarize the recent advances on how cancer cells utilize amino acid metabolism during metastasis, from early dissemination to interactions with the bone microenvironment.</p><p><strong>Recent findings: </strong>Recent studies have suggested that certain metabolic preferences for amino acids may be associated with bone metastasis. Once in the bone microenvironment, cancer cells encounter a favorable microenvironment, where a changing nutrient composition of the tumor-bone microenvironment may alter metabolic interactions with bone-resident cells to further drive metastatic outgrowth. Enhanced amino acid metabolic programs are associated with bone metastatic disease and may be further augmented by the bone microenvironment. Adolescent athletes, particularly those who train indoors and during the winter season, are at risk for vitamin D deficiency, which has important implications for bone mineral density. However, the relationship between vitamin D status and traumatic fracture risk is still unclear. While the female athlete triad is a well-established condition, the current work has led to the recognition of parallel pathophysiology in male athletes, referred to as the male athlete triad. Recent evidence suggests that transdermal 17β-estradiol treatment in amenorrhoeic female athletes is an effective adjunctive treatment to improve bone mineral density in treatment of the female athlete triad. Young athletes are at risk for musculoskeletal injuries unique to the growing skeleton. Before treatment, low bone density for age is more prevalent than expected in TGD youth. Bone mineral density Z-scores decrease with gonadotropin-releasing hormone agonists and differentially respond to subsequent estradiol or testosterone. Risk factors for low bone density in this population include low body mass index, low physical activity, male sex designated at birth, and vitamin D deficiency. Peak bone mass attainment and implications for future fracture risk are not yet known. TGD youth have higher than expected rates of low bone density prior to initiation of gender-affirming medical therapy. In addition, this review aims to outline how these modifications may influence the local mechanical environment of osteocytes and their mechanosensitivity.</p><p><strong>Recent findings: </strong>Reduction in lacunar density with age and increasing lacunar size with lactation are confirmed in multiple studies in human and murine bone. There is also evidence of a reduction in canalicular density, length, and branching, as well as increasing sphericity and smaller lacunae with aging and disease. However, while some studies have found modifications in lacunar density, size, shape, and orientation, as well as canalicular density, length, and size due to specific physiological and pathological conditions, others have not observed any differences. Recent finite element models provide insights into how observed modifications in the lacunar-canalicular network (lacunar and canalicular density) and lacunar-canalicular morphology (lacunar area/volume, shape, and orientation as well as canalicular diameter and length) may influence the fluid flow and local strains around the lacunar-canalicular network and modify the local mechanical environment of osteocytes. Modifications in the lacunar-canalicular network morphology may lead to significant changes in the strains received by osteocytes and may influence bone's response to mechanical stimulation as osteocytes are the primary mechanosensing bone cells.
