Screening for diabetic peripheral neuropathy at community pharmacies in Slovakia. 斯洛伐克社区药房的糖尿病周围神经病变筛查。

{"title":"Screening for diabetic peripheral neuropathy at community pharmacies in Slovakia.","authors":"Zuzana Pagáčová, Milan Grofik, Tomáš Fazekaš, Viera Žufková, Daniela Mináriková","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Aim: </strong>To identify diabetic patients with a potential risk of developing diabetic peripheral neuropathy (DPN) in community pharmacies in Slovakia using a modified Michigan Neuropathy Screening Instrument questionnaire (MNSIq-12).</p><p><strong>Methods: </strong>This cross-sectional study enrolled 703 patients with type 1 and type 2 diabetes mellitus who had not been diagnosed with DPN. The study took place in selected community pharmacies across Slovakia in October 2019. The MNSIq-12 was administered by pharmacy students, and a Michigan score <1.5 was considered risky. The groups divided based on the Michigan score were compared in terms of duration of diabetes, age, body mass index (BMI), sex, weekly physical activity, level of education, and smoking.</p><p><strong>Results: </strong>The risk of developing DPN was detected in 6.6% of respondents with type 1 diabetes and 13.4% with type 2 diabetes. Patients with both types of diabetes (38.2%; 67.0%) reported fatigue and heaviness in the legs as the most common clinical symptoms that may indicate the development of DPN. Those with a Michigan score <1.5 were older (P<0.0001), had a higher BMI (P<0.0001), a lower level of education (P=0.0020), and were less physically active (P<0.0001). Conclusion Approximately one-eighth of patients with diabetes who visited community pharmacies were potentially at risk for developing DPN. First, ChatGPT 3.0 was used to generate 15 case reports, which were then peer-reviewed by expert human reviewers. Second, ChatGPT 4.0 was employed to peer review 15 published short articles.</p><p><strong>Results: </strong>ChatGPT was capable of generating case reports, but these reports exhibited inaccuracies, particularly when it came to referencing. The case reports received mixed ratings from peer reviewers, with 33.3% of professionals recommending rejection. The reports' overall merit score was 4.9±1.8 out of 10. The review capabilities of ChatGPT were weaker than its text generation abilities. Pharmacokinetic parameters were measured from blood samples, and metformin concentrations were determined with validated analytical techniques.</p><p><strong>Results: </strong>There was a significant variability in metformin concentrations among PCOS patients, with body mass index (BMI) identified as a major influencing factor. Twelve studies were included in the final selection.</p><p><strong>Results: </strong>The review highlights the diverse prevalence rates of benzodiazepine abuse and poisoning in the older adult population. Benzodiazepine anxiolytics were more frequently associated with negative outcomes than benzodiazepine hypnotics. DNA extracted from paired tissue and blood samples was subjected to Sanger sequencing to identify alterations in the entire mtDNA. The associations between clinicopathological characteristics and mutations were evaluated. Cox-regression multivariate analysis was conducted to identify factors significantly associated with survival, and Kaplan-Meier analysis was used to compare the survival of patients with and without mutations.</p><p><strong>Results: </strong>Overall, 29.6% of the patients harbored 19 somatic mutations distributed across 15 loci within the mtDNA. Notably, 36.8% of these mutations were not previously documented in MITOMAP. One newly identified mutation caused a frameshift in the ATPase6 gene, resulting in a premature stop codon. Three mutations were classified as deleterious in the MitImpact2 database. Overall, 1097 mtDNA polymorphisms were identified across 331 different locations. Patients with mutations exhibited significantly shorter survival than patients without mutations.</p><p><strong>Conclusions: </strong>mtDNA mutations negatively affected the survival outcomes of Malaysian patients with primary brain tumors. At baseline assessment (in the first three months following BC diagnosis) and follow-up assessment (one year after baseline), participants completed the WAI, Beck Depression Inventory-II, State-Trait Anxiety Inventory, and European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire with a breast cancer-specific module. Mediation analyses were conducted to explore the mechanism by which depressive, anxiety, and physical symptoms influenced the relationship between WAI and depressive and anxiety symptoms.</p><p><strong>Results: </strong>WAI was negatively associated with depressive and anxiety symptoms. The effect of baseline depressive and trait anxiety symptoms on WAI at follow-up was mediated by both depressive and trait anxiety symptoms, as well as by physical symptoms at follow-up. The effect of baseline state anxiety symptoms on WAI at follow-up was mediated only by state anxiety symptoms at follow-up.</p><p><strong>Conclusions: </strong>Baseline depressive and anxiety symptoms affect WAI at follow-up not only through persisting depressive and anxiety symptoms observed at follow-up but also through physical symptoms at follow-up.
