The purpose of this study was to identify and document the serotypes of Dengue virus and to measure the viral load of the virus positive specimens. Methods: This observational study was conducted in a tertiary care hospital of Chattogram during a period of six months from June through December, 2022. A total of 74 patients with symptoms suggestive of Dengue Fever(DF) and NS1 or IgM/IgG positive dengue patients were recruited into the study. From them clinicopathological data was collected on a case record form(CRF). Ten ml of venous whole blood was drawn in EDTA tubes then centrifuged and was sent to the laboratory at SlieaGen L.L.C in Austin, Texas, USA, for Dengue serotyping and viral load analysis by qPCR-LDT method. A total of 73 specimens were analyzed, one specimen was excluded. Ethical clearance was obtained from the hospital ethical review board, written informed consents were also taken from all subjects. Serotyping and qPCR testing data was shared by SlieaGen LLC once analysis was completed. Results: Amongst the 74 patients most were male 57(77%) and most [20(40.5%)] patients were young at 21-30 years. All had history of fever(100%), followed by body aches 67(90.5%), headache 49(66.2%) and weakness 55(74.3%). Positive NS1, IgM Dengue Ab and IgG Dengue Ab were found in 68(91.9%), 8(10.8%) and 3(4.1%) cases respectively. Frequency of serotype distributions revealed DENV 1 and DENV 3 were the most common serotypes 25(33.8%) and 16(21.6%) respectively also DENV 2 was 3(4.1%) and DENV 4 was 1(1.4%). Among 27 virus positive cases single infection DENV 1 was found in 10(13.51%) samples, DENV 3 and DENV 4 found in 1(1.4%) samples each, doubly infected (DENV 1 + DENV 2) was found in 12(16.2%) samples, triple infected (DENV 1 + DENV 2 + DENV 3) was found in 3(4.1%) samples. No virus was detected in 46(62.16%) samples. Viral load (Mean ± SD ) found in DENV 1(890974.48 ± 4156206)cp/ml, DENV 2 (33423.00 ± 9088.35)cp/ml, DENV 3(23991.12 ± 51250.18)cp/ml and DENV 4 (822.00cp/ml) in virus detected samples Conclusions: All four serotypes were found in the 2022 Dengue outbreak in Chittagong and DENV 1 and DENV 3 were common serotype. Simultaneous double and triple serotype infection in the same patients may be related with different unusual clinical features which need further documentations in future. It also stood with a great deal of challenge to the physicians working in the hospitals and treating the pediatric patients yet looking for a straightforward approach to deal with severe cases of Dengue. Observing the low albumin status in every single Dengue Shock Syndrome (DSS) the concept of arresting the progression of DF (Dengue Fever) to DSS (Dengue Shock Syndrome) came in our mind. Therefore, the study topic was selected as “Role of albumin infusion in treatment outcome and preventing DSS in children with DF: A quasi purposive study.” Objective: The objectives of the study were to observe the status of albumin of DF, the relationship of Albumin with Platelets, the effect of albumin infusion on the progress of DF to DSS. Patients and Methods: This is a prospective quasi purposive study conducted in the department of Pediatrics at Bangladesh Specialized Hospital (BSH) from June 2022 to December 2022 in the age group of Pediatric patients (from more than 1 month eighteen years). All the patients admitted with DF or DSS during that period were included in the study. National Guideline of DF and DSS was followed to Diagnose and to hydrate the Dengue affected children. Albumin was administered to manage not only DSS but also DF children to prevent them from progressing to DSS. Journal of Bangladesh College of Physicians and Surgeons Vol. 41, Dengue Fever Supplement Issue, October 2023 They were managed following the national guideline. Additionally, Hct and Albumin status were closely monitored particularly on day five, six and seven of the disease. Albumin was administered according to the set protocol for this study. Results: A total of 44 cases were included in the study. Among them 28 DF and 16 DSS were diagnosed and documented in this study. Twenty-seven male and seventeen female children were admitted with common symptoms like Fever, bodyache, headache, abdominal pain, vomiting and loose motion. A total of thirty-six children were administered Albumin. Among them 21 were DF and 15 were DSS. In statistical analysis no significant difference was found in Age, Weight, Hematocrit (Hct), Ferritin on admission when compared between DF and DSS. No significant difference could be observed also in Hospital Stay as compared between the two groups. However, significant statistical differences were detected in Day 1, 2, 3 platelet levels and Day 1 and 2 Albumin levels. Neither any DIC nor any MISC, nor any death was recorded. No antibiotic was required. No Platelet transfusion, no IVIG, no Methylprednisolone, nor any other steroid was needed. No significant adverse effect was documented in administering Albumin. Conclusion: In this study it has been found that albumin is safe to use in children with DF to prevent DSS. It also is effective in reducing morbidity in Dengue affected children. According to the World Health Organization, fifty million cases of clinical dengue infections occur each year, posing a significant public health hazard, primarily in Southeast Asia and the Western Pacific. The clinical spectrum of dengue fever ranges from asymptomatic infection to dengue shock syndrome. Although dengue virus is non-neurotropic virus, there is increasing evidence for dengue viral neurotropism suggesting there may be an element of direct viral encephalitis. Encephalopathy has been thought to result from the multisystem derangement that occurs in severe dengue, with liver failure, shock and coagulopathy causing a cerebral insult.  Dengue encephalitis is a rare disease. We report dengue encephalitis in a 55-year-old female, admitted in Rajshahi Medical College Hospital, Rajshahi, Bangladesh who presented with fever, vomiting, and altered level of consciousness. Her serum dengue NS1 was positive. MRI of brain showed hyperintensity in both basal ganglia & cerebellar region, which is identical to “Double Doughnut sign” found specifically in dengue encephalitis. The complex interplay between virus serotype, host immune response and environmental factor set a series of changes in dengue presentation over the years. Materials and method: This was a cross-sectional, observational study conducted on 100 diagnosed dengue patients attending a tertiary teaching hospital. The study participants were divided into severe and non-severe categories and compared in terms of their symptoms, severity and lab findings. Statistical analysis was done using SPSS. Result: Majority of the study participants were younger. Female participants were more than male. Apart from fever, headache was found to be the commonest symptom in general. Epistaxis and melaena were significantly higher among severe group than non-severe peers. Gastrointestinal symptoms were frequently present among both groups. Conclusion: Most of the cases of dengue patient didn’t suffer from life threatening infection but when the infection is severe, bleeding manifestation can be a predictor of the onset of critical illness. Acute Coronary Syndrome (ACS) during dengue is rare but poses important management dilemma. Here we present a series of 5 patients with dengue fever who developed different types of ACS during the ongoing dengue endemic in Bangladesh. Five patients admitted with dengue fever and who developed ACS between 1st July to 30th September 2023 at Medicine Department of Uttara Adhunik Medical College, Dhaka, Bangladesh were studied. All the patients had some co[1]morbidity like diabetes, hypertension, or both. Treatment of ACS was tailored according to their CBC report, most importantly, platelet count, and hematocrit. Four out of the 5 patients responded well to anti-ischemic therapy, even though anti-platelet and anti-coagulants could not be given in all cases. There was 1 fatality, but that was due to ARDS consequent due to severe bilateral pneumonia. Management of ACS in dengue is challenging. Physicians should have appropriate preparedness to deal with these cases. Most vulnerable population are children especially children below 5 years of age. However, data regarding dengue among this age group is limited. The objectives of this study were to see the clinical and laboratory profile of dengue fever (DF) in children below 5 years of age and their outcome. Methods: This cross – sectional observational study was carried out in the department of Paediatrics, BIRDEM General Hospital from May 2023 to September 2023. One hundred eighty nine confirmed dengue cases were enrolled in the study. Their clinical profile and laboratory findings including haemoglobin (Hb%), haematocrit (Hct), total count of white blood cells (TC), platelet count (PC), serum alanine aminotransferase (S. ALT), serum aspartate aminotransaminase (S. AST), prothrombin time (PT), international normalized ratio (INR), activated partial thromboplastin time (aPTT), serum calcium and serum sodium level were documented and analyzed. Results: Among 189 children 57(30.1%) were infants and 132(69.8%) patients were aged 1 to 5 years. Depending upon the severity of dengue 98(51.8%) were classical dengue fever, 65(34.3%) and 14(7.4%) presented with dengue shock syndrome (DSS) and dengue heamorrhagic fever (DHF) respectively. Fever was present in all children, other common features were vomiting (34.9%), cough (25.9%), dehydration (26.9%), pleural effusion (34.3%), ascites (23.2%) and skin rash (18.5%). Shock were present in 41.1% cases. Warning signs like restlessness, lethargy, altered level of consciousness, convulsion was present in 16.4%, 9.5%, 2.6% and 11.6% respectively. All the children were improved and discharged. Conclusion: In this study dengue presented classically in most of the children. Dengue shock syndrome and dengue haemorrhagic fever were below 10%. The outcome was excellent with no modality. Early reporting to hospital appeared important. Our two patients resented here with fever followed by chest pain, sweating and collapse of Blood pressure and their lab reports were quite similar such as low platelet, ECG change, raised Troponin I, low ejection fraction on echocardiography. They were rescued by injection intravenous Methylprednisolone therapy. Here we present this case series to improve recognition and prevent progression to fatal outcome. Acute parotitis is one of the important atypical presentations of dengue fever. We report 3 cases of dengue fever manifest as inflammatory enlargement of the parotid glands (acute parotitis). The aim of this report was to create awareness about atypical presentations of dengue fever like acute parotitis.
