Zohra","doi":"10.3329/jbcps.v42i1.70647","DOIUrl":"https://doi.org/10.3329/jbcps.v42i1.70647","url":null,"abstract":"Vaginal cancer, rarest form of gynaecological cancer (0.6%) has been found to be associated with long term use of vaginal ring pessary, which have long been used in alternative management of pelvic organ prolapse (POP) other than surgery. A survey was done in remote areas of Cox’s bazar, refugee camp in 2019 where out of 52 cases of pelvic organ prolapse, 7 cases found to use vaginal ring pessary for long and out of 7, vaginal cancer developed in 4 cases. All of them were aged women above 60 and had defaulted follow up presented with vaginal bleeding, varying discharge and pain of varying intensity. One patient had only local lesion –surgical removal followed by radiotherapy and 2nd one,68 years, received neo-adjuvant therapy and died before planned surgery. The 3rd one, aged 79, did not come for follow up once diagnosis vaginal cancer done and advised for definitive surgical treatment, family refused operation and she passed away after 2 years at 81 years. The 4th one declined further investigations and any form of treatment when diagnosed with vaginal cancer. This is an observational study on retrospectively collected data of gynecological cancer patients at National Institute of Cancer Research and Hospital (NICRH), Dhaka. All cases of cancer seen at the gynecology outpatient department and cases admitted in the gynecological oncology ward at NICRH, between the months of January to December 2022 were audited. The results shown that about 5.8% of all the new cases were young women aged below 25 years. The majority of gynecological cancers were among women aged between 35 and 64 years (73.17%) and only 2.6% patients were aged above 75 years. Cervical cancer (59%) was the leading gynecological cancer among all female cancers attended at NICRH. In ovarian malignancy, most of the patients(61%) had stage lll disease and no patient in stage IV diseases. Due to improper evaluation of abnormal uterine bleeding, majority of the endometrial carcinoma patients (64%) presented with histopathology reports after hysterectomy. This is a crucial health issue for mankind and needs structured treatment and rehabilitation plans. Prevalence and management capacity of disability have wide cross-cultural and geographical variations. Despite the lack of specific data there are reasons to believe that third-world countries have more disabilities and fewer resources and capacity for rehabilitation. So, there is a strong relationship between poverty and disability. Marginalized populations, disadvantaged or at-risk populations such as women, older people, and people who are poor are inappropriately affected by disability. Children from poverty-stricken families, indigenous populations, and ethnic minority groups are significantly at higher risk of experiencing disability. There is a higher prevalence of disability in lower-income countries than in higher-income countries. Poor people from lower-income countries are usually involved in high-risk jobs which make them vulnerable to injury and disability. Because of exaggerated life stress, poor health status, and poor living conditions due to poverty, people at low resource outset are more vulnerable to disease and disability. Of inadequate support from affluent persons as well as from the government, they spend their last savings to meet the treatment costs and get poorer. They are also common victims of road traffic accidents when they travel on risky roads and vehicles. There are many other shreds of evidence for a link between disability and poverty in low and middle-income countries and there is an urgent need for further research and policy action to break the cycle. However, a rare complication, pseudotumour cerebri may arise following All Trans Retinoic Acid (ATRA) therapy, which may provide obstacle to continue this promising therapy. We present the case report of a young female patient with acute promyelocytic leukaemia treated with Arsenic trioxide (ATO) and ATRA that subsequently develop abrupt onset of lateral rectus palsy and decreased visual acuity due to pseudotumour cerebri. We treated the patient with acetazolamide and kept her at frequent follow up. Previously he was diagnosed as ‘O’ RhD positive and had two events of transfusion reaction during ‘O’ positive blood transfusion. In literature search and browsing there is dearth information on Para-Bombay Phenotype with E-Beta Thalassaemia. So far to our knowledge, this is the first reported case from Bangladesh of patient having ‘E-Beta Thalassaemia’ evidenced with rare ‘Para-Bombay blood Phenotype’. {"title":"Kawasaki Disease in Children: A Brief Update","authors":"S. Rahman, K. Laila","doi":"10.3329/jbcps.v42i1.70643","DOIUrl":"https://doi.org/10.3329/jbcps.v42i1.70643","url":null,"abstract":"Kawasaki disease (KD) is a common acute vasculitis of childhood that may lead to coronary artery abnormalities in about 25% of patients if left untreated. KD is reported as the leading cause of acquired heart disease in children replacing rheumatic feverin developed countries. Strong epidemiological data are not available from developing countries, but it is increasingly recognized in rapidly industrializing countries of Asia including India,where it might has replaced rheumatic heart disease as the most common cause of acquired heart disease. This increasing recognition could be due to actual rise in number of KD cases or due to more diagnosis following increased awareness.\u0000The status of KD in Bangladesh is not known. But unpublished data from Bangabandhu Sheikh Mujib Medical University (BSMMU) show a sustained and significant increase in the number of KD cases over the years. The reasons for this increase may be similar to our neighbouring country India. Aetiology of KD still remains unknown. But there have been many hypotheses including infection, immune dysregulation, super-antigen and genetic factors.\u0000The diagnosis of KD is based on the presence of ≥ 5 days of fever and the presence of ≥4 of the 5 principal clinical features (1.Extremity Changes, 2. Conjunctival injection, 3.Oral changes 4.Exanthem/Rash, and 5. Lymphadenopathy).\u0000The diagnosis of KD may be very challenging for even a very experienced paediatrician. It must be considered in a child with prolonged fever, undue irritability and sequential appearance of principal clinical features.\u0000KD patients should be managed timely with intravenous immunoglobulin (IVIG) and aspirin in proper dose. Generally, CVST is a fulminating process with a high rate of mortality (1 in 3 patient) secondary to a spreading bacterial infection specially over head and neck region where coagulase positive Staphylococcus (90%) is mostly responsible. Individuals with hypercoagulable state i.e., pregnancy or puerperium and malignancy can also suffer from this form of thrombosis. Here we are presenting such a case, who developed CVST on the 5th day of puerperium with features of persistent headache and eye problems. The diagnosis was confirmed with MRI in combination with Magnetic resonance venography (MRV). She responded well to low molecular weight heparin (LMWH) and was discharged on long term oral anticoagulant and regular follow-ups. Although short lasting and self-limiting, it creates an uncomfortable situation for patient. Diffusion of Nitrous Oxide (N20) into endotracheal tube (ETT) cuffs and increase intra cuff pressure causes this. Different inflating agents have been studied to see the incidence of intra cuff pressure changes. This study was performed to see the post extubation tracheal mucosal changes and post-operative sore throat after using N20 in Oxygen as ETT cuff with direct visualization of the tracheal mucosa just after tracheal extubation.\u0000Methods: Institutional ethical committee approval and informed written consent were obtained, 50 patients, undergoing elective surgery under general anaesthesia, were enrolled. Patients were randomly allocated in two groups of 25. In Group A, the ETT cuff was inflated with air, in Group B, cuff was inflated with 67% N₂0 in oxygen. The cuff pressure was kept around 30 cm H₂0 and was measured every 10 minutes. The condition of the trachea was seen using fibreoptic bronchoscope and a grading was done. All the data was collected & statistical analysis performed.\u0000Results: The frequencies of pain, difficulty in swallowing, and hoarseness of voice at the recovery room within the first two hours after anaesthesia were lowest in Group B than Group A and significantly reduced within 24 hours. Perioperatively cuff pressure change in group A was significantly high and in group B it was minimum. Bronchoscopic view of larynx and trachea showed 64 % normal. 22% was erythematous, 10% was oedematous in group B. In group A it was 28%, 46% and 24%.\u0000Conclusions: It is evident, that N₂0 in 0₂ filled ETT cuff reduces the incidence of postoperative sore throat. Bangladesh being a high endemic region for dengue infection bears risk of significant healthcare burden of such co-infection cases. During the surge of dengue cases during monsoon season in 2021, the possibility of COVID19 - Dengue co-infection cases also increased. Method: This study was carried out in the Department of Internal Medicine and Pulmonology, Bangladesh Institute of Research and Rehabilitation in Diabetes, Endocrine and Metabolic Disorders (BIRDEM) General Hospital, Dhaka, Bangladesh from June 2021 to December 2021. Dengue and COVID 19 cases were confirmed as per Bangladesh National guidelines. Result: Total 12 cases including 8 males were diagnosed as dengue –COVID 19 co-infection during the study period. Mean age was 47.0 years. Majority of cases (10) were residing in Dhaka. Among risk factors, 5 cases were known diabetic and 4 cases were hypertensive while 1 case was congenital adrenal hyperplasia. Common presentations were fever (100%), headache (75%), bodyache (66.7%), dry cough (41.7%), nausea, vomiting, abdominal pain, and retro orbital pain were 25% while rash and shortness of breath occurred in 16.7%. Mean duration of fever was 5.8 days and mean recorded highest temperature was 102.5°F. Dengue manifestation was predominant in 8 cases (66.7%). Laboratory investigations revealed following mean values: baseline haematocrit (HCT) 39.9%, initial total white cell count 4.64x109/L, absolute neutrophil-lymphocyte ratio 2.4, lowest platelet count 100.4x109/L, C-Reactive Protein 143.8mg/L, serum ferritin 888.1 ng/ml, D dimer 2.0 µg/ml, LDH 668.1 U/L, serum Aspartate aminotransferase 88.1 U/ L, Alanine aminotransferase 73 U/L. Radiological findings were present in 7(58.3%) cases, among which pneumonitis was predominant (4 cases,57.1%). All cases were managed and followed up as per national guidelines till discharge and no one required critical care. Conclusion: Observation revealed that COVID -19 and Dengue co-infection may occur and if managed adequately fatal outcome can be avoided, even in high risk patients. Therefore, high degree of suspicion of co-infection should be considered in appropriate clinical scenario.
