The specific prevalence ranges from 1 in 100,000-300,000 population. Development, genetics, and mutation play a role in the formation of ectopic thyroid. The most common location of ectopic thyroid is at the base of the tongue, followed by sub-mandibular, pre-tracheal, esophageal, mediastinum, diaphragm, and other rare sites. Lateral ectopic thyroid is further a rare clinical entity which is formed due the failure of descend of the lateral thyroid component to the midline with a euthyroid in its normal anatomical position. Here, we present a case report of lateral ectopic thyroid which ordinarily does not flash as a diagnosis title for a painless, firm mass on the lateral aspect of the mandible, making the treatment plan inconclusive. Clinical, radiographic, laboratory tests with a histopathalogical examination make a categorical and final treatment for the ectopic thyroid. {"title":"Microinvasive oral squamous cell carcinoma: A management protocol.","authors":"Uma Shanker Pal, Seema Devi, Meleti Venkata Sowmya, Harshita Maurya, Sandeep Kumar, Ranjeet Singh","doi":"10.4103/njms.njms_81_23","DOIUrl":"10.4103/njms.njms_81_23","url":null,"abstract":"<p><p>Microinvasive oral squamous cell carcinoma (MISCC) is the early stage of oral cancer that shows a breach in the basement membrane, and the depth of invasion of the tumour is limited to 0.5-2mm into the papillary stroma. A review of the available data on MISCC was conducted to formulate a management protocol for MISCC and to assess the association of MISCC with oral potentially malignant disorders. An electronic and manual search of the articles was conducted following the inclusion and exclusion criteria. A total of 14 articles were selected, evaluated, and details were tabulated. Before finalizing the treatment plan, potential diagnostic errors on both the surgeon and pathologist's part and prognostic outcomes should be taken into consideration. An association between oral potentially malignant disorders and MISCC was observed in 12 out of 14 articles. According to the NCCN guidelines, elective neck dissection is only indicated in highly selective cases for a depth of invasion of less than 2mm. However, three out of 14 MISCC cases showed metastasis to lymph nodes during the follow-up period. Multiple recurrences and second primaries in the case of MISCC are also a matter of concern. Management protocol was formulated for MISCC considering the histopathological, clinical, and radiological presentation. Precise management of oral potentially malignant disorders with rigorous follow-up for at least 3 years is advisable to prevent their carcinomatous transformation. Recent research emphasizes the use of CBCT for the anatomic study of the temporal bone. The information about middle ear anatomy on CBCT scans is meager; hence, this retrospective study was conducted to identify and determine the various morphometrical parameters of the malleus using CBCT which can be helpful during reconstructive procedures for the otologic surgeon.</p><p><strong>Materials and methods: </strong>The retrospective study was performed on 200 subjects (101 M and 99 F) in the Department of Oral and Maxillofacial Radiology. These recommendations standardize management, reduce interobserver variability, and standardizes scientific communication.</p><p><strong>Objectives: </strong>The primary aim of this study was to validate the correlation between the NI-RADS category and disease status on clinical follow-up and histopathological analysis. The other objective was to assess the status of primary as well as nodal site at least 8 to 12 weeks after definitive treatment on first post-treatment imaging as per NI-RADS.</p><p><strong>Materials and methods: </strong>We did a retrospective review of maintained a database of patients treated with curative intent radiotherapy or chemoradiotherapy. The diagnostic accuracy of NIRADS was compared with the clinical follow-up and histopathological findings. Data was recorded using the NIRADS lexicon and analyzed using SPSS 25.</p><p><strong>Result: </strong>In our study, 37 cases were followed with CECT, whereas 111 were followed with PET-CT. We observed no significant difference between CECT and PET-CT for predicting recurrence in any of the NIRADS category. Sensitivity, specificity, positive predictive value, negative predictive value, and diagnostic accuracy of NIRADS to predict recurrence for the primary site is 61.54%, 75.21%, 34.8%, 90.1%, and 72.79%, respectively, whereas for the neck, it is 69.54%, 75.41%, 37.5%, 92%, and 74.32%.</p><p><strong>Conclusion: </strong>NIRADS score is strongly associated with positive disease in as Neck as well as primary. Children with cleft palate are more prone to develop this condition. This is caused by impaired eustachian tube function in cleft palate. They may present with hearing loss, delayed speech, and language development or poor social behavior. This is a significant cause of conductive hearing loss in the early speech-forming years of their life. Even after an early surgical repair of palate, speech and language defects are frequently identified during regular postoperative follow-up by speech-language pathologists. When identified, the approach to language delay varies across healthcare systems and includes watchful waiting, providing hearing aids to early otological intervention. The primary objective was to compare the incidence of SOM in children with and without language delay following surgical correction of cleft palate.</p><p><strong>Materials and methods: </strong>A case-control study involving 63 patients was conducted in a tertiary care hospital with a high-volume cleft lip and palate center from June 2022 to March 2023. Thirty children with language delay were identified and grouped as cases and 33 children without language delay as controls. Tympanogram was conducted after the detailed otoscopic examination. Those with Jerger types B and C were diagnosed with SOM. The incidence of SOM in each group was compared statistically.</p><p><strong>Results: </strong>About 70% of cases and 87.9% of controls had SOM. The concept of nonsubmerged healing was introduced in 1988 in which gingival former was placed at the time of implant placement instead of cover screw. The treatment concept of nonsubmerged implant placement enables early implant loading, one surgical procedure only, and a reduced treatment period for the patients.</p><p><strong>Aim: </strong>The aim of this study was to compare clinical and radiographic outcomes of submerged and nonsubmerged implants.</p><p><strong>Materials and method: </strong>A prospective split-mouth study of a minimum of 20 bilateral edentulous patients (total of 40 sites) was selected and randomly distributed in two groups: submerged and nonsubmerged implants. Patients were evaluated clinically and radiographically by assessing the patients on clinical parameters which included peri-implant probing depth, mucosal thickness, and width of keratinizing gingiva. Radiographic parameters were crestal bone height-distance between implant shoulder and alveolar crest compared from the bone level, on intraoral periapical.</p><p><strong>Results: </strong>Our results revealed that the marginal bone loss is statistically significant and has higher values in nonsubmerged implant protocol. All the other clinical parameters were statistically insignificant.</p><p><strong>Conclusion: </strong>The nonsubmerged implants have reduced the chairside time of the patient, but have a comparatively higher crestal bone loss. After the onset of local anesthesia, the first group (Group A) received a submucosal injection of 4 mg dexamethasone adjacent to the surgical site, and the control group (Group B) received no drug. Pain, swelling, and trismus were recorded at follow-up visits on the 1<sup>st</sup>, 2<sup>nd</sup>, and 7<sup>th</sup> postoperative days. Assessment of postoperative discomfort was performed by evaluating responses through a modified postoperative symptom severity scale questionnaire, which was administered to the patients on the 7<sup>th</sup> postoperative day.</p><p><strong>Results: </strong>The difference in subjective pain values and the mean number of analgesics consumed was not significant between the groups. The difference in postoperative swelling was statistically significant on the 1<sup>st</sup>, 2<sup>nd</sup> (<i>P</i> < 0.0001), and 7<sup>th</sup> postoperative days (<i>P</i> = 0.0152). The difference in postoperative trismus was highly significant on 1<sup>st</sup> and 2<sup>nd</sup> postoperative days (<i>P</i> < 0.0001). This study aimed to determine the characteristics and pattern of traumatic maxillofacial injuries in children and adolescents reported to outpatient departments of two tertiary care health centers.</p><p><strong>Materials and methods: </strong>Present study had a retrospective design, and the hospital records of all children and adolescent patients aged between 0 and 18 years, who had undergone maxillofacial fractures and were admitted for the same to two tertiary care health centers between January 2012 and May 2022 were reviewed.</p><p><strong>Results: </strong>77 patients suffered maxillofacial trauma with 115 fractures. The leading cause of maxillofacial trauma in the study was found to be road traffic accidents (RTA) followed by falls and sports-related injuries. Maxillofacial trauma was observed more in boys than girls, with an M: F ratio of 3.8:1. Out of 77 patients in the present study, more than half (51.9%) suffered maxillofacial trauma involving the lower one-third of the face, followed by the middle third (45.5%) and upper third (2.6%). A total of 11 patients (14.2%) were treated within 24 hr of sustaining injury. The majority of the patients, (n = 68; 88.3%) underwent open reduction and rigid internal fixation (ORIF), while 9 patients (11.7%) underwent closed reduction.</p><p><strong>Conclusions: </strong>Pediatric maxillofacial trauma was mainly attributed to road traffic accidents. The objective of our study was to assess the surgical outcome of sutureless skin closures using Octyl-2-cyanoacrylate (Dermabond™) versus Steri-Strip™.</p><p><strong>Method: </strong>The present prospective study was conducted in 20 patients. Patients were divided into two groups. After subcutaneous closure of wounds, either Dermabond™ or Steri-Strip™ was placed. The patients were assessed for wound complication (erythema, tenderness, dehiscence or any discharge), scar hypertrophy and cosmetic appearance also time consumed in surgical skin closure was evaluated. Wound assessment, scar hypertrophy and cosmetic appearance were assessed by using Chi-square test. Time closure was assessed by using Mann-whitney U test.</p><p><strong>Result: </strong>Twenty patients belonging to all age group were included in study. Ten patients undergone closure with Dermabond™ and ten with Steri-Strips™. Assessment of wound complications, cosmetic appearance and scar hypertrophy was done. There was no significant difference found between both the groups, but 2 patients had fair cosmetic outcomes at one month and 1 patient had fair scar hypertrophy at 6 month. However, excellent cosmetic outcome in terms of scar hypertrophy at 6 month was significantly more among group II.</p><p><strong>Conclusion: </strong>Octyl-2-cyanoacrylate (Dermabond™) and Steri-Strip™ provide similar outcomes in terms of wound complications.
