{"title":"Anterior Segment Swept-Source Optical Coherence Tomography-based Assessment of Corneal Refractive Profiles in Stevens-Johnson Syndrome/Toxic Epidermal Necrolysis Patients: A Controlled Comparative Study.","authors":"Dong Hee Ha, Seung Hyeun Lee, Kyoung Woo Kim","doi":"10.3341/kjo.2024.0106","DOIUrl":"10.3341/kjo.2024.0106","url":null,"abstract":"<p><strong>Purpose: </strong>To analyze anterior, posterior, and total corneal refractive profiles and thickness using anterior segment (AS) sweptsource optical coherence tomography (SS-OCT) in Stevens-Johnson syndrome (SJS) or toxic epidermal necrolysis (TEN) patients.</p><p><strong>Methods: </strong>This retrospective study compared 23 eyes from 14 patients with SJS/TEN with 23 eyes from 14 age- and sexmatched controls. AS SS-OCT measured corneal refractive profiles and pachymetry. Parameters included anterior, posterior, and total corneal power values, posterior to anterior curvature ratio, maximal corneal power, and thickness profiles. The SJS/TEN group was subdivided based on corneal opacity presence (subgroup A) or absence (subgroup B).</p><p><strong>Results: </strong>The SJS/TEN group showed significantly higher corneal astigmatism and maximal corneal power values for both anterior and posterior curvatures. Steep total corneal power and total corneal astigmatism were higher in the SJS/TEN group. The thinnest corneal thickness was lower, and its distance from the corneal vertex was greater in the SJS/TEN group. Subgroup analysis revealed that these differences were primarily attributed to the presence of corneal opacity. Additionally, automated keratometry of the anterior corneal surface showed significantly higher values, including steep, flat, and average keratometry and corneal astigmatism, in the SJS/TEN group.</p><p><strong>Conclusions: </strong>AS SS-OCT shows significant alterations in both anterior and posterior corneal curvatures in patients with SJS/TEN. {"title":"Causes and Clinical Characteristics of Compressive Optic Neuropathy in South Korean Patients.","authors":"Sang Hun Yun, Dae Hyun Kim","doi":"10.3341/kjo.2024.0075","DOIUrl":"10.3341/kjo.2024.0075","url":null,"abstract":"<p><strong>Purpose: </strong>We aimed to investigate the causes and clinical features of compressive optic neuropathy in Koreans.</p><p><strong>Methods: </strong>We analyzed the medical records of patients diagnosed with compressive optic neuropathy from March 2014 to December 2023 to determine the cause of optic nerve compression, symptoms and symptom patterns at first visit, accompanying symptoms, types of visual field defects, and visual prognosis after surgery. In addition, the results of visual field tests and optical coherence tomography were analyzed to determine the positivity rate of each test and factors affecting test results.</p><p><strong>Results: </strong>A total of 73 patients were diagnosed with compressive optic neuropathy, and the most common cause was pituitary tumor (37 patients, 50.7%) followed by meningioma (16 patients, 21.9%), hemangioma (4 patients, 5.5%), thyroid ophthalmopathy (4 patients, 5.5%), and paranal sinus tumor tumor (2 patients, 2.7%). In terms of symptom pattern, half of the patients had vision and visual field defects that appeared gradually (41 patients, 56.2%), but there were also quite a few patients whose symptoms appeared suddenly (17 patients, 23.3%). The positivity rate of the diagnostic test was highest for visual field testing (89.0%). In pituitary tumors, tumor size affected diagnostic test results (p = 0.01).</p><p><strong>Conclusions: </strong>The most common cause of compressive optic neuropathy in South Koreans was a pituitary tumor. The patients were divided into two groups based on whether the MH was diagnosed before or after tumor treatment. The clinical features, pathogenesis, management options, and clinical outcomes were reviewed.</p><p><strong>Results: </strong>Of 505 patients with UM in our institution, 5 (1.0%) had a concurrent MH in the ipsilateral eye. The 17 patients reviewed had a mean age of 63.9 years at the time of MH diagnosis. Of 16 patients with available data on sex, 11 (64.7%) were female. There were no major differences in the demographic or clinical data of the groups. Of the 15 known tumor locations, 6 (35.3%) were juxtapapillary or macular. In patients who developed MH after UM treatment, the durations from tumor treatment (radiotherapy or transpupillary thermotherapy) to MH diagnosis were 3 to 56 months (median, 8.5 months). MH surgery was performed in nine eyes, and hole closure was achieved in seven eyes with postoperative data. The mean visual acuity showed a tendency of improvement after surgery. No intraocular or extraocular tumor dissemination associated with surgery was observed.</p><p><strong>Conclusions: </strong>MH is observed in approximately 1% of patients with UM, either before or after tumor treatment. Gene expression was analyzed via microarray. Differentially expressed genes (DEGs) were defined as those with a fold change greater than 2 or less than -2. The DEGs were further explored using Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analyses.</p><p><strong>Results: </strong>Our analysis identified 71 upregulated and 58 downregulated genes. A2m exhibited the highest increase, with a fold change of 4.97, whereas Rho displayed the most significant decrease in expression, with a fold change of -6.38. GO and KEGG pathway analyses revealed substantial involvement in sensory organ development and the phototransduction pathway.</p><p><strong>Conclusions: </strong>This study provides new insights into the impact of CNTF on gene expression in RGCs, suggesting broader neuroprotective mechanisms that could inform future therapeutic strategies for retinal degenerative diseases. The study included 51 cases: 34 traumatic and 17 spontaneous CCFs.</p><p><strong>Results: </strong>A total of 51 patients with CCFs was analyzed. Traumatic CCF (34 patients, 66.7%) was more common than spontaneous CCF (17 patients, 33.3%). Traumatic CCF predominantly affected male patients, while spontaneous CCF was more frequent in female patients (p = 0.005). Clinical signs including proptosis and lagophthalmos were more common in traumatic CCF, with significant differences in eye movement restriction (p = 0.006) and bruit (p = 0.008). According to the Barrow classification, all spontaneous CCF cases were categorized as types B, C, or D, whereas traumatic CCF was predominantly classified as type A (p < 0.001). Endovascular treatment was more effective than conservative treatment in reducing clinical signs of traumatic CCF (p < 0.05), while no significant differences were observed in outcomes for spontaneous CCF between the two approaches.</p><p><strong>Conclusions: </strong>Spontaneous CCF is less common than traumatic CCF in this study cohort. Traumatic and spontaneous CCF may have different clinical characteristics. Eye movement restriction and bruit are more frequently observed in traumatic CCF. Cataract is diagnosed using a slit lamp to evaluate the opacity of the lens. Slit lamps as the primary tools cataract are not available in primary care units. Smart Eye Camera (SEC) is a medical device placed on smartphone, capable of creating slit beam similar to conventional slit lamp. Previous studies have been conducted in Japan. We wanted to further compare ocular examination between SEC and conventional slit lamp with samples taken from UPTD Eye Clinic Ambon Vlissingen in Indonesia.</p><p><strong>Methods: </strong>Observational analytical study with a cross-sectional study design. The research sample consisted of 67 people selected by consecutive sampling. All samples were examined for cataracts using a conventional slit lamp and SEC. To evaluate the accuracy of SEC, diagnosis of cataract was carried out by one observer based on slit-lamp images. The results of examinations were then compared with the examinations by two observers based on the SEC video, where the videos were read two times in a span of 1 week.</p><p><strong>Results: </strong>Sixty-seven eye samples were taken with average age of 61.76 ± 5.83 years, visual acuity was 0.07 ± 0.12, intraocular pressure was 14.12 ± 2.06 mmHg, and SEC video duration was 4.15 ± 1.09 seconds. SEC intraobserver reliability has a κ of 0.795/0.818, SEC interobserver reliability has a κ of 0.795/0.817. SEC assessment accuracy for observer 1 has a κ of 0.606/0.681 and for observer 2 has a κ of 0.629/0.717, with the highest accuracy for cataract grade 3 (86.67% and 91.18%, respectively).</p><p><strong>Conclusions: </strong>This study shows SEC has good consistency and reliability in assessing the degree of nuclear cataract compared to conventional slit lamp.
