Oudet","doi":"10.1016/S0035-2845(72)80001-5","DOIUrl":"10.1016/S0035-2845(72)80001-5","url":null,"abstract":"<div><p>Des tests ventilatoires de contrôle ont été pratiqués chez 40 asthmatiques, allergiques à a poussière de maison, désensibilisés et suivis cliniquement pendant une période variant de 6 mois à 5 ans ; 22 d'entre eux avaient moins de 25 ans. Dans les 2/3 des cas, les malades avaient d'autres réactions cutanées positives, témoignant du contexte atopique. Les injections de désensibilisations étaient pratiquées toutes les semaines, puis tous les 15 jours, enfin tous les mois. La dose maxima atteinte est de 0,50 ml au 1/500.</p><p>Parmi les 17 malades améliorés cliniquement par le traitement et ayant atteint la concentration au 1/500 depuis plus de 1 an, 9 ont une augmentation très significative de la dose liminaire d'allergène, et 7 gardent le même seuil.</p><p>Les traitements dont la durée globale est supérieure à 3 ans ne s'accompagnent que rarement d'une réduction des tests cutanés, et l'amélioration clinique n'est pas toujours objectivée par une augmentation de la tolérance bronchique. Celle-ci est surtout nette chez les malades ayant eu la dose maxima pendant une longue période (plus que 2 ans).</p><p>Chez les malades dont le traitement global est inférieur à 2 ans le test ventilatoire de contrôle est souvent identique au test initial bien que certains soient améliorés cliniquement et aient atteint la concentration de 1/500.</p></div><div><p>Follow-up ventilation tests were carried out in 40 asthmatics with an allergy to house dust, who were desensitized and clinically observed over periods ranging from 6 months to 5 years ; 22 of them were less than 25 years old. In 2/3 of cases the patients showed other positive skin tests indicating an atopic constitution. The desensitizing injections were initially given weekly, then every 15 days and finally once a month. The maximum dose attained was 0,50 ml of 1/500.</p><p>Of 17 patients showing clinical improvement in response to treatment and attaining a concentration of 1/500 for over a year, 9 showed a significant increase in the liminal allergen dose ; in 7 it remained unchanged.</p><p>Treatment, generally lasting more than 3 years, rarely caused a reduction in skin reactions, and clinical improvement was not always verified by improved bronchial tolerance. Pineau","doi":"10.1016/S0035-2845(72)80005-2","DOIUrl":"10.1016/S0035-2845(72)80005-2","url":null,"abstract":"<div><p>La méthode isotopique permet l'interprétation plus précise du Test de Transformation Lymphoblastique (TTL).</p><p>Les résultats expriment une croissance cellulaire, sous forme du rapport R du nombre d'impulsions par minute de l'antigène à celui des impulsions des cultures sans antigène (témoin).</p><p>101 TTL ont été interprétés par cette méthode.</p><p>Dans l'hypersensibilité médicamenteuse, le TTL permet de détecter le ou les antigènes responsables.</p><p>Dans l'hypersensibilité au streptocoque hémolytique, on a relevé une concordance dans 7 cas sur 8 entre les tests cutanés et le TTL à la protéine.</p><p>Dans l'hypersensibilité aux hyménoptères, lorsque l'interrogatoire est insuffisant à révéler l'insecte piqueur, le TTL a permis de déceler l'antigène responsable.</p><p>En fin, dans les affections qui s'accompagnent d'une perturbation de l'immunité cellulaire : on note un effondrement de la réponse des lymphocytes en présence de phytohémagglutinine (PHA) et de Tuberculine. Ce fut le cas d'un malade atteint de maladie de Hodgkin associée à une leucémie lymphoïde chronique.</p><p>Le TTL a encore permis de démontrer la présence d'anticorps anti-nucléaires chez une malade suspecte de collagénose.</p></div><div><p>The blast transformation test can be interpreted mcre accurately using the isotope method.</p><p>Results are expressing cellular growth as the relation R of the number of per minute impulses of the antigen to the number of impulses of cultures free of antigen (control).</p><p>One hundred and one blast transformation test have been interpreted with this method.</p><p>In hypersensitivity to drugs, the responsible antigen or antigens can be detected with the blast transformation test.</p><p>In hypersensitivity to Streptococcus haemolyticus, skin tests and the blast transformation test to protein M correlated in 7 out of 8 cases.</p><p>In hypersensitivity to Hymenoptera, the responsible antigen could be detected with the blast transformation test whenever patient's interview failed to reveal the causative insect.</p><p>Finally, in diseases presenting with modified cell immunity, a spectacular fall in the response of lymphocytes to phytohaemagglutinin and tuberculin was noted. Vialatte","doi":"10.1016/S0035-2845(72)80012-X","DOIUrl":"10.1016/S0035-2845(72)80012-X","url":null,"abstract":"<div><p>Sur un total d'environ 1 500 enfants allergiques, une sélection rigoureuse montre que l'allergie à Candida albicans intéresse 11 p. cent des 822 cas où elle a été recherchée activement, et le 1/5<sup>e</sup> des cas où le test à cette levure a été trouvé positif. Cette allergie est bien moins fréquente, et sensiblement plus tardive dans l'enfance, que l'allergie à la poussière de maison.</p><p>Les syndromes observés diffèrent peu des manifestations les plus courantes de l'allergie infantile. Certaines associations séméiologiques particulières rappellent parfois le polymorphisme clinique de cette hypersensibilité chez l'adulte. La séquence habituelle eczéma-asthme n'est pas observée dans le même ordre et dans les mêmes délais que chez les enfants atteints d'autres sensibilisations.</p><p>En fait, le caractère clinique le plus spécifique de l'allergie infantile à Candida albicans est l'absence de facteur géographique ou saisonnier, ou même l'existence d'un maximum entre la fin de l'hiver et la fin de l'été. Le diagnostic sera surtout posé en fonction de la cohérence des réponses aux injections d'essai. Les tests cutanés ne sont presque d'aucun secours : ils n'orientent guère le diagnostic et ne fournissent aucune information sûre pour le calcul des doses allergéniques efficaces. Les schémas de désensibilisation sont variables, tantôt fixes, tantôt progressifs. Les modalités de ces traitements, qui sont souvent difficiles, sont analysées et discutées.</p></div><div><p>Thorough screening of a total of about 1 500 children showed that allergy to Candida albicans affected 11 per cent of the 822 cases in which it was actively sought, and 1/5 of the cases in which tests using this yeast were found to be positive. This allergy is much less frequent and occurs much later on in childhood than does allergy to house dust.</p><p>The syndromes observed were very similar to the usual symptoms in infantile allergy. Certain special semeiological associations sometimes appeared reminiscent of the clinical polymorphism of this hypersensitivity in adults. The usual eczema-asthma sequence is not found in the same order and over the same period as in children affected by other sensitizations.</p><p>In fact, the most specific clinical characteristic of infantile allergy to Candida albicans is the absence of a geographical or seasonal factor, or even the existence of a peak period between the end of winter and the end of summer. Diagnosis must of course depend on the coherence of the reactions to test injections. Skin tests are of practically no use : they give no guide to diagnosis and no accurate information from which to calculate effective allergenic dosage. Desensitization diagrams vary : they are sometimes fixed, sometimes progressive.
