Le dosage de l'alpha-1-antitrypsine révèle l'existence d'un déficit (120 mg/100 ml).</p><p>Cette malade a deux filles âgées de 7 ans et de 10 ans. Le dosage de l'alpha-1-antitrypsine chez l'aînée est normal (210 mg/100 ml), par contre la cadette présente un taux abaissé à 105 mg/100 ml.</p><p>L'étude du phénotype montre que la mère et la cadette sont M Z (hétérozygotes), la fille aînée est M M. La mère est issue d'une famille de 14 enfants, parmi lesquels deux frères porteurs de la tare sont décédés d'une insuffisance respiratoire aiguë, à 38 et 42 ans. D'autres membres de cette famille présentent actuellement un emphysème associé à cette tare, et il existe des sujets apparemment sains. Enfin, les parents de ces 14 enfants sont consanguins, et la mère est porteuse de la tare, mais ne présente aucun trouble respiratoire. La tare est donc retrouvée sur trois générations.</p><p>La conduite à tenir sur le plan thérapeutique n'est pas définie.</p></div><div><p>The existence of a seric deficiency in alpha-1-antitrypsin in certain chronic bronchopneumopathies now constitutes a definite clinical entity.</p><p>The familial or individual nature of this deficiency is revealed by the early onset of a respiratory insufficiency.</p><p>The case which we describe involves a 34-year-old woman suffering from pollinic asthma for six years but having no discernible pulmonary lesion. Assessment of alpha-1-antitrypsin showed a deficiency (120 mg/100 ml).</p><p>This patient has two daughters aged seven and ten. They show no particular antecedent and a somatic examination was perfectly normal. The amount of alpha-1-antitrypsin in the elder was within the normal range (210 mg/100 ml), the younger girl however had a reduced amount (105 mg/100 ml). A study of the phenotype showed the mother and the younger girl to be MZ (heterozygotes), the older daughter MM.</p><p>The mother was one of 14 children with two brothers with emphysema associated with a deficiency in alpha-1-antitrypsin which manifested itself before the age of thirty. One died at the age of thirty-eight and the other at forty-two. Other members of this family are at present suffering from emphysema connected with this defect and there are some of them who appear to be in good health.</p><p>Finally, the parents of these fourteen children are consanguineous, the mother transmitting the defect but not herself manifesting any respiratory disorder. Parmi les facteurs déclenchants il faut insister sur les émotions. Neuf sujets explorés biologiquement sont porteurs de la tare biologique : absence de l'inhibiteur de la Cl-estérase, baisse du complément total et de la fraction C4, alors que la fraction C3 a un taux normal. Deux sujets ne présentent pas de signes cliniques caractéristiques.</p><p>La transmission se fait selon un mode autosomal dominant sans que nous ayons pu démontrer de linkage entre cette tare et les groupes érythrocytaires et tissulaires étudiés.</p><p>Au plan thérapeutique, corticoïdes, acide epsilon aminocaproïque et alpha-2-macroglobuline humaine purifiée se sont montrés inactifs. Seul le plasma frais congelé a été régulièrement efficace.</p></div><div><p>A family was investigated in which 14 cases of hereditary angioneurotic oedema have been found in the space of four generations. Among the precipitating factors, emotional factors must be stressed. Nine subjects investigated biologically were found to have the biological defect : absence of the inhibitor of CI-esterase, a reduced total amount of the C4 fraction and normal amounts of the C3 fraction. Two of them exhibited no characteristic clinical symptoms.</p><p>Transmission occurred by the dominant autosomal method although we have been unable to demonstrate any links between this defect and the erythrocytic and tissular groups studied.</p><p>From the therapeutic point of view, corticoids, epsilon amino-caproic acid and purified human alpha-2-macroglobulin proved ineffective. Un appareil de Durham a été placé sur le toit de l'Hôpital Thermal pendant ces trois années.</p><p>Sur le plan de l'aéropalynologie, les espèces les plus allergisantes, responsables de pollinoses de printemps, telles que les <em>Graminées</em>, sont largement représentées, la pollinsation est abondante et de courte durée. Par contre, les pollens responsables de pollinoses estivo-automnales en particulier les Composées ne sont retrouvées qu'en très faible quantité, à l'inverse des régions de plaine.</p><p>Les moisissures sont toujours nombreuses et diverses au Mont-Dore et prédominent pendant la période estivo-automnale. Un essai d'inventaire du contenu fongique de l'atmosphère du Mont-Dore a été réalisé. Cette reconnaissance est indispensable au médecin allergiste qui se trouve en présence de malades sensibilisés à l'un ou plusieurs de ces champignons allergisants.</p><p>Les calendriers de pollinisation et de sporulation permettront au médecin, selon la chronologie des accidents allergiques de reconnaître l'allergène responsable d'une part, et d'autre part, de pouvoir conseiller au patient allergique la période optimale de la cure thermale.</p></div><div><p>Regular counts of the pollens and moulds in the atmosphere have been carried out at Mont-Dore over three years, from 1969 to 1971, by the gravimetric method. Durham's apparatus was exposed on the roof of the Hôpital Thermal during those three years.</p><p>From the aeropalynological point of view, the species responsible for most allergies and spring hay-fever such as the Graminaceae, are well represented, pollinisation is abundant and of short duration. On the other hand, the pollens responsible for summer-to-autumn pollinoses, in particular the Compositae, are only found here in very small quantities, unlike regions in the plain.</p><p>Moulds are always numerous and varied in Mont-Dore, and dominate the summer-to-autumn period. An attempt was made at an inventory of the fungic content of the atmosphere at Mont-Dore.
