All patients were divided into hematoma expansion and non-hematoma expansion group according to whether early hematoma expansion appeared; and they were also divided into positive imaging sign group and negative imaging sign group according to whether imaging signs appeared; the clinical and imaging data were compared between these groups, respectively. The accuracies of swirl sign and black hole sign in predicting early hematoma expansion were analyzed using receiver operator characteristic (ROC) curve. Multivariate Logistic regression analysis was performed to determine the independent risk factors for early hematoma expansion. \u0000 \u0000 \u0000Results \u0000(1) In the 57 patients with early hematoma expansion, 21 (36.8%) had swirl sign, and 17 (29.8%) had black hole sign; in the 153 patients without hematoma expansion, 12 (7.8%) had swirl sign and 22 (14.4%) had black hole sign; the differences between the two groups were statistically significant (P 0.05); the hematoma volume in patients with black hole sign (n=39) was significantly increased as compared with that in patients without black hole sign (n=171, P 0.05). (3) The areas under ROC curve of swirl sign, black hole sign, and \"swirl sign combined with black hole sign\" were 0.645, 0.577, and 0.570, respectively. And miR-con and miR-146a-5p were transfected into SK-N-SH cells and treated with 0.3 mmol/L MPP+, and named MPP++miR-con group and MPP++miR-146a-5p group; anti-miR-con and anti-miR-146a-5p were transfected into SK-N-SH cells and pretreated with 20 μmol/L coptisine for 4 h, and then, treated with 0.3 mmol/L MPP+, and named MPP++Cop+anti-miR-con group and MPP++Cop+anti-miR-146a-5p group. Cell viability was determined by MTT assay; Western blotting was used to detect the protein expressions of cleaved cysteine-containing aspartate-specific proteases-3 (caspase-3), CyclinD1, phosphorylated protein kinase B (p-AKT), p-phosphoinositide 3 kinase (p-PI3K); apoptosis was detected by flow cytometry; real-time quantitative PCR (RT-qPCR) was used to detect the miR-146a-5p expression. \u0000 \u0000 \u0000Results \u0000As compared with the NCs, the MPP+ induced SK-N-SH cells had significantly decreased viability and CyclinD1 and miR-146a-5p expressions, and significantly increased cleaved caspase-3 expression and apoptosis rate (P<0.05). After coptisine treatment and miR-146a-5p overexpression, MPP+ induced SK-N-SH cells had significantly increased viability and expressions of CyclinD1 and miR-146a-5p, and significantly decreased cleaved-caspase-3 expression and apoptosis rate (P<0.05). Low expression of miR-146a-5p reversed the effect of coptisine on proliferation promotion and apoptosis inhibition of SK-N-SH cells. The expression levels of p-AKT and p-PI3K in MPP+-induced SK-N-SH cells were significantly increased after coptisine treatment. General information, laboratory examination results, chest CT and other imaging results, hospital stay, antibiotic/ventilator use, tracheotomy and other treatments of these patients were collected and compared to identify the independent factors influencing pulmonary infection in patients with severe neurological disease. \u0000 \u0000 \u0000Results \u0000There was a statistically significant difference in incidence of pulmonary infection among patients with different diseases (P<0.05), and the infection rate of patients with cerebral hemorrhage was the highest, reaching to 50.8% (32/63). Shi","doi":"10.3760/CMA.J.ISSN.1671-8925.2020.01.010","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1671-8925.2020.01.010","url":null,"abstract":"Objective \u0000To summarize the adverse reactions of blepharospasm (BSP) and hemifacial spasm (HFS) after botulinum toxin A (BoNT-A) injection and offer corresponding responses. \u0000 \u0000 \u0000Methods \u0000Clinical data of 163 BSP patients and 221 HFS patients, admitted to and subjected to BoNT-A injection in our hospital from September 2017 to October 2019, were analyzed retrospectively. Albert and Cohen evaluation scale was used to evaluate the therapeutic efficacy after injection. All adverse reactions were recorded and corresponding responses were proposed. \u0000 \u0000 \u0000Results \u0000The effective rates of BoNT-A injection for BSP and HFS were 97.5% and 100%, respectively. The most common early-onset adverse reactions were local pain (BSP: 13.5%; HFS: 17.2%) and hematoma (BSP: 3.7%; HFS: 4.5%); the late-onset adverse reactions included dry eyes/epiphora (BSP: 9.8%; HFS: 11.3%), incomplete closure of eyelid (BSP: 20.2%; HFS: 20.4%), facial stiffness (HFS: 39.4%) and asymmetry (HFS: 11.8%), and eyelid edema could be noted in a few patients (BSP: 1.2%; HFS: 1.4%). Guo, Junlong Huang, Tianqin Liu","doi":"10.3760/CMA.J.ISSN.1671-8925.2020.01.008","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1671-8925.2020.01.008","url":null,"abstract":"Objective \u0000To analyze the clinical features and risk factors of delayed intracranial hemorrhage (DICH) after ventriculoperitoneal shunt (VPS) in patients with communicating hydrocephalus. \u0000 \u0000 \u0000Methods \u0000One hundred and seventy-six patients with ventriculoperitoneal shunt due to communicating hydrocephalus secondary to craniocerebral trauma, hypertensive intracerebral hemorrhage, brain tumor or intracranial aneurysm rupture hemorrhage, admitted to our hospital from January 2012 to August 2018, were selected in our study; these patients were divided into DICH group and non-DICH group according to whether or not DICH occurred. The clinical features, including incidence, time and location of DICH, were analyzed. The differences of age, gender, length of stay, concomitant diseases, previous operation history, incidences of subdural effusion and puncture canal edema after ventriculoperitoneal shunt, and history of down-regulating shunt valve within 2 weeks between the two groups were compared by univariate analysis. The independent risk factors for DICH were further assessed using multivariable Logistic regression. \u0000 \u0000 \u0000Results \u0000Among 176 patients, 23 (13.07%) had DICH; 2-11 d after surgery, DICH appeared, manifesting as subdural, ventriculoventricular end canal and/or hemorrhage in one or more areas of the ventricle. There were significant differences in incidence of subdural effusion and history of down-regulating shunt valve within 2 weeks between the two groups (P<0.05). The reason for its occurrence is caused by lack of control and recombination of pyramidal system and extrapyramidal nerve fibers, and often leads to increased tension and hyperreflexia of affected muscle groups. For patients with chronic disorders of consciousness (DoC) after brain injury, the occurrence of such symptoms is very common, but effective treatment is rarely reported. The current treatment mainly includes intrathecal injection of baclofen and soft splint to solve joint contracture. Other therapeutic schemes still need further research, including acupuncture, botulinum toxin injection, and deep brain electrical stimulation.
