Information was gathered by means of a retrospective questionnaire. The register entered 1130 patients and 9437 plasma exchange procedures. The most frequent treatment indications were neurologic conditions, vasculitis, and hematologic diseases. Cell separation technique was used in two thirds of the sessions, plasma-filtration in one third. Albumin as a replacement fluid was utilized alone or associated with other solutions in over 90% of cases. There were no complications in 60% of the patients. The most frequent complications were circulatory collapse, allergic manifestations, and febrile reactions. In nearly all cases, PE was added to a previously ineffective therapeutic regimen of corticosteroids plus cytotoxic drugs. An improvement in muscle strength was obtained during the PE course in 26 cases (63.4%), especially in diseases with an acute course. Despite their uncontrolled nature, these results confirm the previous publications of P. These included the recycling of disposable sets and their safe, repeated use on over 8000 runs carried out over the past four years. The standardization of the operation of the blood cell separator proved to be a very safe application. Five patients had diffuse scleroderma and one had CREST syndrome (calcinosis, Raynaud's phenomenon, esophageal dysmotility, sclerodactyly, and telangiectasia). All patients had failed to respond to drug therapy; three had worsening cardiopulmonary involvement, three had skin ulcers, and one had myositis. The treatment protocol was for a 40% plasma exchange three times per week for the first three to four weeks, to be slowly tapered in a two-to ten-week period. The replacement solution was 4% albumin in polysaline. Corticosteroid dosage was continued unchanged or reduced during the period of exchange in three patients. In one patient, PE was discontinued because of insufficient venous access. In the five patients who completed the protocol, a transient improvement of general status, skin thickness, and (whenever present) of flexion contractures were observed after PE. Skin ulcerations healed or improved, but cardiopulmonary function was not improved. Despite a course of short-term PE, four of six patients died within two years. The description of immunologic and biochemical disturbances prompted several authors to use extracorporeal methods for treatment of this disease.</p><p>Plasma exchange gave only partial and brief remissions, and a controlled study noted no beneficial effect from sham and true apheresis. Leukocytapheresis may lead to more significant remissions, but no controlled studies have been performed.</p><p>The results concerning dialysis therapy remain conflicting, and controlled studies have reached opposing conclusions. The attempt to recruit and identify donors in minority populations has often been inefficient and has risked offending individuals in the targeted group. A successful and cost-efficient method for identifying blood donors with rare red cell phenotypes has been developed and has attracted large numbers of donors to participate with a minimum of potential problems. Oklahoma has a large Native American population, and a large percentage of blood donors are Indians. In this study we obtained information concerning the donor's racial background. More than 4000 blood donors representing 30 different Indian tribes participated in the program. Samples of blood from the donors were screened for a variety of red cell antigens. Four Di(a + b −) and four R<sub>z</sub>R<sub>z</sub> were identified. Most participants in the study were from the Cherokee, Creek, and Choctaw tribes. The red cell phenotypes in these tribes are reported and compared to previous studies.
