Atsmon","doi":"10.1016/S0278-6222(87)80060-8","DOIUrl":"10.1016/S0278-6222(87)80060-8","url":null,"abstract":"<div><p>Familial homozygous hypercholesterolemia is characterized by markedly increased cholesterol levels ranging between 600 and 1200 mg/dL and premature atherosclerotic morbidity and mortality. Various dietetic, medical, and surgical therapeutic regimens have been tried, with limited success.</p><p>Herein we describe two women homozygous hypercholesterolemic patients born in 1962 and 1971, who underwent intermittent-flow plasma exchange at three-week intervals for almost two years.</p><p>A marked decrease was observed in skin and tendon xanthomata, and the cardiac status of the patients remained stable. The older patient, who had effort angina pectoris prior to plasma exchange, became symptom-free after one year of treatment.</p><p>The procedure was well tolerated with no major side effects. Conté MD","doi":"10.1016/S0278-6222(87)80018-9","DOIUrl":"10.1016/S0278-6222(87)80018-9","url":null,"abstract":"<div><p>Eight patients with severe progressive systemic sclerosis were treated with plasma exchange accompanied by cyclophosphamide and/or captopril, a converting enzyme inhibitor. All patients presented with visceral involvement of heart, lung, or kidney, and/or extensive skin lesions with tissue necrosis. Raynaud's syndrome improved in all patients after plasma exchange, and all patients showed an objective improvement in their most prominent clinical features, resulting in reduction of heart failure, improvement in lung function tests, decrease in creatinemia, and healing of ulcers. These results suggest the use of plasma exchange either with short courses of intensive exchanges for vasculitis or acute kidney or heart involvement, or with longer courses of regular exchanges for pulmonary fibrosis. The well-demonstrated role of autoantibodies and the side effects of conventional therapy with high doses of steroids prompted trials of plasma exchange (PE). While most open studies reported effectiveness of PE, mainly in steroid-resistant cases, a recent controlled study showed no benefit from PE when given with a low steroid dose. This study points to the difficulty of removing pemphigus antibody unless PE is accompanied by effective immunosuppression.
