The lower end was designed with a groove about 1 cm in length, the width of the lower end of the butterfly graft was determined according to the degree of retraction at the caudal end of the septum, then insert to the nasal crest. The posterior end of two lamella grafts was sutured and fixed with the middle part of the nasal columnar strut graft. \u0000 \u0000 \u0000Results \u0000From November 2016 to March 2018, about 150 patients underwent rhinoplasty with this method, follow-up was 3 to 12 months. Two cases had mild nasal tip deviation one month after surgery and were adjusted with auricular cartilage three months later. All patients with epicanthus were treated with the same internal canthus correction method by one doctor. There were 30 patients in each group: 30 cases in the control group were treated without any medication, and the postoperative incision healed naturally; 30 cases were treated with Dermatix Ultra silica gel; 30 cases were treated with asiaticoside cream. Patient and observer scar assessment scale (POSAS) was used as the end points to evaluate the clinical efficacy of Dermatix Ultra silica gel and asiaticoside cream 4 weeks and 12 weeks after the operation. The satisfactory degree from all the patients were collected after 6 months follow-up and we compared the scores of three groups. \u0000 \u0000 \u0000Results \u0000Compared with the control group, the POSAS scores in Dermatix Ultra silica gel group and asiaticoside cream group were low (P 0.05). Follow-up for 6 months, the satisfaction degree of patients in Dermatix Ultra silica gel group and asiaticoside cream group was higher than in the control group (P<0.05); the satisfactory degree of patients in Dermatix Ultra silica gel group was higher than that in asiaticoside cream group (P<0.05). \u0000 \u0000 \u0000Conclusions \u0000Dermatix Ultra silica gel and asiaticoside cream have good clinical effects in repair of scar tissue after the epicanthoplasty. The clinical data were collected such as general medical history and treatment methods, and follow-up for 1 year. \u0000 \u0000 \u0000Results \u0000All patients were female, 22-41 years old, with average age of 33. All patients were injected with hyaluronic acid. For 8 patients the fillers were injected in the forehead, 3 patients were in the glabellar region, 3 patients were in the nasolabial fold, and 1 patient was in the temporal of left eye. All patients had vision lose after injection, the visiting time was 1 to 6 hours. 13 patients were central retinal artery occlusion (CRAO). 1 patient was retinal branch artery occlusion (BRAO), 1 patient was ischemic optic neuropathy (ION), 13 patients manifested as no light perception (NLP), 1 patient was 0.6, 1 patient was CF/30 cm, and 14 patients with cerebral infarction, manifested as headache, dizziness. All patients were given emergency treatment, and 9 patients had treated with interventional thrombolysis therapy. After treatment 11 patients, visual acuity had no significant improvement, but 4 patients improved. Headache, dizziness symptoms disappeared, but cerebral infarction lesions still existed on MRI. \u0000 \u0000 \u0000Conclusions \u0000Human face is a rich blood supply; vision loss and cerebral infarction could occur after injection of hyaluronic acid. Flaps were designed and harvested according to the location and size of the midfacial defect resulted from tumor resection, including propeller flaps, droplet flaps and swallowtail-shape flaps. Flap size was between 1.3 cm × 1.6 cm and 2.0 cm × 4.2 cm with the donor site being sutured in the first stage. \u0000 \u0000 \u0000Results \u0000During the 6 months to 3 years of postoperative follow-up, no tumor recurred. All the 36 cases of individualized designed facial artery perforator flaps survived well. Furthermore, flaps being designed according to the wound defect had a good appearance, the flaps matched the color and texture of the surrounding skin and had a favorable appearance. Group A (28 cases) were only treated with buried polydioxanone threads for nasolabial fold filling; Group B (36 cases) were treated with injected autologous fat and buried polydioxanone threads for nasolabial fold filling. The surgical effect was recorded for 18 months. \u0000 \u0000 \u0000Results \u0000The most common complications were swelling, bruising and pain immediately after the surgery. Polydioxanone threads exposure occurred in two cases in each group 2 weeks after the surgery. Double eyelid line was designed according to the overall characteristics of eyebrows and eyes of beauty seekers combined with aesthetic standards. The surgery was performed to restore the anatomical characteristics of natural double eyelids as far as possible. \u0000 \u0000 \u0000Results \u0000The double eyelid surgery was designed and performed in 162 cases based on this method. All the patients underwent suture removal 7 days after operation, none of them had postoperative infection, and 15 patients had the bruising during the first week. After symptomatic treatment, the bruising had gone in 1 month. All the cases were followed up for more than six months postoperatively, and eight cases presented shallow double eyelid and the satisfactory results were achieved after the second operation of small incision. \u0000 \u0000 \u0000Conclusions \u0000The double-eyelid line should be designed individually before operation and the surgery done according to the natural characteristics of Asian upper eyelid. {"title":"Botulinum toxin A poisoning by local aesthetic injection: clinical and electrophysiological analyses of 20 cases","authors":"X. Jin, Huaying Cai, Wentao Lyu, Hong Jiang","doi":"10.3760/CMA.J.ISSN.1671-0290.2019.06.013","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1671-0290.2019.06.013","url":null,"abstract":"Objective \u0000To observe the clinical and electrophysiological characteristics of botulinum toxin A poisoning induced by local cosmetic injection, and to explore the possible pathogenesis. \u0000 \u0000 \u0000Methods \u0000Clinical data of 20 patients with botulinum toxin type A local cosmetic injection poisoning admitted to our hospital from January 2016 to December 2018 were collected and electrophysiological tests were carried out. \u0000 \u0000 \u0000Results \u0000The clinical manifestations of 20 patients were mainly 15/20 (75.0%) with limb weakness, and the proximal end was heavier than the distal 20/20 (100%), and the upper limb was heavier than the lower limb 26/20 (80%), followed by dysphagia 12/20 (60%), dysarthria 5/20 (25%), cervical muscle weakness 5/20 (25%), extraocular muscle paralysis 4/20 (20%), masseter muscle weakness 3/20 (15%) and respiratory muscle weakness 2/20 (5.0%) with respiratory failure 1/20 (5.0%). The results of neurophysiological examination showed the highest rate of SFEMG (90.0%), followed by RNS (80.0%), EMG (65.0%) and NCV (40.0%), suggesting that neuromuscular junction dysfunction was the main presentation. Patients who actively gave botulinum antitoxin injection therapy and symptomatic support treatment had good prognosis. \u0000 \u0000 \u0000Conclusions \u0000Local cosmetic injection of botulinum toxin type A may lead to poisoning. The symptoms of poisoning are mainly caused by limb weakness. Five indexes representing the symmetry of lip and nose were measured by photoshop software. T test was used to compare the measurement results of each angle or distance index at different times. P<0.05 showed that there was significant difference between the two groups. \u0000 \u0000 \u0000Results \u0000All the patients who were treated with the technique described in this study obtained satisfactory lip and nasal symmetry immediately and one-year follow-up. \u0000 \u0000 \u0000Conclusions \u0000The new theory of oral muscle strength balance is a new breakthrough on the basis of the application of muscle reconstruction. In 104 cases of hospitalized patients with herpes zoster, 46 cases had high blood pressure (hypertension group); 58 cases did not have high blood pressure (non-hypertension group). The skin lesion, numerical rating scale (NRS) and blood pressure changes were compared between the two groups before and after treatment. \u0000 \u0000 \u0000Results \u0000A total of 99 patients completed treatment; 45 cases in the hypertensive group included 25 males, 20 females, aged 35 to 71 (52.40±12.51) years; 54 cases in the non-hypertensive group included 30 males and 24 females, aged 36 to 75 (52.62±12.67) years. There was no statistically significant difference in the time of control, improving and healing of skin lesion (t=-0.457, -0.607 and 1.399, P>0.05) . There was no statistically significant difference in NRS between the two groups before treatment (t=-0.009, P=0.993 >0.05). After treatment, the NRS of both groups decreased compared with those before treatment, and the difference was statistically significant (F=151.130, 181.444, P 0.05). In the hypertension group, the rate of blood pressure increase before treatment was 62.22%, and the blood pressure decreased after treatment compared with that before treatment. The difference was statistically significant (P 0.05). \u0000 \u0000 \u0000Conclusions \u0000Blood pressure can be increased when herpes zoster occurs. After early formal treatment of herpes zoster, skin lesion healed, neuralgia reduced and blood pressure can return to normal. Yao","doi":"10.3760/CMA.J.ISSN.1671-0290.2019.06.003","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1671-0290.2019.06.003","url":null,"abstract":"Objective \u0000To investigate the clinical effect of conjoint fascial sheath (CFS) suspension to correct the complications caused by the frontal muscle flap suspension and levator palpebrae superioris shortening in the treatment of severe blepharoptosis. \u0000 \u0000 \u0000Methods \u0000From February 2017 to April 2018, 19 severe blepharoptosis patients (21 eyelids) were operated before by the frontal muscle flap suspension and levator plapebrae superioris shortening, and repaired through the technology of CFS suspension. \u0000 \u0000 \u0000Results \u0000There were 19 cases, 17 cases operated by CFS suspension showed a good appearance and basically symmetrical of two eyes, and the other 2 cases obtained satisfactory results though reoperation. There were no complications of exposure keratitis, ectropion and infection occurred.
