L’interrogatoire et l’examen clinique visent à identifier des arguments en faveur d’une origine secondaire dont la prise en charge et le pronostic diffèrent de la forme idiopathique.</div><div>L’acrocyanose est un acrosyndrome fréquent et bénin de la femme jeune mince. La prise en charge doit notamment comprendre celle des éventuelles complications associées (engelures, hyperhidrose et impact socioprofessionnel).</div><div>L’érythermalgie (ou érythromélalgie) est une affection parfois invalidante. Des traitements symptomatiques existent. Néanmoins, en cas d’érythermalgie secondaire, l’identification et le traitement de l’affection associée sont indispensables.</div><div>L’acrorighose est un symptôme très fréquent et toujours bénin.</div><div>Les engelures sont responsables d’un acrosyndrome fréquent, le plus souvent bénin. La survenue d’engelures en l’absence d’exposition au froid doit faire chercher une affection associée.</div><div>L’hématome spontané digital est bénin, la prise en charge est principalement antalgique.</div></div><div><div>Raynaud's phenomenon is a common acrosyndrome. It is benign and affects young people in most cases. The history and clinical examination aim to identify arguments for a secondary origin because management and prognosis may differ from the idiopathic form.</div><div>Acrocyanosis is a common and benign acrosyndrome in young and slender women. Patient care must include possible associated complications (frostbite, hyperhidrosis and socio-professional impact).</div><div>Erythromelalgia can be a disabling pathology. Symptomatic treatments are available. However, for secondary erythromelalgia cases, identification and treatment of the associated pathology are essential.</div><div>Acrorighosis is a very common and always benign symptom.</div><div>Chilblains are a common and mostly benign acrosyndrome. Plus de 3 millions de gènes microbiens codant des milliers de métabolites ont été identifiés dans le microbiome intestinal humain. La composition du microbiote intestinal varie quantitativement et qualitativement en fonction des régions anatomiques de l’intestin. Malgré l’environnement acide de l’estomac, le microbiote gastrique est également complexe et diversifié. Le microbiote intestinal est acquis suite à la colonisation du tube digestif pendant les deux années qui suivent la naissance. La communauté microbienne typique qui s’établit chez l’adulte, se caractérise par une grande diversité, une relative stabilité, une capacité à la résilience aux perturbations et une interaction symbiotique avec l’hôte. Le déséquilibre des populations de la flore intestinale ou dysbiose est associé au développement de nombreuses pathologies digestives. La diversité élevée du microbiote gastrique dans l’estomac sain diminue chez les patients atteints de gastrite chronique atrophique, de métaplasie intestinale et de cancer gastrique, la progression du cancer étant corrélée à cette diminution de la diversité. Cette dysbiose, associée au cancer gastrique, se caractérise par un enrichissement en communautés microbiennes à potentiel nocif capables de générer des métabolites favorisant le développement de la tumeur, la modulation de l’inflammation, l’induction de dommages à l’ADN, la suppression de l’immunité anti-tumorale et l’activation des voies de signalisation oncogéniques. La thérapie par modulation du microbiote nécessite de définir l’équilibre de la flore intestinale avec précision et de bien identifier les communautés microbiennes liées au développement du cancer gastrique.&lt;/div&gt;&lt;/div&gt;&lt;div&gt;&lt;div&gt;The microbiota hosted by the gastrointestinal tract, called the gut microbiota or intestinal flora, is the primary human microbiota. Over 3 million microbial genes encoding thousands of metabolites have been identified in the human gut microbiome. The composition of the gut microbiota varies quantitatively and qualitatively according to the anatomical regions of the gut. Despite the acidic environment of the stomach, the gastric microbiota is also complex and diverse. The gut microbiota is acquired following the colonization of the gastrointestinal tract during the first two years after birth. The typical microbial community that establishes in adults is characterized by high diversity, relative stability, resilience to disturbances and symbiotic interaction with the host. {"title":"Stress post-traumatique en périnatalité : prévention, diagnostic et prise en charge","authors":"Sylvie Viaux-Savelon ,&nbsp;Corinne Dupont","doi":"10.1016/j.lpmfor.2024.08.003","DOIUrl":"10.1016/j.lpmfor.2024.08.003","url":null,"abstract":"<div><div>Le trouble de stress post-traumatique (TSPT) est une pathologie fréquente en période périnatale, il concerne environ 3,3 % des femmes pendant la grossesse et 4 % supplémentaires en post-partum et 2 à 5 % des hommes.</div><div>Le TSPT périnatal a des conséquences sur les interactions précoces parents-enfants et à long terme sur les couples et leurs enfants, en affectant leur développement émotionnel et comportemental.</div><div>Les facteurs de risque principaux sont les antécédents de traumatismes dans l’enfance, les complications obstétricales, le manque de soutien social et une perception négative de l’accouchement.</div><div>Le traitement consiste en une prise en charge psychothérapeutique, les thérapies cognitivo-comportementales (TCC), l’EMDR et les traitements médicamenteux, généralement des inhibiteurs sélectifs de la recapture de la sérotonine (ISRS).</div><div>Des axes de prévention sont possibles : la sensibilisation et la formation des professionnels, l’information prénatale auprès des futurs parents et postnatal, le soutien social et les interventions précoces pour les parents à risque.</div><div>Concernant le TSPT consécutif à l’accouchement, les conséquences peuvent être atténuées par une reprise, la plus précoce possible, par les professionnels de ces évènements vécus comme traumatisant.</div></div><div><div>Post-traumatic stress disorder (PTSD) is a frequent pathology in the perinatal period, affecting around 3.3% of women during pregnancy and a further 4% post-partum, and from 2 to 5% of men.</div><div>Perinatal PTSD has consequences for early parent-child interactions and in the long-term for couples and their children, affecting their emotional and behavioural development.</div><div>The main risk factors are a history of childhood trauma, obstetric complications, lack of social support and a negative perception of childbirth.</div><div>Treatment consists of psychotherapy, cognitive behavioural therapy (CBT), EMDR and medication, generally selective serotonin reuptake inhibitors (SSRIs).</div><div>Prevention can be achieved through awareness raising and training for professionals, prenatal and postnatal information for expectant parents, social support and early intervention for parents at risk.</div><div>In the case of PTSD following childbirth, the consequences can be mitigated by professionals dealing with these traumatic events, as early as possible.</div></div>","PeriodicalId":100859,"journal":{"name":"La Presse Médicale Formation","volume":"5 5","pages":"Pages 353-359"},"PeriodicalIF":0.0,"publicationDate":"2024-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"142418543","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
