Sung","doi":"10.15324/KJCLS.2020.52.3.271","DOIUrl":"https://doi.org/10.15324/KJCLS.2020.52.3.271","url":null,"abstract":"Received July 26, 2020 Revised August 15, 2020 Accepted August 20, 2020 “Health examinations” means medical examinations conducted by health examination institutions, including a physical examination, consultation, physical measurement, diagnostic laboratory test (also known as clinical laboratory test, clinical pathology test), pathology test, and Imaging test for checking the health condition and the prevention and early detection of diseases. The types of health examinations include general health examinations, comprehensive health examinations, special health examinations, and other health examinations. The proportion of the general health examination work of medical technologists working at health examination centers has been estimated to comprise more than 50% of the total work. The cost aspect of suspicious diseases can be estimated to be more than 25%. The cost of diagnostic laboratory test has been estimated to be approximately 5%, and the proportion of medical diagnosis use is 70%. The results show that it is time to revisit whether medical technologists are receiving appropriate social, economic, and reasonable policy treatment in health examination work. The future of medical technologists will further expand their professionalism in diagnostic laboratory test and quality control in precision medicine and telemedicine in the future. Therefore, medical technologists will need to participate proactively in government policy decisions related to national health examinations and make efforts to improve treatment. All the isolates were molds that could be identified morphologically, strains that are difficult to identify because of morphological similarities, and strains that require species-level identification. PCR and direct sequencing were performed to target the internal transcribed spacer (ITS) region, the D1/D2 region, and the β-tubulin gene. Comparative sequence analysis using the GenBank database was performed using the basic local alignment search tool (BLAST) algorithm. The fungi identified morphologically to the genus level were 67%. Sequencing analysis was performed on 62 genera and species level of the 65 strains. Discrepancies were 14 (21.5%) of the 65 strains between the results of phenotypic and molecular identification. B. dermatitidis, T. marneffei, and G. argillacea were identified for the first time in Korea using the DNA sequencing method. Morphological identification is a very useful method in terms of the reporting time and costs in cases of frequently isolated and rapid growth, such as Aspergillus. When molecular methods are employed, the cost and clinical significance should be considered. On the other hand, the molecular identification of molds can provide fast and accurate results. FC is also used to determine the effects of treatment and recurrence prediction because of its non-decomposition by bacteria, relative week stability at room temperature, and its uniform distribution within feces. Healthy male and female adults between the age of 30 and 80 living in Jeju were selected for this study. The FC concentration in the healthy control group (N=45) was distributed widely as 0∼545.9 μg/g and showed a significant difference with age in healthy adults. The FC concentration in adults over 70 years old (80.6 years on average) was 160.3 μg/g. The result is approximately 10 times higher than in adults below 50 years (44 years on average), with FC concentrations at 15.88 μg/g. Moreover, adults over 50 years, with an average age of 59.6, had FC concentrations of 35.46 μg/g, which were two times higher than the below 50-year-old group, confirming the significant correlation between age and FC concentration. As the FC test is a non-invasive and cost-effective objective marker in IBD tests, a suitable cut-off value is required for different ages. Distinguishing a mixed culture with non-fermenting bacteria having a similar appearance and oxidase-positive is difficult, and there is a limit to accurate identification with a biochemical identification system. This paper proposes that the Matrix-Assisted Laser Desorption/Ionization Time-of-Flight Mass Spectrometry Platform (MALDI-TOF) method is useful for classifying bacteria that are difficult to identify using biochemical testing methods. As a result of analyzing five cases of O. anthropi examined using MicroScan, it took 6.5 days to the final report, which was 3.5 days more than the 3.0 days of E. coli. The pus sample in patient 5 was a mixed infection with Achromobacter xylosoxidans, and it took 11.3 days because of multiple subculture and retests. Four patients were over 60 years old with an underlying disease, and the possibility of opportunistic and nosocomial infections could not be excluded. Among them, samples collected after 92 days of hospitalization were resistant to imipenem and meropenem. Therefore, an examination using the MALDI-TOF method will be useful for the rapid and adequate treatment of patients with difficult identification, such as O. anthropi.
