Scientific literature and international case studies offer many contributions of evidence relating to how therapeutic green spaces can support traditional treatments. The paper analyze the relationship between indoor and outdoor spaces and healing gardens' features; the main research questions were: \"Can we synthetize experience-based design strategies for therapeutic green spaces and healing gardens? How can we prioritize the most relevant ones for the healthcare infrastructures?\"</p><p><strong>Methods: </strong>Research Method is divided into three different steps: 1st case studies' selection; 2nd case studies' analysis, and 3rd quali-quantitative comparative matrix.</p><p><strong>Results: </strong>ten case studies were identifiedfour of them have the therapeutic green space on the ground floor,despite of the other six having the healing garden on the rooftop.The best experience-based design strategies for the therapeutic green spaces or healing gardens development were identified from the previous comparison matrix, and divided into A.Safety, Security and Privacy; B.Accessibility; C.Physical and Emotional Comfort; D.Positive distraction; E.Engagement with Nature; F.Maintenance and Aesthetics; and G.Sustainability.</p><p><strong>Conclusions: </strong>The results obtained from the comparative matrix are qualitative and quantitative design elements in terms of type of element / space, percentage, perimeter, area, number, materiality, shape, color, among others. The resulting iron overload damages multiple organs, particularly the heart and endocrine organs. This study aimed to describe and assess the predictors of survival and complications in Omani patients with β-TM. Methods: All β-TM patients registered in the day care of Sultan Qaboos University Hospital were included in this retrospective study. Results: There were 187 patients with β-TM with a median follow-up of 24.9 years. The median ages at diagnosis and the start of chelation were 0.7 and 4.8 years, respectively. The following complications developed at different time points [Median (age in years), Complication Free Probability at 20 years]: Death (20.0 years;85%), hypogonadism (15.9 years;50%), insulin-dependent or non-insulin dependent diabetes (20.0 years;88%), cardiac complications (20.3 years;91%), osteoporosis (20.7 years;96%), hypothyroidism (25.7 years;97%), liver complications (7.3 years;54%). The number of complications predicted death (P = 0.0038). Those born after 1980 had a lower risk of death (P = 0.005), hypogonadism (P = < 0.0001), and cardiac complications (P = 0.004). Higher serum ferritin at the start of chelation was associated with the development of diabetes (P = < 0. 001). Conclusions: This long-term study shows complications development at different ages, and the number of complications is associated with survival. Later birth cohorts had a lower risk of death, hypogonadism, and cardiac complications. There was a persistent negative impact of delay in the start of iron chelation that is present even after a long follow-up. There is lack of consensus on the optimal method of reconstructing the acetabular defects. The aim of this systematic review is to take stock of the state of the art on the options available and highlight which type of construct is the most reliable in usual clinical practice.</p><p><strong>Material and methods: </strong>The reporting of this systematic review was guided by the standards of the Preferred Reporting Items for Systematic Review and Meta-Analysis 2020 Statement.Electronic search of MEDLINE was performed from 1991 up to April 2021 to identify relevant studies for this review.  Discussion: various surgical techniques have been adopted and proposed to treat acetabular bone defects: cemented cups, large-sized non-cemented acetabular cups, higher positioned cups, reinforcement rings, cage, oblong cups, custom triflange implants, high porous metal cups and augments. Bone defect defines the type of components to be implanted and among those, outcomes are various depending on the study taken into account, the component used and the degree of initial bone defect.</p><p><strong>Conclusions: </strong>In acetabular revision surgerythe use of TM cups and augment is a valid option in presence of major bone loss and pelvic discontinuities. In clinical practice the use of TM components replaced rings, while the cup-cage implant replaced conventional cages. We aimed to present an uncommon related case, assessing also the role of malnutrition and osteoporosis. We discuss the case of a 49 years old male patient, malnourished and osteoporotic, with an intertrochanteric, multifragmentary, inveterate fracture, that happened three months before. During the prosthetic surgery, a supracondylar fracture occurred. We synthesized it with a 13 holes LISS plate, 7 distal screws and 3 proximal cerclage wires. The leg was immobilized to 0° for 3 weeks and the load forbidden for 3 months. Calcium, Albumine, Total Proteins, Osteocalcin and Vitamin-D levels were below the normal range. Teriparatide was administered for 6 months. Global investigation of the patient's nutritional status is recommended before joint replacement and preoperative planning is mandatory to choose the adequate implant. The purpose of this study is to evaluate the correlation between medullary diameter (MD) and cortical width (CW) ratio, a known risk factor for periprosthetic fracture, with the occurrence of diaphyseal fracture distal to the nail tip.</p><p><strong>Research design and methods: </strong>Study population included patients > 65 years old treated with short CM nail for proximal femur fractures (PFF) at the Cattinara Hospital-ASUGI of Trieste (Italy) Orthopedics and Traumatology Unit between July 2014 and May 2018. Latest follow-up X-rays were evaluated to identify diaphyseal fracture occurrence. We recorded type of trauma, time lapse between CM nailing and diaphyseal fracture and calculate the MD/CW ratio on intra-operative X-rays. Data were analyzed to find out a correlation with the occurrence of diaphyseal fractures.</p><p><strong>Results: </strong>The study population counted 488 patients. Diaphyseal fracture occurred in 14 cases (2.9%, F:M ratio 6:1, mean age 87 years), at mean 26.07 months after CM nailing. The standard surgical treatment is total or partial hip replacement, including a cemented or uncemented stem. The cemented prosthesis is considered the safer option because of a lower rate of periprosthetic fractures (PPFs) as well as an actually reduced risk of bone cement implantation syndrome (BCIS). This retrospective study aims to assess the efficacy and safety of cemented versus uncemented femoral stem for FNF in patients ≥70 years.</p><p><strong>Methods: </strong>139 patients affected by displaced FNF underwent hip replacement, receiving 89 cemented (64%) and 50 uncemented (36%) stems. Inclusion criteria were: ≥70 years of age, an ICD-9-CM diagnose code 820.00, 820.01, 820.02, 820.03, 820.10, 820.8, and a minimum 1-year follow-up. A p value <0.05 was considered statistically significant.</p><p><strong>Results: </strong>Surgical time, overall perioperative complication rate with a particular focus on the thromboembolic events, and PPFs incidence were evaluated comparing cemented and uncemented group. No difference in duration of surgery was found. Intraoperative complications were not detected. Pulmonary embolism and deep vein thrombosis were observed each in 1 case of cemented prosthesis. There are many factors related to the risk of developing AVN, with fracture complexity being one of the most relevant. Aim of the present study is to evaluate the correlation between different classification systems and the risk of post-operative AVN in patients treated with locking plate fixation for Neer 3 and 4-part PHFs.</p><p><strong>Methods: </strong>The study population included 44 patients (F:M 2:1, mean age 62) treated between December 2014 and April 2019. Fractures were classified according to Neer, Russo and Lego classifications and Hertel's criteria. AVN was established on postoperative radiographs after a minimum follow-up of 6 months. The odds ratio (OR) for AVN for each classification subtype was calculated.</p><p><strong>Results: </strong>The incidence of necrosis was 11%. Significant association with AVN was found for Neer 4 part (OR=8), Russo IVB (OR=4), Lego pattern 12 (OR=8) and in presence of 3 mayor Hertel's criteria (OR=15,5).</p><p><strong>Conclusions: </strong>There is a significant association between AVN and Neer 4 part, Russo type IVB and Lego pattern 12 fractures. Aim of this study was to evaluate the clinical and radiological outcomes of RSA performed for comminuted PHFs, without periprosthetic reconstruction of the tuberosities. Our hypothesis was that a large diameter glenosphere could ensure satisfactory RSA mobility and stability, regardless of tuberosity removal.</p><p><strong>Methods: </strong>We selected 32 patients (4 men, 28 women) with comminuted PHFs who underwent RSA with tuberosity excision and implantation of a 44-mm glenosphere between 2009 and 2015. Active range of motion (ROM), stability, Constant-Murley Score (CMS) (1) and Subjective Shoulder Value (SSV) (2) were assessed one year and four years after surgery.</p><p><strong>Results: </strong>Clinical and radiological information were collected for 23 patients (72%). At one-year follow-up, active anterior elevation (AE) was 96±28 degrees, external rotation with adducted arm (ER1) 9±7 degrees, external rotation with abducted arm (ER2) 14±10 degrees, internal rotation (IR) to L4; CMS was 56±10 and SSV 65±22. Clinical assessment at 4-year follow up showed a decrease in active ROM (AE was 88±20 degrees, ER1 8±2 degrees, ER2 12±10 degrees, IR to L4), CMS (52±9) and SSV (62±8). No RSA dislocation occurred during the study. In 4 patients, grade I glenoid notching without any sign of component loosening was observed 4 years after surgery.</p><p><strong>Conclusions: </strong>A large diameter glenosphere does not ensure results comparable to those achieved after RSA with tuberosity reconstruction. The authors report a case associated with massive heterotopic ossifications, with the purpose to analyze risk factors and specific characteristics.</p><p><strong>Methods: </strong>A case of femoral monobloc stem neck rupture is described. A non-systematic literature review regarding risk factors for femoral stem neck fracture was conducted in the PubMed database.</p><p><strong>Results: </strong>We report the case of a 61-year-old male who underwent surgery to remove calcifications four years after THA. Four months later the patient reported acute pain in the left hip, arising after a combined movement of external rotation and axial load while standing on the left foot, in the absence of any prodromic symptom. On radiographs, a displaced fracture of the neck of the hip prosthesis was revealed, together with massive heterotopic ossifications. After THA revision the patient's symptoms were resolved.</p><p><strong>Conclusions: </strong>Prosthetic femoral neck fractures are a rare complication. Less pain, better results in the first few weeks, reduced dislocation rate are the prerogatives of this approach. We analyzed in a group of 26 patients the use of a soft tissue retractor, Alexis Orthopedic Protector, the CPK values in the first third and fifth postoperative day. The results obtained, normalized, demonstrate a slight reduction in tissue damage using the device compared to a control group. In addition to blood values, improved exposure, distribution of leverage, cleanliness of the field and a potential reduction in infectious risk are advantages for the use of an orthopedic surgery retractor. We believe that the adoption of the Alexis Orthopedic Protector provides numerous improvements during the hip arthroplasty surgical procedure, especially when adopting the direct anterior approach, because it distributes the traction forces around the surgical path resulting in greater visibility.
