Sputum specimens were collected shortly after admission and subjected to culture and real-time PCR testing.Results: Out of the 252 patients, 170 (67.4%) met the criteria for analysis and identification of pathogenic microorganisms. The most frequently isolated pathogens were Streptococcus pneumoniae (S. pneumonia) and Haemophilus influenzae (H. influenzae), comprising of 27.0% and 13.1%, respectively. Antibiotic susceptibility testing was conducted on 55 patients (32.3%). Among these cases, there were 16 instances of S. pneumoniae, 10 cases of Staphylococcus aureus (S. aureus), 10 cases of Pseudomonas aeruginosa (P. aeruginosa), 14 cases of gram-negative enteric bacteria, 2 cases of H. influenzae, 1 case of Moraxella catarrhalis (M. catarrhalis), 1 case of Enterococcus faecium, and 1 case of M. pneumoniae. The antibiogram results revealed significant findings; including a 70% prevalence of extended-spectrum beta-lactamase and 90% occurrence of Methicillin-Resistant Staphylococcus aureus. The data also indicated high resistance rates; such as 90% resistance to erythromycin, 40-80% to ciprofloxacin, 30-80% to ceftazidime, and 30-40% to imipenem.Conclusion: Pathogens associated with LRTIs referred from primary care hospitals encompass S. pneumonia, H. influenza, and S. aureus. From this, twelve YGNBR recipes were formulated, and the physicochemical properties of the raw materials and recipes were determined. The thin layer chromatography (TLC) and high performance liquid chromatography (HPLC) fingerprints were investigated, and the microbial limits and heavy metal contamination were determined.Results: YGBNR was prepared by mixing dry powders of kratom: ginger: Java long pepper; chilli in the ratio of 72:5:13:10. The phytochemical properties; including loss on drying, total ash, acid-insoluble ash, ethanol (80% v/v)-soluble extractive, water-soluble extractive, volatile oil, and mitragynine content were: 7.68±0.04%w/w, 5.72±0.06%w/w, 0.26±0.02%w/w, 28.99±0.17%w/w, 21.98±0.21%w/w, 0.40±0.00%v/w, and 0.58±0.28 %w/w, respectively. The microbial limits and heavy metal contaminations were under the THP requirements. Conclusion: The physicochemical properties of YGNBR, an antidiarrheal recipe, was determined according to THP. This study aims to explore the frequency, clinical, and electrodiagnosis of peripheral neuropathy of ALL patients.Material and Methods: This study was a retrospective cohort study, using medical record reviews of children who, diagnosed and undergoing treatment for acute lymphoblastic leukemia, from 1st January 2018 till 31st December 2022.Results: One hundred and ninety-seven patients were diagnosed with ALL and received chemotherapy. Ten patients had clinical and electrodiagnostic studies of peripheral neuropathy. B-cell type and high- risk classification were the most common in these patients. The most common symptom was bilateral foot drop and all patients had hyporeflexia. The mean cumulative dose of vincristine was 13.35±2.12 mg/m2. The axonal neuropathy, with motor dominantly and the peroneal nerves, were the highest abnormal finding in electrodiagnosis testing.Conclusion: This report addresses the frequent and clinical of patients with peripheral neuropathy. Immunohistochemistry analysis of breast cancer type 1 (BRCA1), poly (ADP-ribose) polymerase 1 (PARP1), X-ray repair cross-complementing 1 (XRCC1), and excision repair cross-complementation group 1 (ERCC1) expression was performed. Logistic regression was used to evaluate factors associated with chemotherapeutic response and Cox regression was applied for survival analysis.Results: Chemotherapeutic response was achieved in 205 of 249 patients (82.3%). Low BRCA1 expression was associated with good response (odds ratio [OR] 5.01, 95% confidence interval [CI] 1.78–14.1) and favorable overall survival (hazard ratio 0.61, 95% CI 0.38–0.98). PARP1, XRCC1, and ERCC1 showed no significant predictive or prognostic roles; however, combined low expression of PARP1/BRCA1 (OR 7.62, 95% CI 1.69–34.31) and ERCC1/BRCA1 (OR 6.98, 95% CI 1.5–32.52) additively enhanced response compared to high/high expressions.Conclusion: This study provides evidence that epithelial ovarian cancer (EOC) with low BRCA1 expression is more likely to be responsive to platinum-based therapy and is associated with favorable overall survival compared to tumors with high BRCA1 expression. Herein is reported the success of an open gastrotomy under truncal blocks in a patient for whom general anesthesia and neuraxial blockade were undesirable. A 79-year-old man, with advanced esophageal cancer presented with several comorbidities; including cardiomyopathy, ischemic heart disease, and prior cerebral infarction. Difficulty in airway management was anticipated due to the mass’s compression on the airway. Physical characteristics, ultraviolet (UV) absorbance, browning intensity, protein concentrations, molecular weight distribution, and amino acid profiles of the hydrolysates and MRPs were determined. The anti-oxidation properties of the hydrolysate and MRPs were evaluated by 2,2-diphenyl-1-picrylhydrazyl (DPPH) radical scavenging assay. Anti-cholinesterase activities were evaluated using Ellman’s method. Cell viability of the samples was evaluated in RAW 264.7 cells.Results: The MRPs presented as brown to dark brown liquids with Brix values ranging from 61.33 to 64.17 °Brix. The pH value ranged from 5.31 to 6.20. The UV absorbance at 294 nm and browning intensity at 420 nm of the MRPs were less than the absorbance of the corresponding hydrolysates. The protein concentrations were 41-44% (hydrolysates) and 18-20% (MRPs). The major constituents of the hydrolysates and MRPs were small molecules (MW<300 Da). The samples exhibited anti-oxidative activity (EC50 2.03 - 3.51 mg/mL) and anti-acetylcholinesterase activity (45-52% inhibition at 0.5 mg/mL). In Thailand, this scale was translated and used by nurse practitioners. However, many items designed for use have contextual differences from the hospital and have never been tested for their construct; including issues of cultural differences. Therefore, such a scale must be revised. This study aimed to translate and test the psychometric properties to justify its use in a hospital context.Material and Methods: This was methodological research. The process consisted of two phases; including: 1) translation using the forward-backward translation method, and 2) psychometric properties testing; content validity, cognitive interview, construct validity, and reliability would be used to evaluate psychometric properties. Content validity was considered by a panel of experts. Ten registered nurses were asked to participate in a cognitive interview. Two hundred and fourteen registered nurses were recruited, using convenience sampling for conducting the construct validity and reliability testing.Results: The findings revealed excellent content validity. Twenty-three items were revised in the phase of cognitive interviews. Internal consistency was calculated: Cronbach’s alpha was 0.959. Confirmatory factor analysis yielded four factors, and was consistent with the original version.Conclusion: The scale may be utilized by registered nurses working in Thailand’s hospital context to assess the quality of nursing work life. The production levels of NO, TNF-α and PGE2 was induced by LPS. The inhibition percentages of NO increases in the presence of ginger extract (0.313 mg/ml) and phlai oil (0.625 mg/ml) were 38.22±0.236% and 25.00±0.198% in comparison to the control cells. The combination of ginger extract and phlai oil at a ratio 1:1 w/w exhibited statistically significant inhibitory effects on NO production, having an inhibition percentage of 75.98±0.591%. In the TNF-α assay, an inhibition percentage value of ginger extract (58.74±0.136%) was higher than that of phlai oil (52.13±0.238%), while the combination of ginger extract and phlai oil had the highest inhibitory effect (70.03±0.215%). {"title":"Assessment of Radiation Dose and Phantom Skin Dose in Transarterial Chemoembolization in a Single Center with 2 Digital Subtraction Angiography Units","authors":"Varaporn Pong-Inwong, Woranan Kirisattayakul, Supawan Dawong, Areeya Jitvikham, Anucha Ahooja, Panuwat Pattum, Rattapong Karawek, Waraporn Sudjai, Nittima Rungpin","doi":"10.31584/jhsmr.2023996","DOIUrl":"https://doi.org/10.31584/jhsmr.2023996","url":null,"abstract":"Objectives: This study aimed to assess and compare the radiation dose and phantom skin dose in transarterial chemoembolization (TACE) of two digital subtraction angiography (DSA) units (unit A: Philips Allura Xper FD20, unit B: Artis zee biplane)Material and Methods: The dose area product (DAP), reference air kerma (RAK), number of images (NI) and fluoroscopy time (FT) of 240 cases (120 cases/DSA unit) were retrospectively reviewed and collected. To assess skin dose, 28 nanoDot optically stimulated luminescence dosimeters (OSLDs) were placed on the phantom’s back and the TACE procedure was performed with 2 DSA units.Results: The median DAP, RAK, NI, and FT of unit A were 200.49 Gy·cm2, 379.84 mGy, 115 images, and 9.04 minutes, while for unit B were 109.74 Gy·cm2, 276.55 mGy, 121 images, and 10.19 minutes, respectively. Significant differences were observed in DAP, RAK, and FT. The RAK of all patients was less than 2 Gy. The phantom skin dose obtained from unit B was significantly lower than that of unit A in all positions. The peak skin doses of the phantom studies from both units were 973.15 and 658.66 mGy, respectively.Conclusion: The median DAP of the unit A DSA was higher than the national diagnostic reference levels (DRLs). The TACE procedure from both units is safe from skin reaction. Patients undergoing elective LC; from September 2016 to January 2023, at Hatyai Hospital in Southern Thailand were included. Patients were categorized by difficultly of LC according to the Nassar scale (grades 1-2 as non-difficult LC and grades 3-4 as difficult LC). Pre-operative data were compared between both groups; utilizing multivariable logistic regression. Internal validation was performed via the bootstrapping procedure.Results: In total, three hundred and eighteen patients were categorized into either; difficult LC 121 patients or non-difficult LC 197 patients. From this, 7 variables obtained from the multivariable logistic reduced model (male, cirrhosis, history of ERCP, ASA III, gallbladder wall ≥4 mm, dilated gallbladder, contracted gallbladder) were developed as a pre-operative score. The scoring (range: 0.0-16.6) was classified into 3 groups for clinical practicability. The positive predictive values (PPV) were 18.1 for low-risk, 38.0 for moderate-risk, and 76.0 for high-risk. Internal validation, via bootstrap technique, showed a C-statistic value of 0.76, and bootstrap shrinkage was 0.995.
