Despite the identification of more than 100 causative genes, there are few therapeutic options. We present the case of a 66-year-old man carrying a CMT mutation in the connexin 32 gene with the APOE genotype ε4/ε3 that had exhibited symptoms of slowly progressive muscle weakness and atrophy in the hands and feet and recently developed memory impairment. The muscle weakness, motor dysfunction, and memory impairment symptoms were significantly improved by cytokine-induced neurogenesis combined with physical wave therapy using an arbitrary waveform generator (AWG). ABSTRACT Ectopic thyroid is an uncommon entity that may occur from tongue to mediastinum that is along the line of obliterated thyroglossal duct. The authors report a case of mediastinal ectopic goitre with eutopic thyroid in a 73 years old African woman who was presented in Otorhinolaryngology outpatient department of Indraprastha Apollo Hospitals, New Delhi, India. She presented with breathing difficulty on exertion. The diagnosis was confirmed by ultrasound guided fine needle aspiration cytology and computed tomography scan of neck. She then underwent excision of ectopic thyroid tissue mass after taking a well informed and written consent from the patient. Mostly mediastinal ectopic thyroids are incidental findings but large masses can present with pressure symptoms thus necessitating excision. {"title":"Evaluation of the Effect of Acute Normovolemic Hemodilution on Bleeding Rate and Short-Term Post-Operative Complication of Patient Who Underwent on-Pump Coronary Artery Bypass Graft Surgery","authors":"F. Postoperative bleeding and transfusion remain a source of morbidity and cost after open heart operations. To better understand the role of acute normovolemic hemodilution (ANH) in coronary artery bypass grafting (CABG), we compared ANH with standard intraoperative care in a retrospective cohort study. Methods: This retrospective cohort study is based on 572 patients who underwent on-pump CABG in the cardiac operating room of Imam Khomeini Hospital from June 2016 till March 2022. 221 patients (38.6%) were in the ANH group and 351 patients (61.4%) were in the control group. This study was based on patients documented information. P<0.05 was significant. Result: The prevalence of short-term complications was bleeding (74.96%), AKI (7.38%), CVA (1.92%) and HF (1.05%), respectively. In general, bleeding was more in the ANH group. There was no significant relationship between ANH and days of hospitalization in ICU (P=0.291), CVA (P=0.748), HF (P=1.000), AKI (P=0.411), bleeding rate on the second day (P=0.180), platelet transfusion (p= 0.158) and FFP transfusion (p=0.776). There was a significant relationship between ANH and the reduction of bleeding (P=0.000), the increase in bleeding on the first day (P=0.006), the reduction of mortality (P=0.007), the reduction of transfusion packed cell (p=0.000). Conclusion: It seems that ANH leads to a decrease in mortality and bleeding, and as a result, a decrease in the allogenic blood transfusions and an increase in bleeding on the first day, but it have no effect on the days of hospitalization in the ICU, CVA, HF, AKI, platelet and FFP transfusion. Objective: To present a case with a giant multiple choledocholithiasis, with good evolution. Clinical Table: Female patient of 57 years of age, mixed race, smoker, hypertensive, with a history of vesicular lithiasis that was operated 28 years ago where initially it is treated by videolaparoscopy but in the transoperative it is determined its conversion to conventional cholecystectomy. The same goes to consultation of the area because it begins with pain in the epigastrium, dyspepsia, urine dark urine and sometimes gray stool for which they indicate a study and assess by the services of gastroenterology, general surgery where the surgical intervention is decided. Initially treated by ERCP and subsequently performed a Choledocholithotomy with placement of Kerh probe. {"title":"The Relationship between LDL Cholesterol Level and Cardiovascular Disease Risk: A Case-Study-Based Cognitive Study","authors":"I. High levels of low-density lipoprotein (LDL) cholesterol in the blood have been identified as a significant predictor of CVD risk. This case-studybased cognitive study aimed to investigate the relationship between LDL cholesterol level and CVD risk in a group of 100 patients with CVD. The patients were divided into two groups based on their LDL cholesterol levels: high LDL group (LDL > 130 mg/dL) and normal LDL group (LDL ≤ 130 mg/dL). The medical history of each patient was obtained and analyzed, including factors such as age, gender, smoking history, and family history of CVD. The data were statistically analyzed to investigate the relationship between LDL cholesterol level and the risk of CVD. The results showed that the high LDL group had a significantly higher risk of CVD compared to the normal LDL group (p < 0.05). Age, gender, smoking history, and family history of CVD were also found to be significant risk factors for CVD (p < 0.05). In the next section, we look at the symptoms together with various examples with their similarities and exceptional signs as well as rare long-term studies which are very useful for physicians and can contribute to a better clinical diagnosis. Afterwards, the diseases that are associated with this syndrome are named and being aware of such diseases will not only provide new research topics for physicians, but also help them better treat and prevent them from giving other irrelevant treatment methods. Furthermore, besides the associated diseases other factors related to this syndrome is discussed. In addition, the existing diagnostic and treatment methods together with various examples are investigated. Finally, the results of some of the previously done studies are discussed based on their categories. Objective: To identify the problems and solutions of the treatment of foreign body trauma in the maxillofacial region. Methods: Two cases of foreign body trauma patients include physical examination findings, radiological images and follow-up was documented to find out the after effect of residual part and treatment outcomes. Result: Both patients received treatment immediately after injury. But few months later complications arise due to the remnant of foreign body. In case of first patient location was not suitable for USG and radiology also unsuccessful to notice the presence of foreign body. In case of 2nd patient need further treatment after initial management. Conclusion: Causes for the remnant of foreign body during first steps of management are bleeding, proximity to the vital structures, and hiding in the spaces created by multiple fascia layers in the oro-facial region and causes late complications like swelling, trismus, discharging sinuses, etc. Radiological imaging sometimes fails to detect the presence and position of remnants. The classical clinical presentations are of rapid onset and evolution. This case describes an MH atypical presentation. Case Report: Male, 41 years old, 85 kg, brown, admitted for a hepatic tri-segmentectomy to treat a Klatskin’s tumor. Surgery was performed under balanced general anesthesia with sevoflurane and epidural block. The patient presented with hyperthermia, profuse sweating, need for a progressive increase in minute ventilation to control etCO2 values, metabolic lactic acidosis, and hyperkalemia, progressing to circulatory shock. The acute condition was controlled with dantrolene and supportive measures, but there was progression to death after one week. Conclusion: This case showed a rare presentation of MH. Analysis of the spontaneous activity of neural networks is based on data obtained during neurosurgical operations conducted by W. Penfield. The case of frequentative electrode stimulation of the human brain temporal cortex and the patient simultaneous interview were analyzed. Such a survey revealed that the irritation of a specific localization caused the actualization in a person memory of an episode of her past according to this localization. It is associated with maternal-fetal complications, being early diagnosis and fundamental treatment. Objective: To clinically and epidemiologically characterize pregnant women with vaginal infection belonging to the “Orlando Matos Mosquera” Polyclinic in the period 2019-2021. Method: Observational, descriptive and cross-sectional study of pregnant women with vaginal infection. The arithmetic mean and standard deviation of the quantitative variables and the absolute frequencies and percentages in qualitative variables whose association was obtained through the Chi-square with 5% significance were obtained. Results: 245 pregnant women were analyzed. The mean age was 25 years. 42.4% were mestizo, 56.4% accompanied or single, 41.2% had an upper secondary level, and 12.7% were housewives. At least 7 out of 10 smoked or had unprotected sex. 80.8% of the examined had vaginal discharge. 75 pregnant women suffered infection by Candida albicans and the average gestational age was 18.58 ± 8.48 weeks of pregnancy, with 37.6% of vaginal infections occurring in the first trimester. 73.0% of the mixed infections had vaginal changes and 66.7% of the infections due to infrequent germs were complicated. 3 out of 10 presented premature rupture of membranes, abortion or preterm delivery. Conclusions: Adolescents and young adults, mestizos, accompanied and with a higher secondary level prevailed. Vaginal discharge as a symptom and Candida albicans infection predominated.
