Erik, Franco González Samantha","doi":"10.30654/MJCR.10048","DOIUrl":"https://doi.org/10.30654/MJCR.10048","url":null,"abstract":"The reconstruction of the thoracic wall continues to be a great challenge for the plastic surgeon, since there is no precise algorithm to carry it out; due to the complexity of the thorax, there are multiple alternatives to perform the reconstruction, this involves the restoration of pulmonary physiology, stabilization of the thorax and adequate coverage of soft tissues. Five reconstructions were carried out over 3 years (January 2015-March 2018). These were carried out with pediculated muscle flaps, the patients were followed for a year, in this time interval the death of a patient was reported at 3 months, due to oncological recurrence, however, no associated complications to the procedure were reported. Arredondo","doi":"10.30654/MJCR.10044","DOIUrl":"https://doi.org/10.30654/MJCR.10044","url":null,"abstract":"Introduction: The first references about splenogonadal fusion were described by Boestrom in 1883 and Pomer in 1889. Its incidence is unknown but it is considered according to Montes and Prada in about 160 reported cases. Clinical case: A 3-year-old boy with no perinatal history of importance and who consults for volume increase in the left inguinal region. Physical examination shows a visible mass, not painful on palpation, soft, not attached to deep planes with reduced hernial content. The clinical diagnosis of left inguinal hernia is established and scheduled for surgery. The histopathological report explains in the macroscopic description a vermiform tubular tissue of 7 cm in length and thickness of 1.3 cm of rubberized consistency compatible with splenic ectopic tissue. Discussion: Has been reported a case in a 6-year-old boy who presented clinically as a strangulated left inguinal hernia and this case was a continuous splenogonadal fusion, but our case was a discontinuous splenogondal fusion. Our case is the first one reported in a child with a palpable mass in the inguinal region, similar to an inguinal hernia that can be reduced with inguinal plasty with a biopsy that reported splenic tissue. Tiu","doi":"10.30654/mjcr.10043","DOIUrl":"https://doi.org/10.30654/mjcr.10043","url":null,"abstract":"This case series presents 2 patients (67 and 58 year old females) with resolving vitreous hemorrhage in the setting of diabetic retinopathy. Both patients had around 50% of the retina still visible and the hemorrhage was confined at the posterior vitreous cavity near the retina. Hence, partial panretinal photocoagulation was feasibly applied. On B-scan, both cases showed a liquified vitreous with a complete posterior vitreous detachment (PVD). For both patients, vitreous hemorrhage resolution occurred at less than 4 months from onset. Vitreous hemorrhage differs with bleeding in other tissues with the following characteristics: (1) Rapid Clotting, (2) Slow lysis of fibrin, (3) extracellular lysis of RBCs, (4) Persistence of RBCs for months, (5) Lack of PMN response. The presence of vitreous collagen matrix promotes rapid clotting and hinders resolution of hemorrhage by preventing passive diffusion; hence also delay inflammatory cellular response. Hemolysis is more important than phagocytosis and transportation of debris by macrophages for the removal of whole blood in vitreous. The prognosis for clearing hemorrhage was better when the fundus reflex was brighter and the retina adjacent to the ora serrata was visible. Clearing occurred sooner when hemorrhage was retrohyaloid rather than in the vitreous gel. Posterior vitreous detachment status has also been implicated in diabetic retinopathy progression. This is the basis for androgen ablation treatment, which attenuates, but does not terminate, the malignancy. Instead, it leads to prolactin-dependent malignancy; in which patient death generally occurs within 5 years. This report describes the novel treatment of a patient; which terminated androgen-independent prostate cancer.</p><p><strong>Results: </strong>Patient \"XY\" was diagnosed with prostate malignancy and metastases. He received hormonal androgen ablation treatment, chemotherapy, and radiation treatment. He developed androgen-independent prostate cancer; with expected death in 2-3 years. He was treated with cabergoline (dopamine agonist) treatment, which decreased the plasma prolactin 88%; by inhibiting the pituitary production of prolactin. The subsequent PET scan (positron emission tomography) revealed the absence of malignancy; and the CTC (circulating tumor cells) decreased from count=5.4 to count=0.</p><p><strong>Discussion: </strong>The cause of androgen-independent malignancy has been unknown, and an effective chemotherapy did not exist. The activities of normal and malignant prostate cells are regulated primarily by testosterone. When testosterone availability diminishes; prolactin regulation is manifested. This is represented when androgen ablation results in the development of prolactin-dependent malignancy. An effective chemotherapy would be targeted to eliminate the plasma prolactin-manifestation of the androgen-independent malignancy.</p><p><strong>Conclusions: </strong>This report of a novel chemotherapy for androgen-independent malignancy corroborates our understanding of the implications of prolactin in its development and treatment. There are about 165,000 cases/year with 25,000 deaths/year in the U.S.; and 1.0 million cases/year with 260,000 deaths/year worldwide. Failure to properly diagnose, which occurs in 80% of the time, can result in gradual progression to irreversible Korsakoff Syndrome, characterized by irreversible personality changes, and anterograde and retrograde amnesia.
