Although Extremely Rare, Spontaneous Abortions Can Result Into Severe Sepsis And Endanger Life of the Pregnant Woman. Case Report: A 30 year old multiparous mother was admitted with history of spontaneous abortion at 18 weeks gestation at nearby hospital in teseney, eritrea on 21/03/2022. It was planned pregnancy that ended up in inevitable abortion which was attended by general practitioners and midwives in the hospital and discharged after 48 hours. A week later, on 28/03/2022, she was re-admitted again with a complaint of diffuse abdominal pain, distention and shortness of breath. Despite a week course of broad spectrum antibiotics in the hospital, abdomen was progressively distended, and she was referred to orotta national maternity referral hospital. explorative laparotomy was done and about three liters of serous fluid with foul smelling debris was evacuated from the abdomen. Extensive bowel adhesion was found and gentle adhesion lysis was done. parietal peritoneum was red to black in color and uterus and ovaries were found healthy. on postoperative third day, she developed pulmonary embolism and transferred to intensive care unit. after a month of medical and surgical management, she was discharged home with full improvement. Conclusion: The case is presented because of its rarity in regards to primary etiology and severity of sepsis. The onset was acute that may end in death if urgent interventions were not done. Instead of addressing the disease itself, medical treatment just addresses the symptoms. Due to the fact that the majority of senior patients will have comorbid conditions like diabetes and hypertension, surgery may not be an option for everyone, making medical care more widely available. Notably, even while TURP, TUIP, TVP, TUMT, and TUNA continue to be the gold standards for treatment, medicinal therapy still offers hope for recovery and averting surgery. But one must be aware of the recently identified negative effects that these drugs can have in the elderly male, especially over a lengthy period of time. Though they are not appropriate for every patient, novel minimally invasive procedures have shown promise for the elderly male who has failed medicinal therapy but is not a surgical candidate or wishes to avoid surgery. It has various clinical forms, mainly known as nodular, superficial, and morphea. Other less prevalent forms include polypoid, pore-like, and Jacobi ulcers. Polypoid BCC is rarely observed in individuals and has remarkable clinical features, such as being pedunculated and connected to the skin by a stalk; the BCC incidence is more in women than men and correlates directly with age, as it is reported to be <15% in patients below 40 years, so it is not frequently observed among young adults. In this case, we report one type of BCC presented as a skin tag. A 21-year-old young lady, four weeks gestation, was referred with a skin tag on the right axillary region. Surgical excision was performed; a histopathologic examination of the skin lesion revealed a malignant epithelial neoplasm composed of some nests of basaloid cells and an accumulation of mucin. The IHC staining, such as Bcl2, CK20, CD34, and Ki67, facilitates better evaluations. She was first admitted due to epigastric pain, nausea, and vomiting, which was managed and discharged after three days. She was admitted due to severe right upper quadrant pain, which sonography showed cholecystitis; however, the levels of lactate dehydrogenase level, ALT, AST, and platelet suspects to HELLP syndrome, and after two days of current admission, these parameters became 2148 IU/L, 199 U/L, 230 U/L, and 43000 respectively, which led us to a definitive diagnosis of HELLP syndrome patient underwent cesarean section surgery, which led to significant improvement in laboratory findings Afterward, on day 3, the patient went on laparoscopic cholecystectomy surgery. In this case it is a diagnostic and therapeutic emergency with respect to the many complications of ovarian torsion including necrosis. We report a case of endometrioma diagnosed by a torsion of the ovary. The context clinic was dominated by acute pelvic pain; the ultrasound showed a huge left cystic ovary, and the laparotomy revealed a torsion of the left adnexa. A detorsion and cystectomy was performed and the confirmation of the endometrioma was provided by the histology of the surgical specimen. The difficulty lies in the diagnostic and therapeutic choice to preserve or not the ovary. PAF functions via a G-protein coupled receptor mediated pathway, which ultimately increases intracellular calcium levels. Since its discovery, PAF has been positively linked with fertility, i.e. reproductive outcomes. Roudebush et al. [1] found that boars with high fertility have a significantly more endogenous PAF in sperm than boars with low fertility [1]. Roudebush et al. reported that PAF levels in sperm were significantly higher during the breeding season of squirrel monkeys in comparison to the nonbreeding season [2]. Additionally, Sengoku et al. In Eritrea, as adolescent reproductive health is not given much attention the knowledge is expected to be poor, which could lead adolescents to become victims of adolescence related reproductive health problems and limit their opportunity to build a better future. Therefore, this study was carried out with the aim of investigating the reproductive health knowledge, attitude and practice among secondary school students in Asmara, the capital of Eritrea. Methods: A cross sectional study design was conducted on 428 adolescent high school students with the age range of 14 to 19 years. Sample size was determined by using a single population proportion formula. Initially, population size of each school and each grade was taken and proportional sample size was determined from each school and then from each grade. The list of students was used and participants were selected using simple random sampling. Data was collected through a structured self-administered questionnaire. Descriptive and inferential analysis were made using SPSS (version 22). Statistical significance level was set at P<0.005.Results: The median age of the study participants was 15 (IQR=2), in which 88.8% were between age of 14 to 16 years with 53% of them being females. The level of knowledge of the students regarding RH was low, with moderate to good attitude. Sexual practice was very low (3.5%) with usage of 86.7% of contraceptive use. Meanwhile, the usage of sexually explicit material was relatively higher (53%). Conclusions: Reproductive health knowledge was low almost in all variables. Meanwhile, significant number of students are already engaged in heterosexual romantic relationships and use of sexually explicit material. Objectives: To find out the Quality of Life in female patients with urinary incontinence in a tertiary care hospital. Materials and Methods: A cross-sectional, observational study was conducted in the Gynecology out-patient department. All non-pregnant women ≥21 years of age, attending the Gynecology OPD of tertiary care hospital for various complaints were included in the study. After obtaining written informed consent, demographics, relevant medical and surgical history was noted, and they were then administered two validated questionnaire 'Incontinence Quality of Life Questionnaire' (IQOL) and SF-36. Result: The overall incidence of incontinence in this population was 17.0%, with a steadily rising incidence by age. The incidence was more in postmenopausal than premenopausal women. Menopausal women compared to non-menopausal women also had significantly higher incontinence QOL scores (median of 5 vs. 3; p=0.001). Avoidance and limiting behavior (ALB), Psychosocial impact (PSI) and social embarrassment (SE) scores were studied improved after 24 weeks. Items determining ALB had high correlation coefficient with total ALB and all of them were statistically significant. Items determining PSI had statistically significant correlation with mean PSI with all the items except two items. {"title":"The Impact of Physiological Interpretation of Cardiotocogram (CTG) in Conjunction with Fetal ECG (STAN) on Intrapartum Emergency Caesarean Sections","authors":"E. Fetal ECG (ST-Analyser or STAN) was introduced into clinical practice to reduce the false positive rate of the CTG. Objective: To determine the impact of Physiological interpretation of Cardiotocogram (CTG) in Conjunction with Fetal ECG (STAN) on intrapartum emergency caesarean sections. Methods: A retrospective study at the Royal Gwent Hospital in South Wales following the introduction of STAN monitoring after training in Physiological Interpretation of CTG in 2014. Total Caesarean section rate, emergency caesarean section rate for fetal compromise (i.e., “fetal distress”), operative vaginal delivery rate and spontaneous vaginal delivery (SVD) rate were analyzed for the first 19 months after the intervention (October 2014- 2016) and were compared with 2012 -September 2014). Result: There were 7977 deliveries during the 19-month period from 01 October 2014 to 30 April 2016. The average total Caesarean section rate was 23.5% and the emergency Caesarean section rate for suspected fetal compromise (“fetal distress”) was 13.6%, an observed increase in the rate of spontaneous vaginal births from 63% to 71%. There was a statistically significant reduction in the intrapartum emergency caesarean sections performed for “fetal distress” immediately after the introduction of STAN following training on Physiological Interpretation of CTG from 5.57% to 2.1%. The rate of hypoxic ischaemic encephalopathy (HIE) due to CTG misinterpretation was reduced by 50% following the training on Physiological Interpretation of CTG.
