This study aimed to compare plasma lipid profiles and apolipoprotein levels between children with DS and non-DS controls to understand their cardiovascular implications. A case-control study was conducted with 69 children (31 with DS, 38 without DS), aged 1-15 years, at the Paediatric Endocrinology Unit of University College Hospital, Ibadan. We used enzymatic spectrophotometry to measure lipid parameters such as total cholesterol, triglycerides, high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C), and ELISA to measure apolipoproteins (ApoA1 and ApoB). Data analysis was performed using SPSS version 20, with significance set at p <0.05. Children with DS exhibited significantly higher triglyceride levels (183 mg/dL vs. 171 mg/dL, p = 0.028) and HDL-C levels (42.3 mg/dL vs. 31 mg/dL, p <0.001) compared to controls. LDL-C levels were significantly lower in DS children (76.6 mg/dL vs. 93.7 mg/dL, p = 0.036). No significant differences were found in total cholesterol, ApoA1, ApoB, or the ApoB/ApoA1 ratio between the groups. Children with Down syndrome (DS) have unique lipid profiles, with elevated triglycerides and HDL-C but lower LDL-C, potentially impacting long-term cardiovascular risk. Gene therapy medicines are classified as advanced therapy medicinal products (ATMPs). The use of ATMPs in cancer is a new era in medicine, which requires the support from specialist and experienced staff and the regulatory authorities to introduce gene therapy medicines safely and effectively into clinical practice. \u0000In Europe, the European Medicines Agency (EMA) has approved a Gene therapy Medicinal Product (GTMP), Talimogene Laherperepvec (Imlygic) for melanoma, a Cell Therapy Medicines Product (CTMP), Zalmoxis for patients with haematopoietic Stem Cell Transplantation (SCT) and two Chimeric antigen receptor T-cell therapies (CAR-T cells), Tisagenlecleucel (Kymriah) and Axicabtagene ciloleucel (Yescarta), for the treatment of acute lymphoblastic leukaemia (ALL) and diffuse large B-cell lymphoma (DLBCL). \u0000Regulatory authorities, expert members in this field, and experience from clinical trials have come together to build guidance and provide advice on how to initiate these novel therapies in clinical practice. Risk assessments and standard operating procedures should be in place before gene therapy is introduced in healthcare centres. Gene therapy in cancer is an exciting new era, with challenges for healthcare professionals to implement, manage and monitor this new group of therapy. Clinical exposure and extensive research have provided a deeper understanding and improvement of medicines optimisation and safety. Scientific and safety challenges are being explored and answered in clinical trial settings, with safety precautions always being paramount to ensure the efficiency and the safe delivery of these therapies. While cancer screenings have helped identify BRCA1 variants in patients, there is not enough information on many of these variants to determine if they are deleterious and thus link them with disease predisposition. To infer the cancer risk of these variants of uncertain significance (VUS), many functional assays have been developed to characterize variant BRCA1 protein functions. In this review, we will discuss the assays that have been used to examine the function of variant BRCA1 proteins and how functional assays are increasing throughput from methods that test one variant at a time to multiplexed formats. Multiplex assays utilizing homology-directed repair (HDR) and saturation genome editing (SGE), among others, have been able to evaluate more thoroughly BRCA1 variants and functional regions. However, objective method of assessment of the burden of care for the elderly among caregivers is lacking. This study was aimed at validating the 12-item Zarit Burden Interview (ZBI) among caregivers of elderly persons in Nigeria in correlation with the Katz Index of Independence in Activities of Daily Living (Katz-ADL), the General Health Questionnaire (GHQ-12) and the Modified Conflicts Tactics Scale for Elder Abuse (MCTS). Eighty primary caregivers of community-dwelling elders were selected using a two-stage cluster sampling from two communities. Principal component analysis with varimax rotation was used to determine the factor structure of the ZBI. Construct validity was assessed using Spearman's correlations between the ZBI, the GHQ-12, Katz ADL and MCTS. Internal consistency and instrument reliability were examined with the Cronbach's alpha split-half correlation. Three factors with eigenvalues greater than 1.0 were extracted on the exploratory factor analysis comprising all 12 items accounting for 68.0% of the total item variance. The reliability analysis gave a Cronbach's α of 0.90 and a split-half correlation coefficient of 0.84. Spearman's correlation (rs) showed good correlation between the ZBI scores and GHQ-12 (rs=0.44, p<0.001), MCTS (rs=0.43, p<0.001) and Katz ADL (rs= -0.50, p <0.001). The 12-item ZBI is a valid and reliable tool for assessing burden in informal settings among caregivers of elderly persons. This study was designed to determine the adolescents' knowledge of HIV, intention to engage in risky sexual behaviour and sexual practices. A 3-stage sampling technique was used to select 240 adolescents in senior secondary schools in Ibadan, Nigeria. Data were obtained using a pretested, semi-structured, self-administered questionnaire. Data were analysed using descriptive statistics and Chi-square test at 5% level of significance. The mean age of the respondents was 16.3±1.4 years and 126(52.5%) were females. Only 34.3% had good knowledge of HIV and there were misconceptions about its transmission. Most of the respondents (95.0%) had intention to engage in risky sexual behaviour and those with good knowledge of HIV were willing to engage in risky sexual behavour with familiar people. Good knowledge was associated with intention to engage in low risk sexual behaviour. Seventy-nine (32.9%) were sexually active and they had better knowledge of HIV. Knowledge of HIV was low among this study group and was not associated with their intention to engage in risky sexual behaviour. Two or more plants are usually combined for such use and these combinations cause interactions that may be safe but could also be detrimental to health. Used singly, <i>Andrographis paniculata</i> and <i>Aspilia africana</i> are considered safe plants with low toxicities but little is known about their safety in combined therapy. This study assessed the cytotoxic activities of the aqueous, methanol and chloroform extracts of the two plants in a 1:1 combination at various concentrations on murine hepatocytes, thymocytes and splenocytes. The murine cells were seeded in microtitre plates and tested with the combined extracts at different concentrations and percentage viability values of the cells determined by the tetrazolium salt reduction assay (MTT). The extracts used were: combined aqueous extract of the two plants (CAE), and combined chloroform extract (CCE). At 72hr incubation with 500 μg/ml extract concentration, percentage viability was low. Splenocytes thymocytes and hepatocytes incubated with CAE had viability values of 35%, 28% and 64% respectively and with CCE, 26%, 26% and 36% respectively, relative to controls. From the results, the extracts were cytotoxic to the murine cells at this concentration and incubation period. These low values indicate cytotoxic interactions in the combined extracts. The evaluation of care for diabetes from patients' perspective an important indicator of measuring quality of health services and improving treatment approaches but data of the nature are scarce in Nigeria. The study therefore investigated Patients' Evaluation of the Quality of Diabetes care (PEQD) in Ibadan. This descriptive cross-sectional study recruited 384 diabetic patients from three hospitals where specialised services are offered in Ibadan. A semi-structured pre-tested questionnaire was used to collect data. A 42-point PEQD questionnaire scale was used to collect data and a score of >21 was rated as good Perceived Quality of Care (PQC). Data were analysed using descriptive statistics, chi-square test and logistic regression set at 5% level of statistical significance. Mean age of respondents was 62.5 + 10.8 years and mean diabetes duration was 8.4 + 7.6. Majority (58.3%) of the respondents didn't know the type of diabetes they had. Patients aged less than 60 years were less likely to PQC received as good (OR: 0.21; CI: 0.05 - 0.91) compared to those who were above 60 years. More than half (55.0%) of the respondents perceive quality of care as good. Patients' assessment of the quality of diabetes care received was perceived good. Need assessment for health education service provision on maternal depression therefore becomes necessary. Hence, this study assessed the needs of Primary Health Care (PHC) workers regarding the provision of routine health talk on maternal depression. This study was descriptive in design and it adopted a mixed method. Four Key Informant Interviews (KII) and a survey which involved 100 Health Workers (HWs) were carried out. KII participants were purposively selected from the 4 LGAs where volunteers were found and the 100 HWs were conveniently selected from all the Comprehensive Clinics (CCs) in the 5 LGAs. A KII guide and structured questionnaire were used to collect data and data were analyzed thematically, with descriptive statistics and chi-square. The mean age of the key informant's interviewees was 54±6.1years and the mean age of the survey respondents was 42±5.2 years. The interviewees identified HWs' inability to communicate maternal depression to the service users in appropriate local terms and non-availability of Information, Education and Communication (IEC) materials as barriers to giving health talk on maternal depression. Survey respondents identified certain needs with a statistical significance across cadres. Eighty-three (88.3%) identified training need, education aid materials 61.7%; p<0.05, maternal depression health education guide 27.7%; p<0.05 and need for more staff 22.3%. However, many pregnant women and healthcare providers are either unaware of this or accord this less attention. The study was designed to describe pattern of oral health among a cohort of pregnant women. Seventy-seven pregnant women attending antenatal care at a secondary healthcare facility were evaluated in third trimester following written informed consent. Each participant had a structured data collection form administered and clinical examination conducted. Information on socio-demographic characteristics, dental and obstetric history and examination were obtained. Dental assessment involved the use of the oral hygiene, gingival, periodontal and caries indices. The data collected was entered and analysed using IBM SPSS Statistics version 20. The mean age of the participants was 29.90 (± 4.38) years and all women were in the third trimester of pregnancy. Only 6.5% of the participants had dental complaints, 26% indicated brushing twice a day and 96.1% never had dental health check in the past. A good oral hygiene score was found in 40% of participants; 6.5% and 10.4% had a healthy periodontal and gingival status respectively and only 8 (10.4%) had caries. Less than half of the pregnant women had good oral health status. {"title":"The Factor Structure of the Adapted WHO Quality of Life BREF questionnaire in a sample of adolescents in Nigeria.","authors":"Onoja Matthew Akpa,&nbsp;Kayode Raphael Fowobaje","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Introduction: </strong>The World Health Organization's Quality of Life Instrument (WHOQOL-BREF) is widely validated and popularly used in assessing perceived quality of life (QOL) of adolescents and the general population. Though the WHOQOL-BREF has been used in some studies in Nigeria, its theoretical structure has not been comprehensively investigated. This study examined the factor structure of the Adopted WHOQOL-BREF questionnaire and it theoretical structure in a large sample of adolescents in Nigeria.</p><p><strong>Methods: </strong>Data on demographic characteristics and QOL were extracted from 1,963 adolescents who participated in a state-wide study on psychosocial functioning and quality of life of adolescents in Benue State, Nigeria. Descriptive statistics were used to present the distribution of the data while Cronbach's alpha and Polychoric ordinal alpha were used to describe the internal consistency (reliability) of the adapted WHOQOL-BREF and alpha value of 0.700 was considered reliable. Structural analysis was performed to extract the underlying factors while confirmatory factor analyses were used to assess some hypothesized structure of the adapted WHO-QOL BREF. Relative Chi-square test (<i>χ</i><sup>2</sup>/<i>df</i>) value ≤3.0 was regarded a good fit while multiple fit indexes with values ≥0.90 (for acceptable fit) were used for assessing diverse aspects of the models. All analyses were performed at 5% significance level using IBM SPSS statistics version 20, R package and AMOS version 21.</p><p><strong>Results: </strong>Participants were mostly male (54.8%) and 14.7±1.4 years old with 51.0% residing in rural areas. The overall internal consistency of the 4-factor model was 0.862 (for Cronbach's Alpha) and 0.989 (for Polychoric Alpha) while the 2-factor model had 0.870 (for Cronbach's Alpha) and 0.990 (for Polychoric Alpha). The Cattelle's Scree plot, Horn's parallel analysis and the confirmatory factor analysis revealed a 2-factor model as the best model for the WHO-QOL BREF. The 23-item 2-factor structure had a relative Chi-square test value <i>χ</i><sup>2</sup>/<i>df</i> < 3 = 2.98, p < 0.001 with all fit indices within the acceptable range.</p><p><strong>Conclusion: </strong>The adapted WHO QOL BREF can be safely used to assess quality of life among Adolescents in Nigeria and related settings.
