The mortality rate is approximately 50% even with rapid revascularization, optimal medical care, and use of mechanical support. Aim of the Work : To investigate the outcome of primary percutaneous coronary intervention (PCI) in patients admitted with cardiogenic shock and ST-segment elevation myocardial infarction (STEMI) and the predictors of in-hospital mortality. Patients and Methods : This prospective, observational study was conducted in the national heart Institute, Alazhar University, and Military hospitals in the period from 6/2019 to 9/2021 on fifty six consecutive patients presenting to Alazhar University hospitals, National Heart Institute (NHI), Military hospitals. Results : The prevalence of dyslipidemia and diabetes mellitus were significantly higher among died patients than those who survived. The degree of LV impairment was significantly higher among patients who died than those who survived. As regard to PCI procedure characteristics, TIMI flow post PCI (<Grade III), multi-vessel disease, and procedural failure were significantly associated with high in-hospital mortality rate. Patients who received treatment with IABP were significantly older. Conclusion : Multi-vessel coronary artery disease, TIMI flow after PCI (grade III), and ↑ CK-MB (72-hour serial measurement), were all found to be significant predictors of in-hospital mortality. The onset from chest pain to ED arrival and the door-to-balloon time were higher than that reported in the previous studies. {"title":"A Case Report on a Successful Treatment of Late Abdominal Aortic Aneurysm Rupture after Endovascular Repair","authors":"Brzezinski","doi":"10.29011/2575-7083.100169","DOIUrl":"https://doi.org/10.29011/2575-7083.100169","url":null,"abstract":"Endovascular repair of abdominal aortic aneurysm (EVAR) is currently indicated for most patients with abdominal aortic aneurysm (AAA). Complications that appear within the follow-up period may require quick endovascular or surgical intervention. Untreated type I and III endoleaks may cause aneurysmal sac enlargement and aneurysm rupture. This case report presents the technical aspects of an emergent operation for post-EVAR rupture of AAA in an elderly male patient who was admitted to the hospital due to a hypovolemic shock. The emergent tomographic scan revealed a massive type Ia endoleak and retroperitoneal hematoma. The constant decrease in the patient’s blood pressure required traditional operation. After clamping of the abdominal aorta, the proximal part of the stent graft was extracted from the infrarenal aorta and shortened by 5 cm. This maneuver allowed the interposition of a tube graft between the infrarenal aorta and remaining part of the stent graft. This technique is commonly used for the primary operation of AAA; however, some issues may arise when it is performed after stent grafting. A thin-walled stent graft is out of proportion to the thick-walled Dacron prosthesis and may easily be damaged by the surgical suture. Our case proves that it was possible to bevel the body of the stent graft despite its metal parts; moreover, a special technique of applying the sutures between the prosthesis and the stent graft stump resulted in a good anastomosis and satisfactory final outcome. One of its main locations is in the carotid artery which is implicated in 20% of ischemic strokes. This study aimed to determine the frequency of carotid atherosclerosis in a group of asymptomatic patients with at least one cardiovascular risk factor. Methodology: This was a cross-sectional study from January to June 2021 (6 months). We included consenting adults followed up at the Yaounde Central Hospital and the Yaounde University Teaching Hospital, aged at least 50 years and having at least one cardiovascular risk factor such as hypertension, diabetes, dyslipidemia, smoking, or obesity. A Doppler ultrasound of the supra-aortic trunks was performed on each patient. Statistical analyses were performed using the chi-2 test for bivariate analysis and binary logistic regression for multivariate analyses. The threshold of significance was set at 0.05. Results: A total of 75 participants were included of which 26 were men and 49 women. The mean age (SD) was 62.96 (8.87) years. Hypertension was seen in 93.3% of the participants, dyslipidemia in 77.3%, obesity in 48%, type 2 diabetes in 34.7%, and 9.3% were smokers. High cardiovascular risk was seen in 22 (29.4%) participants. Twenty-seven participants (36%) had one or more carotid atheroma plaques, and 11 (14.7%) participants had a stenosis plaque. Hypercholesterolemia and high cardiovascular risk were associated with the presence of carotid atheroma plaque. The prevalence of severe malnutrition in this category of children ranged between 31.5% and 42.5% in developing countries and this figure may be rising with the declining global economic and security profile. Objectives: To determine the prevalence of under-nutrition among children with un-operated congenital heart defects and the relationship between types of heart defects and nutritional status of the patients. Methods: A cross sectional study of the nutritional status of children with unoperated congenital heart diseases attending the Pediatric Cardiology Clinic of the University of Abuja Teaching Hospital, Gwagwalada, Abuja was done. Results: Of the 63 patients enrolled, 50 patients were analyzed: 29 (58%) were males. Median age was 24 months, median weight was 7.4 kg and the median length/height was 70.5cm; 75% of the weight-for-age Z-scores and 60% of the height-for-age Z-scores were below the 5th percentile. 52% had moderate-to-severe malnutrition, 54% were stunted. 38 (76%) patients had acyanotic heart defects. Total protein was lower in stunted children than in the normal height group: 65.0±7.6 g/l vs.70.2±8.9 g/l, p=0.03. Hypoproteinemia and hypoalbuminemia occurred in 10 (27.8%) and 8 (22.2%) undernourished children, respectively. Overall, serum albumin level correlated positively with patient’s weight-for-age percentile (p=0.008) and weight-for-height Z-score, p=0.026. Conclusion: Malnutrition is prevalent in children with congenital heart diseases in this community. Therefore, aggressive nutritional rehabilitation should form an integral part of the wholistic management of children awaiting surgical correction of their congenital heart diseases. There is evidence of subclinical left ventricular myocardial dysfunction in individuals with metabolic syndrome (MetS). However, the effect of MetS on the right ventricle (RV) is yet unknown. By using 2D Speckle Tracking Echocardiography, we investigated the link between MetS and right ventricle function. This study was conducted on 50 MetS patients and 25 age and gender matched individuals (control group). The MetS is diagnosed when presence of ≥3 American Heart Association/National Heart, Lung, and Blood Institute criteria. All individuals had sufficient laboratory assays as well as a thorough 2D examination including tissue Doppler imaging (TDI) and R. V global longitudinal strain (GLS) during the period between November 2019 and December 2020. The metabolic group had a statistically significant lower RV- GLS (-18.27±2.16 in MetS subjects vs. - 26.64±3.05 in control subjects, P<0.001), a significantly lower E/A ratio (0.95±0.12 in MetS subjects vs. 1.14±0.15 in controls, P<0.001), and a statistically significant increase in the E/Em ratio (5.66±1.03 in MetS subjects vs. 4.24±0.46 in controls, P<0.001). Other echocardiographic or functional capacity tests revealed no significant differences between the two groups.
