{"title":"Myocardial Infarction with Non-obstructive Coronary Arteries (MINOCA): A Case Report and Review of the Literature","authors":"Jie Chen, Wen-yan Liang, Mei Wei, Mingqi Zheng, Gang Liu, Guoping Ma","doi":"10.11648/j.ccr.20210503.15","DOIUrl":"https://doi.org/10.11648/j.ccr.20210503.15","url":null,"abstract":": Background : About 5-10% of all myocardial infarction patients undergoing diagnostic coronary angiography for typical chest pain have no significant coronary stenosis (stenosis severity <50%). We classified these patients as myocardial infarction (MI) with non-obstructive coronary arteries (MINOCA), which is characterized by both the diagnostic criteria of myocardial infarction and no significant coronary stenosis (stenosis severity <50%). The pathophysiology of MINOCA is multifactorial. Potential underlying mechanisms of MINOCA include coronary causes such as coronary spasm, coronary microvascular dysfunction, plaque disruption, spontaneous coronary thrombosis/emboli, and coronary dissection; myocardial disorders, including myocarditis, takotsubo cardiomyopathy, and other cardiomyopathies; and noncardiac causes, for example, pulmonary embolism. Early secondary prophylaxis with drugs including statins and renin-angiotensin-aldosterone system inhibitors may be beneficial to prognosis. Objective : To report a case of MINOCA, analyze its clinical manifestations, imaging and related treatment, and improve the understanding of clinicians on MINOCA. Methods : A case of MINOCA diagnosed in the First Hospital of Hebei Medical University on June 8, 2021 was retrospectively analyzed. Clinical data related to clinical manifestations, examination results, diagnosis and treatment process were collected, and literatures related to non-obstructive myocardial infarction were reviewed to summarize its diagnosis and treatment characteristics. Results : The patient was a 73-year-old male, who was admitted to hospital for 8 hours due to sudden precardiac pain. After admission, relevant preoperative examination was completed, and preoperative electrocardiogram (ECG) and troponin examination results were considered as acute inferior wall myocardial infarction, percutaneous coronary angiography showed no significant narrowing of the coronary vessels and was diagnosed as MINOCA. The patient regularly applied drugs outside the hospital and was generally in good condition. He did not go to the hospital again. Conclusion : 1. MINOCA accounts for 5-10% of all MI patients, and a higher proportion of MINOCA patients are female. 2. It was previously believed that MINOCA patients had a good prognosis, but the incidence of major adverse cardiovascular events was still high in clinical work. The pathogenesis of MINOCA is complex and diverse, and early drug intervention and secondary prevention targeting the etiology can improve the prognosis of patients. 3. Aim: To determine the functional class of dyspnea in patients with MS and to investigate the relationship between Left Atrial (LA) deformation as measured by two-dimensional Speckle Tracking Echocardiography (STE)-derived LA strain and heart failure symptoms. Patients and methods: This was a prospective study carried out on 25 patients referred for evaluation of MS using echocardiography and 25 control subjects, who were evaluated using 2D and color Doppler echocardiography, including STE-derived LA strain (LAS) and peak positive filling strain rate (LASRr) during the LA reservoir phase. Those results were then correlated with New York Heart Association (NYHA) functional class. Results: There was a statistically significant difference between the patients and the control groups in the LAS in the apical 4 chamber view (LAS A4) (64.240±15.271, 40.360±21.785, respectively; p=0.001), LASRr A4 (2.504±0.358, 1.740±0.716, respectively; p=0.001) and LASRc A4 (-2.796±0.226, -1.820±0.928, respectively; p=0.001). also, there was a highly significant relation between NYHA class and LAS A4, LASRr A4 and Peak Negative Conduit Strain Rate (LASRc A4), with P<0.001 with all data. Conclusion: This study showed that all left atrial (LA) reservoir, conduit and contractile functions showed prominent impairment in mitral stenosis patients. Speckle tracking strain and strain rate were useful in pointing out the different effects of MS on left atrial functions. The assessment of LA dimension and function can provide further insights in patients with MS, LA dimension and volume in patients with MS had a positive significant correlation with NYHA classes (heart failure symptoms). In stable patients (NYHA classes I & II) with MS, NYHA functional class independently correlated with LAS and LASRr as LAS and LASRr values of stable patients and control aspects were normal. The aim of our study was to determine the prevalence of hypertension and prehypertension in high school students from the capital city of Gabon and to identify their associated factors. Methods : This was a cross-sectional and analytical study carried out on a population of students enrolled in two establishments in Libreville during the 2018-2019 year school. Information on hypertension family history, eating habits, alcohol and/or tobacco consumption, oral contraception, age, gender, body mass index and blood pressure were collected. The National High Blood Pressure and European Cardiology Society 2013 classifications were used for the analysis of blood pressure in children and young adults respectively. Results : A total of 613 students with mean age of 20.3 ± 2.5 years were included. Sex ratio was 2.3. Prevalence of hypertension was 19.4% and the following factors were found to be independently associated with hypertension: overweight (aOR: 2.77 [1.29-9.99]) and male gender (aOR: 2.10, [1.22-3.62]) or not independently: overweight (OR: 2.52, [1.30-4.89] and obesity (OR: 2.62, [1.30-5.27]) (p<00.1). The prevalence of prehypertension was 23.0%, obesity (aOR:3,93 [1.02-15.2]) was independently associated with prehypertension, while overweight (OR: 2.16, [1.03-4.53]) and male sex (OR: 1.72, [1.10-2.78]) (p<00.1) were dependent factors. Conclusions : Hypertension and prehypertension are not unfrequent in school students from Libreville. Objective: to determine the risk factors and predictors of adverse cardiovascular events in patients with coronary artery ectasia without obstructive CAD using metabolic syndrome risk score and Framingham risk score. Methods: The study was conducted on 187 patients, 36 patients with angiographic evidence of isolated coronary artery ectasia and 151 controls. All patients were subjected to detailed history, physical examination, laboratory investigations, electrocardiography, conventional echocardiography, diagnostic coronary angiography and follow up by metabolic, Framingham risk scores and TIMI flow. Results: We used M$S and FRS to stratify the risk of all the patients. However, none of them showed any significant difference between the 2 groups. However, in correlating between these different scores and the different ectatic segments in number and sizes; only TIMI flow had the significant correlation (P <0.001). TIMI flow was the main significant variable when correlating to the incidence of cardiovascular events (P <0.001). Aims: This study aims at evaluating the value of speckle tracking echocardiography in predicting LVR after successful PCI in AMI patients. Materials and Methods: Eighty-four acute myocardial infarction patients. A thorough physical examination, electrocardiography (ECG) and a complete echocardiographic assessment, including speckle tracking study, was performed two days after PCI and then a follow up echocardiography with speckle tracking study was done two months afterwards. Patients were then divided into two groups based on the presence of remodelling (R+, R-). RESULTS: at baseline study global longitudinal strain (GLS) (-11.14±0.5 VS -16.78±0.4, P<0.0001), longitudinal strain rate (LSr) (-1.01±0.05 VS -1.07±0.04, P<0.0001), culprit longitudinal strain (CulLS) (-9.74±0.59 VS -15.68±0.49, P<0.0001), culprit longitudinal strain rate (CulLSr) (-0.95±0.05 VS -1.02±0.04, P<0.0001) were all lower in group R+ than in R-. In the follow up study, all strain parameters studied were significantly lower in the R+ group than R- group. The most sensitive and specific parameters were the GLS and CulLS (sensitivities of 91.7% and 95.8% respectively) and (specificities of 95% and 96.7% respectively). An appropriate use criteria (AUC) in echocardiography is essential to improve clinical outcomes while preserving hospital and patient resources. METHODS We reviewed the echocardiographic requests of patients referred for transthoracic 2d-echocardiography. Echocardiography indication were documented mainly using the echocardiography order/request form. The echocardiography indications were adjudicated by at least two investigators. If there was more than one indication that fit into the AUC 2011, the most appropriate one was retained. RESULTS A total of 1006 echocardiographic requests were included in this study. The patients were predominantly males (68.2%) and with a mean age of 56.27 ± 16.89. majority of the referrals came from the medicine wards (37.3%). The top three cardiac diagnosis are: Heart Failure (25.9%), Acute Coronary Syndrome (20.7%) and Hypertension (15.5%). Of the top 15 most common indications, 11 were appropriate while 2 were maybe appropriate, and 2 were rarely appropriate. The three most common specific indication is the evaluation for heart failure (21.4%), the search for a cardiac etiology for a particular symptom (14.3%) and the initial evaluation of a suspected hypertensive heart disease (9.9%). The most common rarely appropriate indication is the routine perioperative evaluation of ventricular function (5.5%). Majority of the echocardiography requests have appropriate indications (87.9%). CONCLUSION This is the first application of the AUC to the transthoracic 2d-echo here in the Philippines.
