Seydou","doi":"10.19070/2332-2780-19000113","DOIUrl":"https://doi.org/10.19070/2332-2780-19000113","url":null,"abstract":"Background: The aim is to investigate the feasibility and advantages of intratracheal topical anesthesia with self-made tracheal topical anesthesia tube combined with isoflurane (ISO) inhalation in the intubation under unconscious and spontaneous breath in patients with giant goiter. Methods: A total of forty-four patients undergoing general anesthesia during elective thyroidectomy were enrolled in our hospital. The patients with ASA grade I~II, aged 21~65 years, were randomly divided into group A (the self-made tube topical anesthesia group) and group B (those with the traditional topical anesthesia group). Each group was 22 cases. The SBP, DBP, MAP, HR, and SpO2 data and the indexes of evaluation during induction and postoperative intubation time, tidal volume (VT) during ISO induction, respiratory rate during ISO induction, level 4 score of cough during ISO induction, glottal score during intubation, score of mandibular slackness, score of difficulty in placement of laryngoscope, level 4 score of assessment of airway patency during ISO induction, level 5 score of tracheal intubation comfortability, level 3 score after endotracheal intubation, the different complications in the second day after surgery, postoperative memory sore and postoperative throat pain score were recorded. Results: In the comparison of vital signs between the 2 groups: SBP, DBP, MAP and HR of group B were significantly higher than those of group A (P<0.05). In the comparison of vital signs in the same groups between before and after operation: SBP, DBP, MAP and HR of T2 in group A and group B were higher than those at T1 (P<0.05), and DBP and MAP in group A were higher than those at T3 (P<0.05). The SBP, DBP, MAP and HR in group B were higher than those at T3 (P<0.05). The SBP, DBP, MAP and HR of group B were higher than those at T5 (P<0.05). The hemodynamics during induction and intubation in group A are more stable than in group B. In comparison with the group B, the intubation time of group A was shorter (P<0.05), the tidal volume (VT) during ISO induction was larger (P<0.05), postoperative throat pain score was lower (P<0.05). Conclusions: The self-made tube topical anesthesia is more complete than that on the traditional glottic and upper glottic area. This technique benefits survival and quality of life. Long-term outcomes in pediatric transplantation have significantly improved over the past 20 years; however, children less than 5 years of age weighing 20 kg or less still remain a considerable challenge, with higher mortality rate and graft loss. Methods: In this article, we present the pediatric RT experience at Hospital Universitario de Cruces, the main center for transplants in Spain. Children who underwent RT within the period of January 2012 January 2017 were retrospectively reviewed to identify those with weight less than 20 Kg. The following parameters were collected: pre-transplant characteristics, surgical technique, anesthesia characteristics, intra-operative and post-operative surgical or medical complications, pre and post-transplant creatinine levels, renal graft survival, and late post-operative complications. Results: Within a period of 5 years, a total of 13 pediatric patients (weight ≤ 20kg) underwent RT at Hospital Universitario de Cruces. The patient sample represented 37.14 % of the 35 renal transplants performed on pediatric patients at this institution. All 13 patients received a standard surgical and anesthetic perioperative management. Post-transplant creatinine levels significantly decrease after surgery during the early postoperative period from 6.45 mg/dl preoperatively (range, 1.90 12.26) to 0.59 mg/dl postoperatively (range, 0.27 1.27). The mean follow-up period was 1.5 year (range, 1 3) with 12 patients out of 13 (92.31%) presenting with 1-year graft survival. Conclusions: A multidisciplinary collaboration, including surgeons, nephrologists, and anesthesiologists specialized in handling transplants for underweight children should be a priority. In Anesthesia OPD, preanesthesia evaluation is done to assess patient’s medical condition before surgery and formulate anesthetic plan based on risk assessment and streamline the preoperative experience of patients. But the patient experience and satisfaction with such clinics has not been adequately studied in Bangladesh. Objective: The main objective of this study is to assess the satisfaction of patients who have availed services of preanesthesia evaluation, identify problems, and suggest measures to improve the services. Materials and Methods: We conducted this study at Anesthesia Out Patient Department by means of a questionnaire designed to assess patient satisfaction about preanesthetic evaluation services in Shaheed Ziaur Rahman Medical College Hospital, Bogura, Bangladesh. A total of 100 adult patients were randomly selected to respond to a multiple item questionnaire. A database was created and analyzed using Microsoft Excel. Results: In this study, major reasons leading to dissatisfaction of patients were difficulty in locating Anesthesia OPD, delayed fitness, lack of information about purpose and prerequisites of Anesthesia visit by surgical faculty and lack of explanation of preanesthesia instructions. Also 40 % patients found that their privacy and confidentiality not maintained. Most of the patients were not satisfied by basic amenities in hospital. Case report : We are discussing anaesthetic management of a patient with isolated structural TR who underwent live donor kidney transplantation. Intraoperative use of noradrenaline caused significant derangements in haemodynamics and bradycardia produced prevented accurate noninvasive blood pressure monitoring. Settings and Design: prospective, double blind randomized controlled trial. Material and Methods: Sixty children in the age group of 1 – 8 years undergoing sub umbilical surgeries were included in the study. After induction with general anaesthesia, caudal block was given with injection Ropivacaine 0.25% 1ml/kg and Clonidine 1µg/kg in group RC and injection Ropivacaine 0.25% 1 ml/kg and Ketamine 0.5 mg/kg in group RK. Results: The mean duration of analgesia was 530±44.2 mins in RC group as compared to 395±43.4 mins in RK group. The duration of analgesia was significantly prolonged in group RC with p value of 0.000. The mean pain score of RK group was higher than RC group at 6 th (3.5±0.57 vs. 3.0±0.37) and 8 th hour (3.93±0.25vs3.63±0.49) postoperatively which was statistically significant. There was no significant difference between the two groups with respect to haemodynamic parameters as only one patient had bradycardia in RC group. Right sided Thorax deformity is the most common feature of this syndrome with the absence of underlying bones and muscles making this condition similar to open chest along with ipsilateral hand anomalies such as brachydactyly, syndactyly or ectodactyly [1]. The incidence of this syndrome is 1 per 30,000-50,000 live births [2]. Patients with Poland syndrome require anesthesia during surgical procedures to correct the associated deformities or the organ systems associated with the disease process. Literature shows that these patients may be more susceptible for malignant hyperthermia due to associated musculoskeletal abnormalities under general anesthesia [3]. Moreover respiratory problems and hypoxia in the perioperative period may be related to associated paradoxical breathing, lung hypoplasia and chest wall abnormalities in otherwise asymptomatic patient at rest. A comprehensive plan is imperative keeping in mind that the surgery is extensive, the patients have other significant organ involvement and the need for neurophysiological monitoring to assess cord function and prevent neurological deficit. Meticulous prone positioning and the application of various blood- sparing techniques are an integral part of the intraoperative management. The pre-operative status and the intra-operative events could help in predicting the need for post-operative ventilatory support. The present article aims to elucidate the importance of a streamlined evaluation, monitoring, management strategy and stratification of these patients for a favorable outcome.
