{"title":"Comparison of postoperative pain relief by using intrathecal midazolam added to bupivacaine and buprenorphine","authors":"M. Inadequate pain control, apart from being inhumane, may result in increased morbidity or mortality. Aims and Objectives: To study the postoperative pain relief by intrathecal midazolam added to bupivacaine and buprenorphine. Methodology: This was a cross-sectional double-blinded comparative study carried out at our institute. The patients of age between 18 to 60 yrs. with ASA I and ASA II to be posted for surgery under spinal anaesthesia were enrolled to either Group A or B. Group A (n=45) patients received 15 mg hyperbaric bupivacaine and 75 mcg. Buprenorphine or Group B: (n=45) who received 15 mg hyperbaric bupivacaine and 75 mcg buprenorphine with additional Inj. Midazolam 2 mg (0.4 ml). The data was entered to excel sheets and analysed unpaired t-test, chi-square test calculated by SPSS 19 version software. Result: We observed that the demographic data was comparable. The duration of post-operative analgesia was significantly higher in Group B as compared to Group A i.e.11.18 ± 2.31 hrs. And 8.67± 3.27 hrs respectively. The VAS score at 10 hours post operatively was significantly lesser in the Group B as compared to Group A i.e. 4.23±1.94 and 6.93± 2.19 (t=6.19, df=88, p<0.001) respectively. The anti-epileptic drugs such as gabapentin and other non-steroidal anti-inflammatory drugs (NSAID) such as celecoxib were used as a part of multimodal analgesia to control such pain. Aim of the study; The aim of this randomized double-blinded study was to asses and compare the efficacy of using gabapentin versus celecoxib as a part of multi modal analgesia in perioperative hemodynamic control and pain relief in patients underwent posterior approach lumbar spine disc surgery. Methods: A prospective, comparative blinded randomized study was carried out on one hundred patients of both gender, ASA I and II, aged between 21-60 years old scheduled to undergo elective posterior approach lumbar spine disc surgery. Patients were randomly assigned into two groups (50 patients each); Group (G) received gabapentin 300 mg capsule 2 hours preoperative and the same dose 6 hours postoperative and Group (C) received celecoxib 200 mg 2 hours preoperative and the same dose 6 hours postoperative. Intra operative hemodynamics and post-operative following parameters were recorded; mean arterial pressure, heart rate, respiratory rate, Visual Analogue score and Patient satisfaction score: at the end of postoperative period. Results: No significant difference was found in demographic data between both groups. Gabapentin administration associated with a better control of intra and post-operative mean arterial pressure and heart rate P values were (p<0.016) (p<0.018) respectively when compared to celecoxib. Regarding VAS scores in both groups there was decrease but more significant in Group (G) especially in first 8 hours (p<0.001). the same as satisfaction score. It is challenging to the anaesthesiologist because of the presence of huge swelling over the posterior aspect of the neck resulting in inadequate extension of the neck and inability to lay down the neonate in supine position. The authors report a case of a 18 day old neonate weighing 2.4kg with a huge occipital meningoencephalocele (80x76mm), scheduled for excision. There were no signs of meningeal irritation, convulsion, no other signs of raised intracranial pressure and no neurological deficit. CVS and RS examinations were normal. The airway management was challenging as expected, hence before intubation pre oxygenation was done for 5 minutes in lateral position using a donut ring to support and stabilize the swelling and intubation was done in lateral position, the modified Cromack-Lenke score was found to be 2b, hence Burp manoeuvre was used to aid in intubation. Intra operatively patient was managed successfully and extubated on table. Materials and Methods: We evaluated peripheral nervous system involvement in 67 critically ill children, admitted in ICU with different types of infectious diseases (viral encephalitis, meningoencephalitis, meningitis, acute gastroenteritis). Age of the group varied from 4 months to 17 years. All patients underwent conduction studies and neurological investigation. Sensory and motor fibers of n. ulnaris et n. medianus, motor fibers of n. Tibialis and sensory fibers of n. Suralis were tested. Lowering of the amplitudes, conduction velocity slowing and asymmetry were accounted for the motor and sensory fibers. Results: In 47 cases (n=71) diagnosis of critical illness polyneuropathy (CIP) was established. Lesions mostly involved lower limbs nerves. According to our data, severe course of CIP was seen in 40% of all cases. Average time of CIP onset in children was 5-7 days from the beginning of mechanical ventilation. Conclusions: Critical illness polyneuropathy in children with infectious diseases is a severe condition which may lead to the disability of the patients. Average time of its onset is the 5-7 days from the beginning of the mechanical ventilation in 71% of the patients. More often sensory and motor fibers of lower limbs nerves are affected. Conduction studies is a valuable tool in diagnostic process in establishing the critical illness polyneuropathy in children with infectious diseases.
