The main symptoms of ALS are muscle atrophy, fasciculations, spasticity and loss of muscle strength. Loss of muscle strength in the respiratory system eventually leads to death. Without artificial ventilation, death occurs within 2 to 4 years after the onset of symptoms. Five to ten percent of ALS patients have contracted ALS due to genetic causes, having relatives with ALS [1]. Mutations in SOD1 (superoxide dismutase 1) and several other genes have been identified as genetic causes of ALS. The remaining 90% of patients are classified as sporadic ALS patients [2]. The cause of the disease in these cases remains unclear. At present, since no curable treatment of ALS exists, current treatment methods focus on managing symptoms. Gastrostomy is used to counter dysphagia and artificial ventilation is used to provide respiratory support.","PeriodicalId":91880,"journal":{"name":"Integrative clinical medicine","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2020-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67470206","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Maxvida™ is a commercially available balanced nutrition supplement with antioxidants, bone strengthening and hemoglobin forming nutrients. However, its benefits in patients have not yet been established. Therefore, the present study aimed to assess the efficacy and tolerability of Maxvida™ in ICU patients. Design : This was a two-week, prospective, open label, investigator-initiated study conducted between June-2016 to August-2016. Methods: Participants of either sex, aged ≥18 years, hospitalized for minimum two days, requiring enteral feeding were given two feeds of Maxvida™ (30 gm diluted in 100 mL of water) along with four standard kitchen feeds at different timings. Primary endpoint was to monitor continuous GI tolerance. Secondary endpoints were change in leukocyte count, nutrient, renal, hepatic and hematological parameters. Data was analyzed descriptively using paired t-test and p value of <0.05 was considered statistically significant. Results : Fifteen (11 males, 4 females, mean age: 46.3 ± 8.9 years) participants were enrolled. None of them reported gastric intolerance. A significant improvement in hemoglobin (p=0.03), hematocrit (p=0.003), white blood cells (p=0.0001*) levels was reported while RBC, platelet, differential leukocyte count also improved gradually. Conclusion: Maxvida™ administered with hospital kitchen feed were found to be safe and gastro-intestinally well tolerated by all the participants.","PeriodicalId":91880,"journal":{"name":"Integrative clinical medicine","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2020-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67470388","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In 2017, a 12-year old, not vaccinated boy with severe delay of neurological development suffered from varicella infection with severe deterioration of general condition and dyspnea due to a pneumonia. According to our knowledge this was the first reported SHPC patient with a vaccine preventable disease (VPD) so far. Methods: Retrospective analysis of other VPD cases. All physicians of SHPC and hospices are asked to complete to a questionnaire. Results: 34 physicians (32 SHPC and 2 hospices) answered. Overall, 25 patients with a diagnosed VPD (group 1) were reported. The most frequent VPDs were influenza (44%), varicella (16%) followed by one case of diphtheria, hepatitis A/B, measles, meningococci, pertussis, pneumococci, rotavirus, respiratory syncytial virus (RSV), and rubella. Suspicions of VPDs (group 2) were reported in another 22 cases. Influenza (56%), pertussis (14%), rotavirus (14%) followed by one case of meningococci, measles, RSV and varicella with one case each. Severe symptoms like dyspnea and deterioration of general health occurred in both groups. Main complications were admission to intensive care unit (ICU) in 21,3%, ventilation in 14,9%, death in 4,3% and infection of a health care professional in 10,6%. Physicians agreed it could be helpful to prevent VPD. Conclusion: VPDs appear to play a certain role in SHPC in Germany. They can cause distressing symptoms up to ICU treatment and death. Further investigations are warranted.","PeriodicalId":91880,"journal":{"name":"Integrative clinical medicine","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2020-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67470522","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Aerobic exercise has been widely studied in painful conditions subjects and is related for the improvement of the fragility syndrome, increasing the pain threshold/tolerance during and after its practice. For healthy subjects, aerobic exercise has already shown to prevent various diseases and strengthen the immune system, fighting infection diseases. The objective of this review was to report the modalities of aerobic exercises associated with the treatment of adjuvant pain reduction of experimental pain or tolerance) in the skeletal muscle system a systematic review of clinical studies. search the recommendation. were selected, reporting results related to pain modulation through aerobic exercise. A total of 20,790 studies from which we evaluated 110 records and, finally, we included 7 studies in the quantitative analysis, following the eligibility criteria. The results revealed that healthy men and women (about 97 patients) increased the pain threshold /tolerance through aerobic exercise. Protocols with different stimulations were evaluated. With low quality of evidence, it is indicated that aerobic exercise increases threshold / tolerance of peripheral pain in healthy people.","PeriodicalId":91880,"journal":{"name":"Integrative clinical medicine","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2020-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67470736","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Expiratory Flow Accelerator (EFA) technique removes secretions noninvasively by accelerating expiratory flow. The aim of our study was to compare EFA efficacy with the non-oscillatory positive expiratory pressure (PEP) bottle in airways clearing in COPD with hypersecretion and reduced cough efficiency after severe exacerbation. Methods: In an exploratory prospective, randomized, open-label study, we analyzed COPD patients with severe airflow obstruction (forced expiratory volume at 1st second-FEV1<50%), mucus hypersecretion (sputum volume >30 ml/die) and reduced cough efficiency (peak cough expiratory flow-PCEF <300 l/min) referred to pulmonary rehabilitation after severe exacerbation. Comparison was made between group using EFA and group using PEP. Primary outcome was change in perceived bronchial encumbrance (visual analogue scale-VAS). Secondary outcomes were changes in peak expiratory flow (PEF), PCEF, lung volumes, arterial blood gases exchanges, maximum inspiratory and expiratory pressure (MIP and MEP), health status (COPD questionnaire score-CCQ) and impact of disease (COPD Assessment Test-CAT). Results: Twenty seven patients completed the study (14 EFA, 13 PEP). VAS, CAT, and CCQ improved in both groups (p<0.05). A greater improvement in VAS was recorded in the EFA group. Static and dynamic volumes (residual volume-RV and forced vital capacity-FVC) improve in both groups while total lung capacity (TLC) and MIP improved in the EFA group. No significant changes were recorded for arterial blood gases, PEF and PCEF. Conclusions: EFA technique can enhance airways clearing in these patients, improving lung volumes and inspiratory muscle strength. *Correspondence to: Ernesto Crisafulli, Department of Medicine, Respiratory Medicine Unit, University of Verona and Azienda Ospedaliera Universitaria Integrata of Verona, Address: P.le L.A. Because of the advance in microsurgical technique and aftercare, replantation has been one of the commonly performed microsurgical procedures in emergent settings. In our previous retrospective study with multilevel generalized linear regression model, smoking did not increase the risk of replant failure [1]. The survival of replants was not affected by underlying medical issues such as diabetes or hypertension either. However, the success rate of replantation was significantly affected by the age of the patient and the mechanism of injury. Patients older than 50 years and those with avulsion or crush injuries tended to have a higher risk of replant failure. Our results do not mean that smoking has no adverse effect on replantation; its effect might be outweighed by that of patients’ age and trauma mechanism.","PeriodicalId":91880,"journal":{"name":"Integrative clinical medicine","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2020-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67470081","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
