Design: Multicenter, retrospective, interventional, consecutive case series. Participants: 30 selected eyes of 26 patients (out of 256 eyes of 124 consecutive patients) with a mean diabetes evolution period of 18.4 ± 4.8 years SD who underwent successful vitrectomy techniques for tractional retinal detachment involving the macula, between January 2016 and December 2020. Methods: All patients underwent pars plana vitrectomy and successfully surgical resolution of tractional retinal detachment forming only one surgery group and more than one surgery group in cases of recurrent diabetic TRD. Structural, perfused and functional postoperative findings were statistical analized. Main outcome measures: The primary outcomes were to analized the perfusional, structural and functional postoperative findings of the intentionally selected eyes with a fully resolved diabetic TRD that underwent timely sophisticated pars plana vitrectomy (PPV) techniques. The secondary outcomes were correlating the longterm final postoperative BCVA with the multimodal, serial postoperative multimodal evaluation. Results: The mean difference between the pre-, 3 months and final postoperative BCVA was significant (p < 0.05). The mean duration of vision loss before surgery was 11.6 ± 2.3 weeks SD. The mean time for macular detachment resolution was 3.6 ± 1.7 weeks in the group showing pure macular tractional detachment and 1.8 ± 0.8 weeks in the group showing combined tractional and rhegmatogenous macular detachment (p < 0.05) for the eye group with one surgery only and 12.2 ± 8.1 weeks SD for the eyes with more than one surgery (p < 0.05). Mean follow-up 11.4 ± 5.7 months. Longitudinal multimodal imaging tests demonstrated abnormal superficial and deep microcirculation patterns with multiple micro-abnormalities of the foveal avascular zone and different but distinct areas of non-perfused macula on OCT angiography, as well as the presence of disorganization of the retinal inner layers (DRIL) and chronic ischemic macular edema in 82% of the eyes examined by SD-OCT. Microperimetry, chromatic macular visual field testing and mfERG testing revealed an abnormal retinal response with an unstable eccentric foveocentral fixation pattern and a profound reduction in the N1and P1-wave nV amplitudes with a prolonged P1 implicit time. Methods: A 25-year-old man was admitted to the ophthalmology department because of painless blurred vision on the right eye over two weeks. The patient worked as a soldier and the symptoms started three weeks after having a seasonal anti-flu immunization. The medical chart review and the ancillary investigation disclosed the cause of the visual impairment. Results: The patient was treated with prednisolone 100 mg intravenous. After three months the patient had also a facial palsy. Therefore, we did a cerebral Magnetic Resonance Imaging (MRI). There were no pathological findings. Conclusions: Previous case reports already suggested a possible relationship between various immunizations and APMPPE onset. Methods: One year data from June 2017 to June 2018 was taken from records of the tertiary eye care hospital in Bangladesh. Total 28 cases were included in the study. Data was calculated in percentage. Results: Most common age group was 21-40 years (57.15%) with male predominance (96.43%). Right eye was involved in 53.57%. Out of 28 FBs, 26 were iron particles. Location of the FB was retina in 53.57% cases and vitreous in 46.43% cases. Most common complication was cataract. There are however atypical presentations that involve other systems. This report aims to highlight the clinical features of a patient who presented to the eye clinic with complaints of blurry vision and binocular diplopia. Ocular examination unaided distant visual acuity was 6/6 in the right eye and 6/4 in the left eye. The near visual acuity was N5 in each eye at 33 cm. Ocular motility showed right esotropia of about 60 prism diopters due to an isolated right abducens nerve palsy and unexplained bilateral disc edema. Investigations for a suspected optic neuritis were carried out. Due to his bizarre symptoms, a nasopharyngeal swap was also taken for SARS-CoV-2 polymerase chain reaction. The COVID-19 test came out positive and the other test results were essentially normal. The patient subsequently developed rashes one week after a diagnosis of COVID-19 was made. A brain magnetic resonance imaging (MRI) performed in the fifth week showed features of small vessel ischemia. The patient was seen fortnightly during the follow up period. His vital signs, oxygen saturation, visual acuity testing, OCT and skin examination were performed and the signs and symptoms resolved in the order of their onset over a three month period without any medications for COVID-19. Porro","doi":"10.23937/2378-346x/1410111","DOIUrl":"https://doi.org/10.23937/2378-346x/1410111","url":null,"abstract":"Background: Despite general difficulty to measure visual field (VF) in Neurologically Impaired (NI) children, the timely finding of a VF impairment may contribute to finding the right diagnosis and treatment as well as to explaining their visual behaviour to parents and caregivers. We describe the Standard Conventional Perimetry (SCP) and a potential gain in time to diagnosis of a visual field defect (VFD) when using a behavioural visual field (BVF) test. Methods: NI children who underwent SCP at the University Medical Center Utrecht were retrospectively analysed and scored with the examiner-based assessment of reliability (EBAR) scoring system. The difference in time to diagnosis between the BVF and SCP tests was calculated. Results: In this cohort of 115 NI children (69 boys and 46 girls), the majority suffered from neoplasms (36%) and stroke/haemorrhage (34%). The mean age at which they were able to complete an SCP test was 8.3 years (range 4.5 17.4). 44% were tested with the Full Field Peritest (FFP), 23% with the Goldmann VF test, 29% with the Central Peritest (CP) and 4% with the Humphrey Field Analyzer. Among the FFP 44% had “good” reliability versus 22% in Goldmann tests and 17% in CP (p < 0.001). The mean age of NI children performing SCP was 8.3 years versus 4.6 years with BV F (p < 0.001). Conclusion: The Full Field Peritest is the most reliable VF test in this cohort of NI children. The diagnosis of the disease is made clinically and radiologically. Clinically characterized by periorbital edema, ptosis, propitosis, diplopia and painful eye movements. The diagnosis is made by observing the enlargement of the involved muscles in the orbital CT. The most common differential diagnosis is thyroid ophthalmopathy. In this study, we presented two patients with idiopathic orbital myositis who were referred to our clinic with complaints of eye rash, eye movement pain, bilateral visual acuity. Idiopathic orbital myositis was diagnosed and 1 mg/ kg/day oral prednisolone was started. Methods: Between 1st January 2018 and 1st December 2018 all patients referred into Loughborough Community Hospital with a suspected diagnosis of glaucoma or ocular hypertension were booked for a visual field assessment prior to their initial new patient outpatient attendance. If the visual field test at the optician was documented as being “normal”, “within normal limits”, “no abnormality detected”, or “OK” on the referral letter then the patient was included in the study population. These patients underwent a visual field assessment with a Humphrey’s visual field analyser on a 24-2 SITA standard protocol. The outcome of the visual field test was documented including patient age, reliability indices, Mean Deviation and Pattern Standard Deviation. This data was statistically analysed by one of the authors. Results: Data on visual fields of 501 consecutive patients, 1002 eyes/visual field assessments were collected. The positive predictive value of a normal visual field at the patients referring optometrist was 97% for the right eye and 98% for the left eye. From all the visual field assessments the rate of a visual field where the glaucoma hemifield test (GHT) was outside normal limits (ONL) was 16% for the right eye and 7% for the left eye. There was very little observed association between the referral diagnosis, the IOP at referral, age of patient or the sex of the patient on the likelihood of having an abnormal visual field. Conclusion: Our data suggests that, in the presence of a normal visual field assessment at the referring optometry practice, there is very little yield of positive findings when the visual field test is repeated prior to clinical assessment. Methods: Clinical SER data from adolescent emmetropic human subjects are measured and analyzed in terms of ensemble and time-series and fluctuations of the left eye , right eye , and left-right differential measurement, N = 20 subj. Results: Results include random fluctuations for left and right eyes of human subjects age 11 to 23 years. Ensemble R L differential measurement for this group is +/0.21 diopters RMS. Left-Right coupling ratio is CR = 0.88 for both the differential and average /2 control system input signals. Data from human subjects show individual fluctuations of +/0.15 D to +/0.35 D for the left and right eyes. Normal emmetropic eyes exhibit a slow and steady trend towards myopia, at the rate of -0.40 to -0.50 diopt./decade. Conclusions: Ensemble and time-series results show that 15.9% of emmetropic adolescents are expected to progress into myopia during ages 12 to 22 years. Substantial shifts of greater than +/0.4 diopters will produce a negative myopic exponential time constant response of the focal status of the eye.
