Lateefat, Dalley Abike, Odubela Tolu","doi":"10.23937/2378-346x/1410148","DOIUrl":"https://doi.org/10.23937/2378-346x/1410148","url":null,"abstract":"Aim: To report the visual and anatomic outcomes of surgery for complex rhegmatogenous retinal detachment in a tertiary eye hospital in Africa over a 5 year period. Methods: Data was obtained from case files of patients who had surgery for complex rhegmatogenous retinal detachment between January 2014 and December 2018 in Eye Foundation Hospital Lagos, Nigeria. Visual and anatomic outcomes were analyzed and logistic regression done to identify factors associated with primary anatomic success, final anatomic success and good visual outcome. Results: A total of 343 eyes of 340 patients that met the inclusion criteria were analyzed. There were 249 (73.2%) males and 91 (26.8%) females with age ranging from 7 to 87 years with a mean age of 49.43 years. Only 41 (12.9%) eyes presented within a week of onset of symptoms, 263 (76.9%) eyes had vitrectomy with silicon oil exchange, 13 (3.8%) eyes had vitrectomy with gas, 15(4.4%) eyes had scleral buckling surgery alone while 51 (14.9%) eyes had combined scleral buckle with vitrectomy. Primary anatomical success was achieved in 281 (88.7%) eyes, Final anatomical success was achieved in 288 (92.9%) eyes and good visual outcomes achieved in 208 (61.4%) eyes. Using a simple logistic regression, eyes with normal or near normal visual acuity at presentation were more likely to have good visual outcomes (p = 0.00034). When compared with macula off rhegmatogenous retinal detachment, eyes with macula on at presentation also had a higher likelihood of final anatomic success (p = 0.001). Conclusion: Despite complex pathology in eyes with rhegmatogenous retinal detachment, good anatomic and visual outcomes can be achieved after surgery. Primary anatomic success was achieved in 88.7% of eyes and 92.9% of eyes achieved final anatomic success. Good visual outcomes were achieved in 61.4% of eyes. Eyes with normal or near normal visual acuity at presentation were more likely to have good visual outcomes (p = 0.00034). {"title":"Comparative Study of the Snellen iPad App versus Traditional Light Box Snellen Chart in Distance Visual Acuity","authors":"Celis Gerald Paul","doi":"10.23937/2378-346x/1410146","DOIUrl":"https://doi.org/10.23937/2378-346x/1410146","url":null,"abstract":"This study aimed to the Snellen whether measurements of distance visual acuity using Snellen Chart displayed on iPad tablet is comparable with traditional light-box Snellen Chart among patients in Ophthalmology Outpatient Department by using Snellen iPad application (Eye Chart HD iOS application) as a visual acuity assessment tool. It utilized a cross sectional study design among 100 new patients identified through purposive sampling. Only those who met the criteria (new patients, male or female and aged 8-years-old and above) were included in the study while patients who had a visual acuity of less than 6/30 were not included. Visual acuity was assessed through two testing modalities: (1) The traditional Snellen Light-Box Chart and (2) An iPad Mini 2 with Eye Chart HD iOS application. Findings showed that results of the Snellen iPad application (Eye Chart HD iOS application) and traditional light-box Snellen Chart are comparable with each other. Confounders were also identified such as patient-related factors wherein participants may have memorized the traditional Snellen Chart prior to their eye examination, thereby invalidating the accuracy of the results from the first testing modality employed. Methods: We did a 5-year retrospective chart review among patients who presented at the Jose B. Lingad Memorial General Hospital in the Outpatient Department, In-Patient and Emergency Room under the services of Ophthalmology, General Surgery, Otorhinolaryngology–Head and Neck Surgery, Orthopedics and Emergency Medicine. Results: A chart review included one thousand nine hundred and ninety eyes. Most patients were 18 to 59-years-old (75.18%) and males (75.53%). The majority have a severity of OTS 5 (86.98%) and a high behavioral risk (83.22%). Working age group (18-59), elderly age group (60 and above), male gender, sharp injuries, and low behavioral risk had a significant odd of having an open globe type of injuries. Compared to the elderly age group (60 and above), workplace injuries and sharp injuries have a higher odd of severe damage (OTS 1). Conclusions: The majority of ocular injuries are mild (OTS 5). Methods: A 67-year-old female presented with central visual loss on left eye for more than a year. Pre-operative visual acuity of counting fingers at 2 feet. Baseline macular Optical Coherence Tomography (OCT) showed a large full thickness macular hole measuring 710 μm in diameter associated with chronic cystoid degenerative changes and posterior vitreous detachment. The patient underwent Closed Vitrectomy with Inverted Flap Technique of ILM Peeling. Intra-ocular air tamponade was done and face down position was advised for 5 days. Result: One-month post-operative showed reduction in size of the macular hole with a decrease in diameter to less than half (310 μm) of its initial measure (710 μm). Three months post-operative showed decreased diameter to 262 μm. Visual acuity improved from counting fingers at 2 feet to 3/60 to 5/60 respectively. In this study we tried to evaluate the impact of our cobinar (Conference + Webinar) about PANIS method on ophthalmologists either in-person or online. Methods: This study was designed as a quasi-experimental evaluation including a pre-/post-test investigation, conducting on 53 ophthalmologists. The participants were asked to complete a questionnaire provided by the authors including 14 questions before and after the cobinar. Results: The cobinar had a positive effect on the participants’ knowledge about the topics, however this increase (11%) was significant only among participants who didn’t attend previously. The results indicated that the enthusiasm of the participants on use of PANIS method as their first line of treatment of OSDs or periorbital lesions was increased. Conclusion: The results showed that ophthalmology cobinars have the potential to enhance the knowledge of the participants as well as changing their attitude about a certain subject.
