Aim: To describe the epidemiological, clinical, therapeutic and prognostic features of ocular trauma in Kara. Materials and methods: We conducted a retrospective and descriptive study from January 1 st , 2017, to December 31 st , 2020, a period of 4 years. Mechanical traumas were classified according to the Birmingham Eye Trauma Terminology (BETT), and prognosis was assessed according to the Ocular Trauma Score (OTS). Results: During the study period, 418 cases of ocular trauma were recorded, out of a total of 14652 consultations, corresponding to a frequency of 2.8%. The mean age was 28.5 ± 15.6 [1.92] years with a sex ratio of 2.22 (288M/130 F). In 5.5% of cases, patients consulted within 6 hours of trauma, and in 66.7% of cases, the delay of consultation time was more than 72 hours. The injury was bilateral in 11.5% of cases. Mechanical trauma accounted for 379 cases (90.7%), chemical burns 20 cases (4.8%) and thermal burns 19 cases (4.5%). Of the 379 cases of mechanical trauma, 342 cases (90.2%) were closed globe trauma, and 37 cases (9.8%) were open globe trauma. The circumstances of injury were domestic accidents with 115 cases (27.5%) followed by road accidents with 115 cases (25.1%), work accidents and brawls in equal proportion with 65 cases (15.6%), According to the OTS score, 345 cases (82.5%) had a score of 5, there were 35 cases (8.4%) with a score of 4, the score of 3 were found in 3 patients(6.2%) and the score of 2 and 1 were found in 6 patients each (1.4%). Treatment was surgical in 8.9% of cases and medical in 91.1% of cases. Conclusion: Ocular trauma is relatively common, better awareness will reduce consultation delays and improve prognosis. Rarely, growth involving the orbital apex can cause intra-orbital or intracanalicular optic nerve compression leading to permanent vision loss. Any sign of optic nerve compression is an indication for surgical resection. We report a case of compressive optic neuropathy secondary to craniofacial FD that was treated with combined endoscopic and trans-orbital resection as well as orbital wall and facial reconstruction, a novel technique for optimal management. Surgical challenges including avoiding thermal and mechanical injury to the optic nerve, reduced access to the posterior medial orbital wall, and meticulous reconstruction of the medial and inferior wall were overcome by the combined approach. Methods: This is an observational case report. The medical history of the patient was assessed. Ophthalmologic examination including slit-lamp biomicroscopy, dilated indirect ophthalmoscopy, fundus autofluorescence and optical coherence tomography (OCT) imaging, full-field electroretinography, multifocal electroretinography and corneal topography was performed and literature was reviewed. Results: Previous genetic analysis (fluorescence in situ hybridization method) had confirmed the diagnosis of William-Beuren syndrome in this patient by detection of the deletion on chromosome 7q11.23. Visual acuity of the patient was 0.7 for both eye with Snellen chart. OCT imaging showed the flattening of the foveal contour and corneal topography revealed the finding of irregular corneal surface. All other ophthalmologic tests were normal. To clarify which surgical technique is better depending on the origin and magnitude of the giant tear this study aimed to analyze the anatomic and functional outcomes. To analyze trans- and postoperative surgical complications, we used long-term final postoperative structural, optical coherence tomography (OCT) and correlated the results with the final postoperative best-corrected visual acuity (BCVA) in three different groups of eyes. Methods: A long-term, comparative, retrospective, consecutive case series on seventy-six eyes of 66 patients that were recruited and classified according to the degree of GRT-associated RRD extension as follows: group 1 (n = 42 eyes) with GRT-associated RRD extension < 180°; group 2 (n = 23 eyes) with GRT-associated RRD extension = 180°-270°; and group 3 (n = 11 eyes) with GRT-associated RRD extension > 270°. Structural and functional outcomes were compared across groups. Results: Of the 76 eyes analyzed, 63 were phakic, and 13 were pseudophakic. The mean age of the patients was 43.0 ± 13.0 years (range, 19-76 years); 36 females, and 40 males. The mean preoperative time for GRT surgery was 1.8 weeks, the mean preoperative and postoperative BCVA was 1.87 logMAR and 0.35 logMAR, respectively (p < 0.05), and the mean postoperative follow-up was 28.1 months. Five patients (6.6%) had bilateral GRT-associated RRD, 61 patients (80.3%) had a monocular condition, and 21 eyes (27.6%) had final BCVA of ≥ 20/40. Proliferative vitreoretinopathy resulted in multiple surgeries in 31.6% of the eyes. Postoperative OCT yielded abnormal retinal thickness, ellipsoid zone (EZ) disruptions, and external limiting membrane (ELM) line discontinuities in all groups, predominantly in GRTs macula off-associated RRD requiring multiple surgeries. Conclusions: Multiple structural alterations in spectral-domain OCT biomarkers were observed. Eyes that developed secondary epiretinal membrane (ERM) proliferation showed significantly improved BCVA after proliferation and the internal limiting membrane (ILM) was removed. Methods: The present study retrospectively analyzed the data pertaining to 18 patients (25 cases) ≥ 90 years of age who underwent cataract surgery at the Jikei University Daisan Hospital in 2019. The data regarding age, preoperative and postoperative visual acuity, underlying diseases, ocular comorbidities, intraoperative and postoperative complications, and living environment were extracted and assessed. Results: Among the subjects included in the current study, five had ocular comorbidities, two had intraoperative complications, and one patient had postoperative complications. Moreover, seventeen patients had underlying diseases and six patients lived alone. All the patients displayed improved visual acuity. Conclusion: Cataract surgery is beneficial in improving the QOL and ADL in elderly patients above the age of 90 years. We analyzed the postoperative incidence of myopic traction maculopathy (MTM) progression and long-term postoperative structural and functional findings after macular surgery for myopic foveoretinal detachment (FRD). Methods: A retrospective, consecutive, comparative, interventional, one-surgeon, multicenter control case series was conducted on 35 highly myopic eyes of 31 patients who underwent fovea sparing internal limiting membrane removal technique for myopic FRD between October 2016 and April 2020. Long-term postoperative SD-OCT and functional follow-up evaluations with microperimetry, chromatic campimetry and multifocal electroretinography were performed. The main outcome was the incidence of surgical success and progression of myopic FRD. Results: The mean evolution time of myopic FRD was 6.2 ± 3.5 months. The mean follow up time was 23.9 ± 12.1 months. The mean time for the myopic FRD resolution was 5 ± 2.1 weeks. Using a paired-samples permutation test, we found that surgery was associated with a significant improvement in visual acuity: BCVA (in logMAR) decreased from 0.87 ± 0.15 pre-surgery to 0.48 ± 0.52 logMAR, with P = 0.00075. Thirty-two eyes (91.4%) showed resolved myopic FRD. Three eyes (8.5%) showed progression: Two (5.7%) developed a full-thickness macular hole, and one (2.8%) developed a macular hole retinal detachment. Postoperative test results revealed retinal sensitivity abnormalities in 10-2 and 30-2 visual-field examinations in 18 of 19 eyes (94.7%) examined; 16 of 17 eyes (94.1%) tested showed an abnormal response on microperimetry with a stable foveocentral fixation pattern, and 20 of 21 eyes (95.2%) tested showed a profound reduction in N1and P1wave amplitudes. Conclusions: Results showed a high incidence of microstructural regression (91.4%) with low incidence of MTM structural progression after surgery (8.5%).
