The repair was performed in two stages using a special delivery system designed to be introduced in an antegrade manner through a median sternotomy due to a lack of iliofemoral access. At 1 year, the patient remains in good health, with his aneurysm completely excluded and decreased in size, without migration, and all branch vessels patent. The patient presented with transient neurologic symptoms, with no clinical signs or symptoms of sepsis. Multiple preoperative imaging modalities revealed critical carotid stenosis but no indication of an infection. Secondary carotid infection was discovered incidentally intraoperatively, and carotid reconstruction was completed with autogenous tissue. Down syndrome is associated with multiple congenital cardiac malformations but is rarely associated with developmental vascular anomalies. Midaortic syndrome may result in severe renovascular hypertension that requires early intervention to prevent life-threatening complications. We report a child with Down syndrome who presented with occlusion of the aorta and was treated with aortic bypass. More than 4 years after the procedure, the patient's renal function remains normal, and there is no evidence of recurrent hypertension. When severe, manifestations include rest pain, hand dysfunction, and tissue loss. Dialysis access attempts on the affected extremity are usually abandoned after a diagnosis of steal syndrome, and patients are often left catheter-dependent. Prophylactic distal revascularization with interval ligation has been described in patients at high-risk for steal syndrome. Aneurysms in relation to these variant splenic arteries are even rarer. Open surgery, laparoscopic technique, or endovascular management could be chosen. We report a patient with an anomalous splenic arteries aneurysm that was excluded with coil embolization in a minimally invasive endovascular way. This case report describes an otherwise healthy patient with a simultaneous onset of buttock ischemia combined with sciatic nerve compression caused by a small distal internal iliac artery aneurysm. Coiling of the aneurysm prevented thromboembolism recurrence but was only partially successful in reducing the symptoms of nerve compression.
