However, bypass surgery still plays an important role for a select group of patients.</p><p>The indication to perform a high-flow or low-flow bypass in cases of acute symptomatic artery occlusion is not defined. Neurologic symptoms in acute symptomatic occlusion are usually blood pressure dependent, and acute blood flow restoration may be considered. We report a renal transplant recipient who presented with <em>Pseudomonas aeruginosa</em> bacteremia and a new saccular infrarenal aortic aneurysm. A temporary extracorporeal axillofemoral bypass was placed to maintain perfusion to the transplant kidney. A contained rupture was found, and excision and in-line reconstruction were performed using a cryopreserved human aortic allograft. At 3 years, the patient remained well, with normal renal function while on oral antibiotic suppressive therapy. The “gold standard” for symptomatic ECAAs is surgical intervention. In distally located ECAAs just below the base of the skull, endovascular monotherapy may be beneficial. We present the case of a 21-year-old man with a symptomatic saccular aneurysm in the distal internal carotid artery receiving a flow-diverting stent. The stent was successfully positioned without adverse procedural events. At 6 months, computed tomography angiography revealed secondary occlusion of the aneurysm without further complications. We present a case in which dual allografts, both originating from the right iliac arterial system, were protected from warm ischemia during aortic cross-clamping by creation of a temporary renofemoral shunt between the native left renal artery and right femoral artery. After discussion of surgical management, he wished to proceed with comfort measures only. The patient had an infected bicuspid aortic valve, aortic coarctation, and a left vertebral artery arising directly from the aortic arch. Evidence of disseminated embolization affecting her posterior cerebral circulation, the left axillary, and the superior mesenteric arteries was noted. The patient had a considerably small aorta. An urgent thoracic endovascular aortic repair was performed successfully with a chimney stent to the left vertebral artery. The spur typically entraps the brachial artery and median nerve, resulting in compression-related symptoms. In advance stages, compression could lead to endothelial damage and thrombotic occlusion of brachial artery. Spur is also associated with an anomalous higher insertion of the pronator teres muscle, which could result in multilevel entrapment of the brachial artery. We report a patient with acute upper limb ischemia secondary to brachial artery compression and distal embolization from a medial supracondylar spur and anomalous attachment of the pronator teres. We present a rare case of a MAAA with a contained rupture into the inferior vena cava in the setting of phlegmasia alba dolens of the bilateral lower extremities, sepsis, and significant hemodynamic compromise.
