Although the pathophysiology behind ischemia reperfusion injury (IRI) comprises various mechanisms, oxidative stress (OS) has been recognized as a key factor. {"title":"How to improve disease management of chronically ill patients? Perception of telemetric ECG recording and a novel software application","authors":"Michael Lang, Stefan Ringbauer, M. Cepek","doi":"10.15761/crt.1000266","DOIUrl":"https://doi.org/10.15761/crt.1000266","url":null,"abstract":"Background and Objective: During the last years, mobile applications and telemetry have been discussed to become an attractive tool to support and monitor patients with chronic diseases. This study aimed to evaluate the usability and perception of both telemetry and a novel app in MS patients scheduled for an ECG due to medication adjustment or follow-up. Methods: In 211 neurological centers throughout Germany, telemetric 2-channel ECG was applied in 1082 patients with fingolimod prescriptions, who have to be subjected to ECG recording as a mandatory monitoring step. In a second approach, patients were offered to use the PatientConcept app, an application designed to ease patient-physician contact and to support patients in their daily life. Patients as well as physicians from the participating centers were surveyed to evaluate handling, acceptance and utility of both devices. Results: The majority of patients perceived telemetric ECG recording as an interesting tool and appreciated shorter waiting times. 95%, 95% and 98% of the centers believed that using telemetric ECG recording eliminates waiting times, accelerates medical reporting and provides a relief for test persons, respectively. Both, patients and physicians perceived the use of PatientConcept app as useful. Conclusion: Being both well accepted, telemetric ECG recording and the use of a mobile app might provide new concepts to improve patient care. We performed partial mandibulectomy and reconstruction via the intraoral approach with relatively acceptable clinical results in two patients with ameloblastoma. Preoperative preparations were full skull computed tomography, virtual surgical cuttings, pre-bent load-sharing titanium plates that conformed to the additive stereolithographic models, and complex computer-aided design/computer-assisted manufacturing surgical guide for intraoperative screw placement and plate fixation. Procedures were performed transorally, but two tiny stab skin incisions were made on the unilateral posterior cheek for transbuccal screw fixation on proximal segments. Bony defects due to ameloblastoma were created by load-sharing plates and autogenous iliac bone grafts plus platelet rich plasma/fibrin followed by 6-week intermaxillary fixation. Transplanted autogenous bone grafts consolidated uneventfully, occlusion remained well and stable with intact facial profile, frontal symmetry, and inconspicuous incision marks on posterior cheeks. In selected cases, we resected huge pathologic lesions of the mandible and immediate reconstruction through a relatively conservative approach, attaining less donor site morbidities, reasonable functional and esthetic outcomes, and maintenance of adequate life quality. He was diagnosed with mitral valve endocarditis and Granulicatella adiacens was isolated. He was treated with antibiotics and discharged as he refused surgery. He came back on August 2019 with progressive increase of abdominal perimeter, lower limbs edema and chest pain. On cardiac examination pericardial rub and III/VI apical holosystolic murmur were found. The transthoracic echocardiogram showed situs solitus, atrioventricular discordance, double-outlet right ventricle with transposition of great arteries, interventricular septal defect, moderate tricuspid regurgitation, vegetations in both leaflets of mitral valve and severe mitral regurgitation, subvalvular pulmonary stenosis and huge pericardial effusion. The aim of this study was to investigate the effects of simvastatin (SIM) and ω-3 polyunsaturated fatty acids (ω-3 PUFAs) on blood lipid profile and insulin resistance (IR) in patients with T2DM and definite CAN. Methods: The study involved 72 patients with T2DM and definite CAN. Patients were divided into four groups: 1st received standard hypoglycemic therapy control (n = 15); 2nd (n = 22) – in addition SIM 20 mg/q.d.; 3rd (n = 18) in addition 1 capsule/q.d. of the ω-3 PUFAs; 4th (n = 17) in addition SIM 10 mg/ q.d and 1 capsule/q.d of the ω-3 PUFAs for three months. The concentration of glucose, glycated hemoglobin A1c, immunoreactive insulin (IRI), total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), triglycerides (TG) in the blood were determined. Homeostasis model assessment IR (HOMA-IR), atherogenic coefficient (AC), TG/LDL-C, TG/TC, TG/LDL-C and TG glucose (TyG) were calculated. Results: Prescription of SIM was accompanied by a statistically significant decrease in TC, LDL-C, TG concentrations. In parallel, SIM induced a decrease of AC, TG/HDL-C, increase in HDL-C, and does not affect the IRI, HOMA-IR, TG/LDL-C, TG/TC, TC/LDL-C/HDL-C, TyG. The use of ω-3 PUFAs has contributed to a significant reduction in TG, AC, TG/LDL-C, TG/TC, TG/HDL-C, TyG index, increase in HDL-C, and was not accompanied by changes in IRI content, HOMA-IR, TC, LDL-C, and TC/LDL-C/HDL-C. The combined prescription of SIM and ω-3 PUFAs was accompanied by more pronounced, statistically significant changes in the blood lipid spectrum, as well as a decrease in the IRI and HOMA-IR. Therefore, we evaluated the efficacy and safety of 1 mg/day finasteride in 532 Japanese men who were treated for androgenetic alopecia for 10 years. We performed subjective evaluations, using questionnaires administered to patients, in addition to the objective evaluation by doctors. The efficacy was assessed using the Norwood-Hamilton scale and modified global photographic assessment score, the standardized 7-point rating score using scalp photographs. For the Norwood-Hamilton classifications, stages IIa and IIv were combined as II, IIIa and IIIv were combined as III, IVa was combined as IV, and Va was combined as V. The proportions of patients with improvement (score ≥ 5) and prevention of disease progression (score ≥ 4) were 91.5% and 99.1%, respectively. The Norwood-Hamilton classification grading improved by approximately 1 grade from 3.35 ± 1.11 to 2.55 ± 1.30 after the 10-year treatment. The groups that showed Norwood-Hamilton: I/II/III and IV/V/VI/ VII at the first visit showed statistically significant differences in the modified global photographic assessment score at the 10-year treatment subjective evaluation (6.27 ± 0.62 vs 5.52 ± 0.78, P<0.001). Furthermore, the quantitative analysis of the objective evaluation using the questionnaire was also significantly different (P<0.001). During the study period, no serious adverse reaction was recognized. Long-term (10-year) treatment with 1 mg/day finasteride in Japanese men with androgenetic alopecia showed high efficacy in subjective and objective evaluations. *Correspondence to: Akio Sato, MD, PhD.
