Presence of appendix in the inguinal hernia sac is a rare condition seen in less than 1% of all inguinal hernia cases. Usually this appendix is on right side, but its presence on the left side is extremely rare. \u0000Case Report: A 70 year male patient with scrotal swelling and inability to pass stool and flatus presented to JNMCH emergency. Upon examination and evaluation a diagnosis of intestinal obstruction secondary of obstructed inguinal hernia was made. Further, upon exploration, a healthy appendix and caecum was seen in left sac whereas right sac contained small bowel, which was unhealthy.

Upon admission, the patient was given enoxaparin for DVT prophylaxis for hypercoagulability due to COVID-19. After 3 days of initiation of enoxaparin, the patient experienced hyponatremia with trending upward levels of potassium. It was believed to be due to the furosemide and was discontinued. Despite furosemide discontinuation, the patient continued down trending of serum sodium and an uptrend of potassium occurred. After 24 hours of discontinued enoxaparin, the sodium and potassium levels plateaued. We believe this patient experienced LMWH-induced hypoaldosteronism.

The low prevalence rate of the disease and scarcity of previous literatures is the main aim for this report. \u0000Our case study is a 29-years-old lady who presented to the Radiology department for a lumbosacral spine MRI, on account of both lower limb weakness, paresthesia, unsteady gait and finally inability to walk. The symptoms have been there since she was 14 years (15 years of progressive symptoms). She also presented with multiple macules on the skin (café au lait spots). Magnetic Resonance Imaging (MRI) of the lumbosacral spine revealed bilateral spinal neurofibromas at the spinal roots of the fourth lumbar vertebra to the sacral/coccygeal vertebrae with destruction of the sacro-coccygeal bones. MRI of the brain revealed no abnormalities in the brain and optic tract. The patient fulfilled the National Institutes of Health (NIH) criteria and that of revised diagnostic criteria for Neurofibromatosis (NF1). \u0000The patient also presented with lack of pain which could be due to impairment in sensory neurons. We describe a case of spinal neurofibromatosis diagnosed by MRI in a young lady with progressive lower limb weakness, paresthesia, sensory loss, café-au- lait spots and paraplegia.

A contributing factor to DAH is anticoagulant treatment. Warfarin has been used as prophylaxis and treatment of venous thrombosis, thromboembolic complications from atrial fibrillation or cardiac valve replacement. Warfarin-induced diffuse alveolar haemorrhage is a serious medical condition. Initially, the diagnosis of DAH and for other pulmonary conditions clinical presentation and chest X-ray were enough to suggest the diagnosis. \u0000Case Report: In this report a 72-year-old female patient with a history of Coronary Artery Disease (CAD), Hypertension, and surgical procedure of Aortic Valve Replacement (AVR) and Percutaneous Transluminal Coronary Angioplasty (PTCA). She was on warfarin therapy for 5 years and was admitted to the hospital for severe cough, haemoptysis and dyspnoea. DAH was suspected clinically and confirmed by Computed Tomography (CT) chest and bronchoscopy. The patient required treatment with non-invasive ventilation and fresh frozen plasma and other supportive care. Early diagnosis and treatment are highlighted in our case and an anticoagulation reversal is required. Therefore, it is important to carefully monitor patients receiving warfarin therapy for the main symptoms of such an incident and to start treatment as soon as possible in order to reduce mortality.

There are very few reported cases of this complication in the medical literature. \u0000Case Presentation: We present an unusual case of a 64-year-old male who presented with acute peritonitis due to biliary peritonitis resulting from the spontaneous rupture of a large hydatid cyst of the liver, as seen on contrast-enhanced computed tomography, which also showed intraperitoneal fluid. During the operation, approximately 4L of infected bile was found in the peritoneal cavity, and a cystobiliary communication was observed with rupture of the anterior wall of the cyst. Treatment involved unroofing the cyst and closing the communication. The patient was free of recurrence and in good health after 6 months of follow-up. \u0000Conclusion: Although this complication is rare, it should be considered in the differential diagnosis of acute abdominal pain in patients living in endemic areas.

Her normal state weight was 84.6kg, height 1.70m2 (BMI 29.27kg/m2) which dropped to 70kg (BMI 20.26Kg/mg2) at the point she was diagnosed with type 2 diabetes mellitus (T2DM). She was counselled about dietary and lifestyle modification. She was then commenced on Metformin and she achieved a good blood glucose level control but her weight dropped further to 58kg despite adherence with diet and lifestyle. She was then changed to Sulphonylurea (Glipizide). She had a significant improvement in her weight which she had maintained with good glycaemic control. \u0000Discussion: Oral hypoglycemic agents (OHAs) are the most common initial pharmacologic treatments for type 2 diabetes and they are associated with changes in weight. Sulphonylureas (SUs) are widely used in routine clinical practice, not only in combination with metformin, but also as first-line monotherapy. Patients often gain weight due to the side effects of current therapies, particularly SU, insulin and glitazone therapies. \u0000Conclusion: It is important for clinicians to consider not only the antihyperglycemia effects of selected medications used in the treatment of diabetes, but also the effect they may have on the patient’s body weight.
