This article explores the efficacy of the medial hemisoleus flap as a valuable surgical technique for addressing these complex wounds. Two case reports are presented to illustrate the successful use of this flap in clinical practice. The medial hemisoleus flap offers consistent blood supply, improved arc of rotation, and reduced donor site morbidity, making it an excellent choice for reconstructive surgery in this anatomical region. In the first case, a 17-year-old patient with a history of smoking developed an open fracture in his left leg two years after a car accident, which led to a soft tissue defect with exposed bone in the middle third of his left leg. In the second case, a 25-year-old male involved in a traffic accident presented with a displaced open bone fracture of the middle third of the tibia and fibula with bone sequestrum. Surgical technique, advantages, patient selection criteria, and outcomes are discussed, highlighting the significance of the medial hemisoleus flap in addressing these challenging wounds.

Early diagnosis is life-saving for patients because it is an aggressive cancer. Cryptogenic organizing pneumonia is defined as a form of idiopathic interstitial pneumonia stem from pulmonary reactions against injuries. An interesting case that mimics cancer radiologically will be presented in this study.
A 51-year-old male patient admitted to the our clinic with cough and sputum complaints. No pathological finding was detected and, there was no obvious pathology on chest X-ray. A solitary pulmonary nodule of 10 mm in size was detected in the lateral aspect of the left upper lobe of the left lung on CT. Solitary pulmonary nodule was reported as malignancy border involvement in FDG-PET. The nodule was excised completely with the help of VATS. The pathology of the patient was reported as cryptogenic organizing pneumonia.
Cryptogenic organizing pneumonia mimics many lung pathologies, most notably lung cancer. According to the symptoms and signs, clinicians should definitely rule out lung cancer if there is suspicion. Because a late diagnosis or delayed treatment of lung cancer reduces the life expectancy.

We present a case report of a 21-year-old patient with a ten-year history of a subcutaneous mass on the left flank of the abdomen. The mass exhibited a hard consistency and was adherent to the deep plane. Palpation elicited pain, and magnetic resonance imaging (MRI) initially suggested a dermoid cyst in intimate contact with the external oblique muscle. The patient underwent wide excision of the mass with a 2 cm margin and subsequent closure of the resulting muscle and aponeurotic defect by direct suturing. Histopathological examination confirmed the diagnosis of a completely excised subcutaneous schwannoma. This case emphasizes the importance of considering schwannomas as a differential diagnosis for abdominal wall masses and highlights the challenges in diagnosing and managing abdominal wall schwannomas. By presenting a comprehensive review of the literature, we aim to contribute to the existing knowledge of this rare entity, facilitating early recognition and appropriate management of abdominal wall schwannomas in clinical practice.

Approach includes a careful history and examination to rule out peritonitis and removal by conservative or operative approach. Most commonly bottles and glasses found. Other objects include toothbrushes, deodorant bottles, food articles, knives, cell phones, flash lights, ornaments, etc. Reasons for insertion include autoeroticism, concealment, attention seeking behaviour, accidental, assault and to alleviate constipation. \u0000Case Report: A 41 years old male came to emergency with complaint of pain per rectum from a bottle he had inserted in his rectum 10 hours prior to presentation. Multiple attempts to remove it at home failed. On examination abdomen was soft, non-tender, non-distended. An abdomen xray showed a large bottle shaped object in pelvis. The foreign body was palpable in the rectum but due to it’s shape, size and smooth surface it was impossible to retrieve it with simple manoeuvring even with simultaneous suprapubic pressure. All relevant investigations were done and patient was prepared for surgery. Exploratory laparotomy was done under spinal anaesthesia. Lower midline incision was given and bottle was pushed from above and taken out of anal canal. A glass bottle of size 11*5 cm was retrieved. Antibiotics and painkillers started. Patient recovered well and discharged on post op day 2. \u0000Conclusion: Management of patients with rectal foreign bodies can be challenging. Even though the majority of cases can be successfully managed conservatively, the operative techniques might also be needed.

It is a benign condition, however, its diagnosis can mimic or degenerate into peritoneal leiomyosarcoma, making the diagnosis tricky. In addition, clinical manifestations can be very nonspecific. \u0000We discuss the case of a 38-year-old nulliparous patient who presented to the clinic with a 2-year history of lower abdominal swelling and a 5-month history of severe menorrhagia. Intraoperative findings showed an enlarged uterus with subserosal fibroids and several deposits on the serosa and intestines. The patient had Total hysterectomy and bilateral-salpingoophorectomy. Cefoperazone + sulbactam is a combination of two medicines: cefoperazone and sulbactam. Cefoperazone is an antibiotic that works by preventing the formation of the bacterial protective covering, which is essential for the survival of bacteria [1]. Sulbactam is a beta-lactamase inhibitor that reduces resistance and enhances the activity of cefoperazone against bacteria [2]. \u0000Pharmaceutical agents play a central role in diagnostic and therapeutic activities for patient care. However, all agents carry the risk of adverse drug effects. While most of these are clinically insignificant, some drugs may cause unacceptable toxicity that negatively impacts patient morbidity and mortality [3]. Recognizing adverse effects is important for administering appropriate drug doses, implementing preventive strategies, and discontinuing the offending agent when toxicity occurs [2]. In most cases, nephrotoxicity leads to discontinuation of the causative drug, thereby limiting treatment options. \u0000One of the biggest challenges lies in our ability to identify nephrotoxicity in its early stages. Monitoring serum creatinine and blood urea nitrogen (BUN) levels is an effective method for identifying renal diseases [4]. In this study, the authors report three cases of isolated cefoperazone-sulbactam-induced uraemia in patients without chronic kidney disease (CKD). A Naranjo assessment score of 5 was obtained for the three case scenarios, indicating a probable relationship between the patient’s uraemia and use of the suspected drug.

Reinsertion could worsen the economic burden on patient, particularly those paying out of pocket. \u0000Methods: Management was anchored on dialysis-based nitrogenous waste, antigen and cytokines clearance, in addition to antibiotic treatment that was based on sensitivity results and, local microbial susceptibility pattern, to minimize cost. \u0000Results: He was febrile (38.40C), had a non-tunnelled jugular vein catheter with a dirty, wet dressing with a greenish tinge. He had tachycardia (110/min), fine crepitaions in the lung bases and ascites. Blood and central line samples grew Pseudomonas aeuriginosa sensitive to Imipenem ++ (both), Vancomycin + (blood) and Ciprofloxacin + (central line). He had 4 haemodialysis sessions through the infected catheter, a dose of Vancomycin but none of Imipenem (on account of cost). He had a full course of intravenous Ciprofloxacin and Ceftazidime which was introduced on the basis of local microbial susceptibility pattern as it had been very effective in managing infective conditions in our CKD patients (either in mono or in combination therapy), particularly in patients unable to afford culture and sensitivity test . He had a good clinical and, microbiological recovery as a repeat culture after seven days of antibiotics treatment showed no growth. The catheter was retained for use in further dialysis sessions. \u0000Conclusion: Treatment of infected non-tunnelled dialysis catheters could be very challenging particularly in resource poor settings. The use of low cost antibiotics with positive local microbial susceptibility pattern could be very beneficial, additive and effective in treatment and in minimizing complications.
