The current standard of care in the emergency room involves an initial plain radiograph of the chest followed by a computed tomography scan, considered the gold standard for diagnosis. In this case report, we present the unique scenario of a 3-year-old child initially diagnosed with pneumonia and subsequently revealed to have necrotizing pneumonia through the utilization of lung ultrasound. This case highlights the potential efficacy of employing lung ultrasound as a primary diagnostic tool for identifying necrotizing pneumonia in the pediatric population, challenging the conventional diagnostic pathway.","PeriodicalId":90548,"journal":{"name":"International journal of medical and pharmaceutical case reports","volume":"20 6","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-07","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"138591513","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Severity of symptoms is dependent upon the site of the arterial occlusion and the extent of the collateral blood supply around the shoulder and elbow region. The most common cause of occlusion in subclavian artery is thromboembolic occlusion. Rheolytic pharmaco mechanical thrombectomy effectively removed the heavy thrombus after placing an embolic protection device in the vertebral artery. Good recanalization was achieved precluding the need of stenting.","PeriodicalId":90548,"journal":{"name":"International journal of medical and pharmaceutical case reports","volume":"50 3","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-11-17","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"139264209","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Clinical findings incorporate fringe Facial Nerve Paralysis and neuromuscular shortcoming or loss of motion of at least one appendage. This is an exceptionally uncommon condition assessed to influence 1 out of 10, 00, 000. Men (aged 40-60 years) are bound to foster the condition than ladies. Case Nuances: A 45-year-old male patient had intensely passed a-sided facial droop 4 days prior to presenting with weakness on the left half of his upper and lower appendages. He, along with his attender likewise referenced that he had recently had a 2-minute loss of cognizance, discombobulation, and vertical shifting of the right eye both when he lost his consciousness. He had low serum Vitamin D and was determined to have Millard Gubler Condition as an Infarct in the pontine locale was recognized in imaging, and was dealt with restoratively and with Non-intrusive treatment. Discussion: Millard-Gubler disorder might cause cerebellar ataxia if the paramedian pontine dead tissue region somewhat expands horizontally, influencing the centre cerebellar peduncles. Detached pontine localized necrosis might have decent progress whenever analyzed and overseen early. Discussed here is one abnormality associated with the genital tract.
 Case Description: A 19-year-old female with Fraser syndrome presented to the pediatric emergency department for abdominal distention and pain for two days, noted to have hematometra on CT guided imaging. Decision was made to proceed with total abdominal hysterectomy bilateral salpingoophorectomy.
 Conclusion: This case highlights the rarity of Fraser syndrome with associated hematometra secondary to both internal and external genital abnormalities post-menarche. Hematometra should be a differential diagnosis in patients with syndromes that are associated with genital abnormalities as failure to drain the retained blood and tissue products, can act as a foci for infection.","PeriodicalId":90548,"journal":{"name":"International journal of medical and pharmaceutical case reports","volume":"103 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-18","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135883855","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It is an autosomal recessive disease that is characterized by hypokalaemia, hypomagnesemia, metabolic alkalosis, hypocalciuric hypercalcemia and hyperaldosteronism.
 Case Report: 70-year-old female patient who was diagnosed with Gitelman syndrome with the symptoms of loss of appetite, muscle weakness, and abdominal pain. Electrolyte levels were low. After inconclusive evidence from laboratory investigation, genetic testing was performed. The detection of the SLC12A3 gene mutation encoding thiazide diuretic sensitive sodium chloride cotransporter located in DCT of kidney, confirmed the diagnosis of GS with c.1331delA variant. No pathological variant of CLCNKB for Bartter syndrome has been found. The patient was treated with parenteral normal saline, potassium chloride, Arachitol, spironolactone, metoclopramide, and MgD3 (magnesium glycinate and vitamin D3), the symptoms was significantly relieved. After discharge the intravenous medicines were shifted to oral and followed up with serum electrolyte levels in our institute.","PeriodicalId":90548,"journal":{"name":"International journal of medical and pharmaceutical case reports","volume":"30 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-10","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"136352412","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

 Presentation of Cases: One male patient aged 67 years was suffering from vomiting, hiccups, fever with chills and weakness. The post-liver transplant patient was also suffering from diabetes, hypertension, and chronic kidney disease. There was evidence of acute phase inflammatory reactions. Blood culture revealed polymyxin B sensitive Vibrio cholerae. After treatment, patient became normal and was discharged. Another male patient aged 68 years was suffering from jaundice, vomiting, abdominal pain and fever. The post-cholecystectomy patient was also suffering from COPD. LFT and P-time were grossly abnormal with 1.94x105/ml HBV DNA. CT scan of the lungs showed bronchiectasis, fibrosis and emphysema. Blood culture revealed polymyxin B-resistant Vibrio cholerae. Gradually severe metabolic acidosis and congestive cardiac failure developed and the patient expired.
 Discussion: Male dominance of Vibrio cholerae bacteraemia as observed also by others was probably due to protection in females by oestrogen; polymyxin B sensitivity, invasion of tissue from colonization and comorbidity conditions may be important factors for treatment outcome.
 Conclusion: Vibrio cholerae bacteraemia, although rare, may be present in difficult-to-treat cases.","PeriodicalId":90548,"journal":{"name":"International journal of medical and pharmaceutical case reports","volume":"15 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-09","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135141189","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

 Presentation of Reports: We present series of cases of parents and guardians whose children are less than fourteen (14) years of age and have sickle cell disease (SCD). In two of such cases the parents wished that their wards were dead to relieve them (the parents) of the challenges they go through to take care of the children. In the third report, the husband divorced the wife, blaming her for the plight of their two children; and the final one presents a couple who contemplated withdrawing their wards from school, so they would not have to spend money to educate them and lose them eventually to death.
 Discussion: The case series give clear indications of the challenges guardians with children living with SCD go through. The disease which is hereditary affects the haemoglobin of the red blood cells, making the cells sickle-shaped. The clinical hallmarks of frequent infection, bone-pain crisis, anaemia, fatigue, organ failure and many other symptoms associated with the condition lead to frequent hospitalization. This is where the frustrations of the parents come from. With proper management of the condition and education to the guardians, SCD patients can go through normal life and the guardians can also have relative peace of mind to support such children.
 Conclusion: Individuals who are carriers of the sickle cell traits should be discouraged from getting married to other carriers; and if they do, they should be cautioned about the challenges they stand to face if they decide to have children. By doing this, perpetuation of this condition and its attendant problems may be reduced. Newborn babies should be tested for the condition and proper management given from the onset to minimize crisis.","PeriodicalId":90548,"journal":{"name":"International journal of medical and pharmaceutical case reports","volume":"4 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-06","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"134943830","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

HLH is clinically characterized by uncontrolled activation of cytotoxic T lymphocytes, natural killer cells, and macrophages that can lead to a cytokine storm. Fever, hepatosplenomegaly, cytopenia, elevated liver enzymes, hypertriglyceridemia, hyperferritinemia, and hemophagocytosis in the bone marrow, spleen, or lymph nodes are the hallmarks of the disease. Its primary (genetic) form is typically observed in pediatric patients whereas its secondary, acquired form is seen in adult patients with an underlying autoimmune, malignant, or infectious disease. Here, we present a case of secondary hemophagocytic lymphohistiocytosis in a 70-year-old male with a history of chronic lymphocytic leukemia, severe prolonged COVID-19, and multiple infections who presented with persistent pyrexia of unknown origin (PUO). Diagnosis and management of HLH in adults are often extrapolated from the pediatric HLH-94 and HLH-2004 protocols. The management includes etoposide-based regimens containing corticosteroids, cyclosporine A, intravenous immunoglobulins (IVIG), intrathecal therapy, and liberal allogeneic stem cell transplant use in higher-risk patients. Due to the diverse clinical manifestations and the presence of numerous diagnostic mimics, the diagnosis of HLH is challenging. Since the prognosis is typically poor, prompt diagnosis and strong treatment are required. Here is a description of how our patient is diagnosed and treated based on the HLH 2004 protocol.","PeriodicalId":90548,"journal":{"name":"International journal of medical and pharmaceutical case reports","volume":"25 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-02","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135830014","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
