Thoracic trauma occurs in 20–25% of all trauma patients worldwide and is the third cause of trauma-related deaths. Approximately 5% of trauma admissions are liver injuries. In some blunt or penetrating injuries, both intrathoracic and intra-abdominal organs may be injured. It is aimed to present the case of a broken knife entering from the right paravertebral location, causing hemothorax as a result of diaphragm and liver injury without injuring the lungs, due to the importance of the multidisciplinary approach among surgical departments in the study. \u0000A 44-year-old male patient was admitted to the emergency department due to penetrating injury. An oblique injury of approximately 5 cm was seen in the posterior part of the right hemithorax. Radiologically, It was detected that the knife, which entered through the 10th intercostal space, was stuck inside and the knife handle was broken. It was observed that the broken blade entered from the paravertebral region, passed into the intrathoracic area, causing hemothorax, and passed from the edge of the inferior vena cava and embedded itself in the liver. Multidisciplinary operation planning was made among the departments. All department surgeons were present in the operating room at the same time. The paravertebral space was assessed and the embedded blade was slowly withdrawn by the neurosurgeon. Explorative laparotomy was done by the general surgeon and gastroenterology surgeon. Transdiaphragmatic injury was evaluated by the thoracic surgeon and major vascular structures were evaluated by the cardiovascular surgeon. In the postoperative period, the patient was discharged on the 5th postoperative day with full recovery without any complications. \u0000Penetrating trauma is one of the most fatal cases of patients admitted to emergency departments. In some cases, intrathoracic and intra-abdominal penetrating injury may occur together, rarely from a single entrance location. The timing of critical intervention varies depending on whether the patient's vitals are stable or unstable. If the hemodynamics are stable in penetrating injuries, all affected anatomical structures should be evaluated with radiological examinations. For successful, uncomplicated surgical treatments, controlled techniques, interdepartmental consensus and, when necessary, simultaneous presence of surgeons in the surgery are essential

If left unmanaged, malnutrition will have negative consequences on rehabilitation. Nutritional intervention provides caloric requirements and the nutritional support necessary to cover the daily nutritional needs and help contrast hospitalised infections. Our hypothesis is that integration of organised and continuous nutritional assessment and management can enhance prevent the state of malnutrition and increase rehabilitation outcomes. \u0000Patient Concerns: We present the case of an adult man with Poly-trauma post caused by road traffic. The patient underwent tracheostomy and percutaneous endoscopic gastrostomy (PEG) procedures, had severe consciousness disorder. \u0000Diagnosis: He has severe consciousness disorder that necessitated the tracheostomy and the PEG. \u0000Interventions: Our approach included regular nutritional assessment and caloric calculation and implementation of as well as the gradual introduction of diet from NPO to clear liquid, as well as using of supplements and enteral formulas. \u0000Outcomes: The patient was followed for seven months during which the metabolic/nutritional pattern and the blood tests improved, normal weight restored, and consciousness regained. \u0000Conclusion: Nutritional intervention integrated has prevented drastic loss in weight, a better recovery, and the stabilization of the metabolic-nutritional framework. Renal artery stenosis is the source of arterial hypertension, which is common in TA. It is estimated that there are 2.6 incidences of Takayasu's arteritis per million people worldwide each year. Case Details: We report on an 18-year-old girl whose significant arterial hypertension on her left arm, identified during a routine check-up, was the primary manifestation of Takayasu's arteritis (TA). The left arm's systolic blood pressure was noticeably greater than the right's. Additionally, there was a difference in blood pressure between the legs and left arm. Auscultation revealed the presence of vascular bruits over the left subclavian regions. A CT angiography was used to establish the diagnosis of TA. The results indicated a substantial constriction at the right renal artery's origin and characteristics compatible with aortic arteritis of the thoracic and abdominal aorta with active disease given wall enhancement. Due to the disorder's rarity and the varied ways in which it manifests clinically, a delayed diagnosis and treatment are likely. Conclusion: Our study emphasizes that this illness can and does affect young people, and as such, patients presenting with suggestive symptoms and signs—especially young patients with unexplained hypertension—should be taken into consideration. Appropriate imaging and clinical suspicion are essential for the accurate diagnosis and treatment of TA patients. Several findings were found during the clinical examination, including signs of claudication, fever symptoms, involvement of major vessels including the aorta and its branches. This illustrates the condition of Takayasu's arteritis and what medical professionals should anticipate when treating it.

Isolated male epispadias is due to failure of the urethral plate to tubularize on the dorsum of the penis. It is rare with an incidence rate of 1 in 117,000 live births in males. The severity depends on the position of the urethral opening and ranges from peno-pubic to penile and glandular. Epispadias is usually repaired within the first year of life though some patients have presented in their second decade. Epispadias is classically associated with bladder exstrophy in over 90% of the cases while isolated epispadias with continence is very rare constituting less than 10% of cases. Separation of pelvic bones is seen in 70% of peno-pubic epispadias affecting the bladder neck and external sphincter leading to incontinence and stress urinary dribbling. The goals of repair include; achieving a cosmetically acceptable and functional penis, which is straight and adequate in length, enabling penetrative sexual intercourse and urinary continence. There have been reports of isolated epispadias in adults but none was as old as 40 years. This patient has been married for 20 years and has been unable to impregnate his spouse. He had modified Cantwell-Ransley procedure after a detailed clinical and psychological evaluation to achieve the goals of repair. Adults presenting with isolated continent peno-pubic epispadias are rare. Presenting at this age and after having been married for two decades could have a derogatory effect on body image, self-confidence, psycho-sexual and reproductive life. Surgical correction of this anomaly as we did has the potential to lead to acceptable outcomes as demonstrated in this index case.
