Our study on drug substance involves Acid/ base Stress testing. In our recent research we study the effect of different environmental conditions by subjecting the different brands of pharmaceutical formulation i.e. celecoxib (Nuzib, Seleco, Celbexx) under acidic and basic conditions by using spectrophotometer. It is usually preferred over other methods because of less equipment cost and economical maintenance advantage. When celecoxib (Nuzib, Seleco, and Celbexx) subjected to 0.1 N HCl and 0.1 N NaOH, celecoxib showed decreased availability in acidic medium as well as in alkaline medium. The absorbance in basic medium of different brands of celecoxib i.e., Nuzib, Seleco and Celbexx was found to be 0.055, 0.094, 0.071 and their percent availability was found to be 50%, 42.30%, 35.14%. In acidic medium the absorbance of Nuzib, Seleco and Celebex was found to be 0.047, 0.128, 0.074 percent availability was 42.70%, 57.60%, 36.63%. We conclude that the absorbance of all brands of celecoxib decreases in acidic and basic medium which is due to the degradation of celecoxib in both mediums.

One of the complications of DM is chronic kidney disease, which accounts for nearly 44% of all new cases of kidney failure in the US in 2011. Zinc (Zn), an essential trace element, plays an important role in regulating carbohydrate metabolism. Several studies have shown the beneficial effects of Zn on renal pathological changes in Type 1 and Type 2 diabetic subjects, by reducing levels of oxidative stress, glomerular damage, and urinary albumin excretion after Zn supplementation. In contrast, other studies have shown little effects of Zn supplementation on renal damage. This paper reviewed recent research developments and found promising results of Zn in reducing, and in some cases, completely preventing renal damage. Nevertheless, the use of Zn as a potential treatment and especially its long-term impact against renal pathological symptoms in DM patients needs to be further studied.

Beyond pharmaco-therapy, overweight individuals have been seeking functional food supplements to manage their weight. Several functional foods have been suggested for the management of obesity. Particularly, certain functional foods that contain bio active compounds are believed to interfere in cell metabolism that brings changes in the metabolic parameters. The two functional foods green tea and cinnamon have been intensively studied individually on insulin sensitivity, blood glucose, body weight and lipid fractions using animal and human models. However, there has been seldom study available experimenting with both the functional ingredients (Green Tea and Cinnamon) together in over weight individuals. The inmates of St. Joseph Prasanth Nivas Ashramam, Jeppu Mangalore, India were selected as participants. A total of forty volunteers were recruited according to predetermined inclusion criteria were grouped into experimental and control groups equally. Supplementation of green tea with cinnamon twice a day for 30 days yielded a significant reduction in BMI (P<0.05) waist circumferences (P<0.05) and lipid parameters like TGL (P=0.000), HDL (P=0.000) and that of LDL (P<0.05). Thus, it is imperative that regular consumption of Green Tea with Cinnamon can modulate lipids favourably and moderately reduce body weight which on a long run may help one to prevent or decrease the risk of CVD.

Such high mortality in inpatient units has been attributed to either co-morbidity such as Human Immune Virus, Tuberculosis, diarrhea and malaria or due to poor adherence to the World Health Organization therapeutic guidelines for the management of severe acute malnutrition. \u0000Objective: To assess survival status and identify the predictors of mortality among children aged 0-59 months with severe acute malnutrition admitted to stabilization center in Sekota hospital, Waghemra zone of Amhara region. \u0000Method: A retrospective cohort was conducted on 415 children aged 0-59 months who were admitted for complicated severe acute malnutrition at Sekota hospital from January1/2011 to December 30/2013. The data collection was undertaken from March 15-25, 2014 using standardized checklist. Data were cleaned, edited and entered by Epi data version 3.1. and analysed by SPSS version 16.0. Descriptive summary of child characteristics and outcome of interests were computed by using tables, graphs and Kaplan Meier curves. After checking for assumptions Cox- proportional regression model was used to identify the potential predictors of survival status. Finally variables that had P-value < 0.25 in bivariate analysis were candidates for multivariate analysis to determine independent predictors of mortality. \u0000Results: From 441 expected samples, the data were collected on 415 children with baseline records. The most frequently 185(44.6%) associated co-morbid was diarrhoea. Independent predictors of mortality were Malaria (AHR= 2.13, 95% CI = 1.12.7.15), severe anemia (AHR = 6.71, 95% CI: 3.22, 13.97). And TB (AHR= 2.88, 95%CI = 1.72, 4.65). Other predictors of mortality of the children were: children not supplemented folic acid (AHR=2.30, 95% CI=1.54, 3.4), not supplemented for vitamin A (ARH= 1.53, 95% CI= 1.05, 2.24) and children not managed by intravenous antibiotic (AHR= 2.73, 95%CI = 1.9, 4.0). Conclusion: The overall mortality among children aged 0-59 months with complicated SAM admitted to Sekota hospital was higher than the minimum SPHERE standard for stabilization centers. The majority of death was attributed to malaria, severe anemia, TB and mismanagement of complicated severe acute malnutrition. So improving this gap may have paramount effect on child survival.

This study aimed to explore the essences of the lived experience of Public Health Midwives (PHMs) as primary care givers to the children and nutrition counselors for mothers and care givers. The methodology of the study was interpretive phenomenology described by Heidegger. There were three Focus Group Discussions (FGD) involving six PHMs in each FGD. Altogether, eighteen PHMs participated in the in-depth experiential conversations using an FGD guide. The discussions were video and audio recorded and transcribed. Thematic analysis and interpretation were carried out according to van Manen. Four major themes have emerged through the interpretation process. The themes were: (1) habit of fast food, (2) busy life of mothers and care givers and (3) lack of knowledge among mothers and care givers (4) lack of health care personnel. A sub theme ‘critical financial situation’ was also emerged under the theme busy life of mothers and care givers and another sub theme ‘interference by mother-in-laws in child nutrition’ was emerged from within the major theme, lack of knowledge among mothers and care givers. The study was quite specific to Eastern province of Sri Lanka, where the nutritional status of the children aged 6-36 months was lower than the average level during the study period. The findings of the study will be helpful to the health care personnel in the Eastern province; in planning and providing the optimum care to children aged 6-36 months and nutrition counseling for their mothers and care givers in future.

The aim of this comment is to make a brief review of the research on circadian rhythm and their participation in the development of obesity in humans. Recent findings onto the molecular, genetic, neural, and physiologic basis for the generation and organization of circadian clocks in mammals have revealed interactions between the circadian clock system and critical pathways for the regulation of metabolism and energy balance [2]. Circadian rhythmicity that has been shaped by evolution over millions of years generates an internal timing controlling the sleepwake and metabolism cycles [3]. Also, life on earth has evolved under the daily rhythm of light and dark. Consequently, most creatures experience a daily rhythm in food availability [4].

Accumulating researches suggested vitamin D deficiency is associated with obesity and dyslipidemia. But the relationship is still controversial. This study was aimed to evaluate the vitamin D status in Chinese adolescents, to find out the risk factors of hypovitaminosis D and its association with obesity. \u0000Methods: A cross-sectional survey of serum 25(OH)D, glycolipids, anthropometric measurements and life style investigation was done in 441 youths. The prevalence of hypovitaminosis D was determined. The influence factors of 25(OH)D concentration were analysed. Multi-variation logistic regression was utilized to find the risk factors of hypovitaminosis D. The association of vitamin D3 and the metabolic indice was analysed by gender. \u0000Results: The mean serum 25(OH)D concentration was 21.9 ± 8.1 ng/ml. Boys had higher 25(OH)D than girls (22.4 ± 8.1 ng/ml vs. 19.0 ng/ml (P25: 15.5 ng/ml, P75: 25.2 ng/ml), p<0.05). The prevalence of hypovitaminosis D was 42.4%. The prevalence of vitamin D deficiency in girls was significantly higher than in boys (55.4% vs. 39.8%, p<0.01). The mean concentration of 25(OH)D in winter was 19.6ng/ml, significantly lower than that in summer(25.6 ± 7.8 ng/ml, p<0.001). Significant differences were found in gender (p=0.005), age (p<0.001), BMI-SDS (p=0.003), WCSDS (p=0.005), seasons (p<0.001) and NAFLD ratios (p=0.018) among groups of vitamin D sufficiency, insufficiency and deficiency. The logistic regression suggested that female (OR=2.45, p=0.001), older ages (OR=1.29, p=0.011) and winter (OR=1.96, p=0.038) were risk factors of vitamin D deficiency. In male adolescents, HDL was protective factor of 25(OH)D concentrations (OR=0.22, p=0.009). BMI and WC were of no association with vitamin D. No relationship between 25(OH)D and metabolic indice or anthropometric measurements in girls was found. \u0000Conclusions: The vitamin D3 status in adolescents was very worrying. The risk factors of hypovitaminosis D were female, older ages and winter. 25(OH)D concentration had no relationship with BMI, WC, NAFLD in both genders.
