At early stages diabetic retinopathy may not have any symptoms but finding it in the early stages can protect from the loss of vision. Recognition of early childhood signs could prompt earlier diagnosis of NF2, improving management. We present two case reports of children with NF2; both with cortical dysplasia and one with additional with retinal folds. The cases demonstrate corticaland retinal dysplasia as early manifestations and emerging signs of NF2. {"title":"Relationship between Preoperative Corneal Thickness and Postoperative Visual Outcomes after Posterior Lamellar Corneal Transplant","authors":"E. Neiter, M. Sot, L. Lhuiller, N. Houmad, Goetz, Péron Jm","doi":"10.4172/2324-8599.1000230","DOIUrl":"https://doi.org/10.4172/2324-8599.1000230","url":null,"abstract":"Purpose: To evaluate the impact of preoperative corneal thickness on visual outcomes following Descemet’s stripping endothelial keratoplasty (DSAEK) or Descemet membrane endothelial keratoplasty (DMEK). Methods: This retrospective, observational study included patients aged > 18 years that underwent a posterior lamellar graft surgery (DSAEK or DMEK) for Fuchs dystrophy, pseudophakic endothelial decompensation or other endothelial dysfunction from October 2013 to November 2016. Exclusion criteria were the technical inability of preoperative CCT measurement, a history of penetrating keratoplasty in the study eye, the need for a penetrating keratoplasty following endothelial graft failure, preexisting stromal scarring before surgery (clinically assessed by the slit-lamp exam) and severe postoperative complications resulting in unquantifiable visual acuity. Visual acuity, central corneal thickness (CCT) and graft thickness were measured preoperatively and at postoperative day 7 and months 1, 3 and 6. Results: Forty-seven eyes (40 patients) were included in the final analysis. No significant association was observed between preoperative CCT and visual acuity at any postoperative time up to month 6 (r = 0.01; p = 0.94 at month 6). Preoperative CCT showed no association with visual acuity gain at any time up to month 6 (r = -0.02; p = 0.91 at month 6). Patients and methods: A total of 429 eyes of 429 patients were retrospectively analyzed. A low-optical coherence reflectometry device (Lenstar, Haag-Streit) was used for the calculations. SRK/T, Hoffer Q, Haigis, Holladay-2, Olsen, Hill-RBF and Barrett Universal II formulae were studied. The study included patients who underwent phacoemulsification surgery with a 2.8 mm corneal cut by a single surgeon. Refraction measurement was done at the first postoperative visit, and then spherical equivalent values were recorded for each patient. Mean absolute error (MAE) was calculated as the absolute value of postoperative spherical equivalent value minus predicted error for each formula. Results: The mean age of the patients was 67.51 ± 5.75 years. Mean axial length was 23.42 ± 2.02 mm (min: 20.03, max: 32.68, median: 23.09). Mean IOL power was 21.1 ± 5 diopters (min: -1.5, max: 3.5). Postoperative spherical values were between -1.50 and +1.00 diopters. Mean absolute errors were 0.317 for SRK/T, 0.280 for Holladay-2, 0.273 for Hoffer Q, 0.264 for Haigis, 0.347 for Barrett Universal II, 0.339 for Hill-RBF, and 0.351 for Olsen. Conclusion: Hoffer Q, Holladay-2, and Haigis formulae had significantly lower MAE values than SRK/T, Barrett Universal II, Hill-RBF, and Olsen formulae. Nonneural GCTs (nnGCTs) are morphologically indistinguishable from classic GCT but lack the necessary S100 positivity. nnGCTs are usually benign, although three cases with metastasis have been reported. The histogenesis of nnGCTs is uncertain. Here we report a case consistent with nnGCT arising in the skin of the eyelid with immunohistochemical and ultrasctructural correlation. The patient underwent biopsy of the mass via an endoscopic trans-sinus approach, only rarely discussed previously in the literature. The Pathology results obtained from the muscle showed atrophic and fibrotic muscle with mild chronic inflammatory infiltrate, compatible with the diagnosis of inflammatory process. Medial orbital wall fractures with associated incarceration of the medial rectus muscle have been reported as the most common cause of pseudo-Duane’s retraction syndrome. We report a case of a 10-year-old patient with isolated medial wall trapdoor orbital fracture with right medial rectus muscle entrapment visible on the CT scan. The patient complained of nausea and vomiting especially with abduction of the right eye and showed signs of pseudo-Duane’s syndrome.
